Trends in preventive care: caries risk assessment and indications for sealants.

BACKGROUND: In the 21st century, risk assessment models will continue to be developed. By understanding patients' susceptibility to disease, better treatment and preventive regimens can be offered. As the causative agent of dental caries is bacterial, the interaction between the susceptible host, the causative agent and the environment determine whether caries occurs--regardless of the patient's age. CLINICAL IMPLICATIONS: This article reviews risk assessment for dental caries and the implication for developing preventive strategies. It also describes the indications and uses of sealants in the prevention of dental caries.

The next generation in pit and fissure sealants.

Although the benefits of pit and fissure sealants are widely recognized, many clinicians may not routinely include their application as part of a preventive oral health care regimen. This reticence may be due to lack of knowledge regarding technique, appropriate patient populations, and additional advantages to sealant use. This article reviews the background, properties and advantages of Helioseal F (Ivoclar Vivadent, Amherst, NY), which is a resin-based, light-curing, fluoride-releasing pit and fissure sealant. The author also provides a checklist of advantageous sealant properties and discusses sealant techniques and the patient profile of sealant candidates.

Controlled-release periodontal chemotherapy: the evolution of antimicrobial delivery systems.

Bacterial plaque contributes to the development and progression of periodontal diseases. Therefore, therapeutics designed to stop or slow periodontal destruction must limit or eliminate the deleterious effects of plaque. Despite the recognized beneficial effects of in-office surgical and nonsurgical periodontal therapies and home mechanical plaque control, antimicrobials have been used locally, systematically, or in combination to limit plaque activity. This article discusses a new, localized antimicrobial delivery method. The technique discussed employs an ethylene/vinyl acetate copolymer, containing tetracycline hydrochloride. Description of this polymeric fiber, indications for its use, placement technique, patient education, the role of auxiliaries, and practice management aspects are reviewed. The future potential of sustained-release local pharmaceutical devices is addressed.

Current concepts in irrigation therapy.

The delivery of chemical agents by subgingival irrigation has become increasingly important as an adjunct to mechanical procedures such as scaling and root planing in the treatment of gingivitis and early stages of periodontal diseases. This article presents the rationale for subgingival irrigation therapy in the dental office, and outlines proper irrigation technique as well as treatment planning guidelines.
