[New minimally invasive methods of stabilizing distal femoral fractures].

New, minimally invasive methods of stabilizing distal femur fractures offer the advantage of superior bone healing rates and lower infection rates despite of a decreased necessity of bone grafts. In comparison to conventional plating, the employed indirect reduction method does not lead to an increase in postoperative malalignments. The Less Invasive Stabilizing System (LISS) and the Distal Femur Nail (DFN) are introduced as examples of these new methods of stabilization. Although bone biology is widely respected by these new implants and operation techniques, distal femur fractures remain a serious injury, often resulting in subnormal function.

[Disappearance of shame and puberty in girls].

The sense of shame seems to vanish for more than a decade now, manifested e.g. by an increasing tolerance toward the naked female body. However, it is supposed that shame has still its significance especially during female puberty. Psychological considerations referring to shame and to the puberty of girls are reported and the significance of feeling ashamed in this salient segment of female development is delineated. Finally a concept is proposed within which the feeling of shame is understood as a possibility to create accordance between the positive aspects of being ashamed and the tendency of going beyond the borders of shame usually guaranteed by social rules.

[25 years kidney replacement therapy in childhood and adolescence--success of somatic and psychosocial rehabilitation].

In the late sixties, renal replacement therapy (RRT) was started in terminal renal insufficient children and adolescents. The high mortality rate and extreme therapeutic difficulties gave doubts to the possibility of longterm survival as well as somatic and psychosocial rehabilitation in these patients. But nowadays due to improvements in medical and technical possibilities of dialysis and kidney transplantation as well as to individually adapted treatment of the metabolic problems 5-years survival rate is more than 90%, body growth and development is in the lower normal range. Successful psychosocial rehabilitation despite RRT has also improved over time. In the beginning only 29% dialysed patients and 51% transplanted children attended school and 65% completed school. A recent analysis of educational status employment rate and social situation in 617 patients between 20 and 35 year of age who started RRT as children in Europe and 276 terminal renal insufficient adolescents in Germany, gave following results: one third went to vocational training, 11-17% attended university. Thereafter 40-65% of all patients were employed. Unemployment was a big problem in dialysed adolescents and young adults. With increasing age the patients gained independence in their life style. About 20% lived in their own houses, 28% were either married, divorced or widowed, 8% had children of their own.

Adaptation, Psychological↗

Psychosocial adaptation of children and adolescents with chronic renal failure.

In a multicentre study comprising five paediatric nephrology centres in Western Germany, psychosocial and educational parameters were assessed (during 1987) in 479 children and adolescents with chronic renal failure (CRF) in order to gain insight into their psychosocial adaptation to the disease. At the time of assessment, 31% of patients were on conservative treatment, 14% on haemodialysis, 9% on continuous ambulatory peritoneal dialysis and 46% had a functioning transplant. The mean age at assessment was 13.6 years. Additional disabilities were noted in 29% of patients. School attendance of the 233 children of school age was in general satisfactory; 22% of patients attended schools for disabled or handicapped children. Vocational training was frequently inadequate, especially for dialysed patients, and only 14 of 53 adolescents over 16 years had graduated. Of 49 adult patients, only 21 were in some form of employment. A lack of age-appropriate independence was observed in a large proportion (86%) of patients over 17 years, who continued to live with their parents or other persons taking care of them, whilst only 14% were living alone or with a partner. We conclude that, despite improved survival, psychosocial adaptation continues to be impaired in paediatric patients with CRF, especially in adolescents and those on dialysis.

Cytological observations on spruce needles after prolonged treatment with ozone and acid mist.

Light and electron microscopic studies of four clones of 5-year-old Picea abies trees subjected to ozone and acid mist treatment showed, that: (1) Clones respond differently to the treatment, with clone 14 the most sensitive clone. (2) Main effects were observed in the mesophyll; the vascular strand showed minor cellular changes. (3) Needle shape and ratio of intercellular area to cross section was clone- age-dependent, with a clear increase in intercellular space associated with the treatment (clone 14 and 11 only). (4) Accumulation of tannins in vacuoles was clone-specific. (5) Strong starch formation was found in all samples; in clones 14 and 133 this formation was enhanced by the treatment in older needles, if number of starch grains per cell was calculated. (6) The accumulation of plastoglobules in plastids depended on clone and age, with the older needles containing more globules. In clones 11 and 133, the treatment led to an increase in the number of plastoglobules. (7) Grana stacking in all clones and both needle ages subjected to ozone and acid mist was significantly reduced. The observed changes in the ultrastructure of needle tissue are comparable to those found in field investigations with similar conditions, or phytotron studies evaluating pollution effects on spruce trees.

[Contractility parameters of the right heart in patients with chronic obstructive lung diseases without pulmonary hypertension].

By means of the radionuclide ventriculography certain contractility parameters of the right heart are presented in 15 patients with chronic obstructive bronchitis without pulmonary hypertension. The results confirm that also without increase of the pulmonary arterial blood pressure the ejection fraction of the right ventricle in rest is decreased in possible dependence upon the size hypoxaemia. The increase of the ejection fraction of the right ventricle under stress is insufficient in 7 out of 15 patients, in which case a connection with the left-ventricular ejection fraction is observed. A characteristic valuation of the right- and left-ventricular ejection fraction for pathophysiology and prognosis of the pulmonary heart disease is discussed. The necessity of further investigations is emphasized.
