[Regional statistical analysis of peripartal mortality caused by cardiovascular diseases 1962-1984 in East Germany].

The analysis of peripartal mortality showed a total number of 273 death cases caused by cardiovascular diseases in the GDR in the period from 1962-1984. - Based on the finding, that in the GDR all preconditions for a country wide statistical analysis of death cases during gestation are fulfilled, we can result from a complete registration of all maternal cardiovascular death cases. Rheumatic heart diseases are the largest group with a percentage of 44.3. They will be followed by death cases as a result of brain and sub arachnoid hemorrhage with 13.9%, by such owing to congenital anomalies of the cardiovascular system with 9.9% and by death cases caused by a pulmonary heart disease with 9.2%. Frequency rates of the investigated period show a continuous decrease from 0.98 per 10,000 newborn children in the period from 1979-1983. Death cases as consequence of rheumatic illness, acute and subacute bacterial carditis and pulmonary heart diseases could be considerably reduced in the investigated period. We have to notice a stagnation of death cases caused by brain and subarachnoid hemorrhage and death cases as a result of congenital anomalies of the cardiovascular system. A temporal relationship between mortality decrease on one hand and improved of diagnostic, therapeutic and organizational measures on the other could be proved. Despite the considerable decrease of death cases owing to cardiovascular diseases - considering the likewise noticed simultaneous reduction of the wide peripartal mortality - the percentage has been hardly changed high with figures between 9.1% and 7.0%. The same is to say about its portion in the group of indirect or non-gestational caused death cases, too. From that follows, that at present times, too, cardiovascular diseases belong to that death-causes groups, which burden peripartal mortality on a large scale.

Detection of Toxoplasma-specific IgM in cord blood sera by antibody-class capture enzyme-linked immunosorbent assay.

An antibody class capture (ACC) enzyme-linked immunosorbent assay (ELISA) utilizing an F(ab)2 conjugate was established and its specificity and reproducibility were determined. The test was further validated by comparing its results with those obtained by 2 overseas reference laboratories which used the indirect immunofluorescence antibody test (IIFT) for Toxoplasma-specific IgM. It was found that the F(ab)2ACC-ELISA was not affected by the presence of rheumatoid and antinuclear factor in serum, that it was reproducible (coefficients of variation less than 10.0%), and that it compared well with tests currently used overseas. ELISA was then used to test for the presence of Toxoplasma-specific IgM in cord blood sera. The results obtained to date are consistent with the hypothesis that the prevalence of congenital toxoplasmosis in the population of Adelaide is less than 1/2,000.

Detection of Toxoplasma cysts and oocysts in an urban environment in a developed country.

Feces of 115 stray or unwanted cats were screened for Toxoplasma oocysts by sucrose density centrifugation and inoculation into mice. None of the feces examined contained detectable levels of viable oocysts. Using a laboratory infected cat, the sensitivity of this detection technique was found to be about 250 oocysts/g of feces by microscopic analysis and about 2 oocysts by mouse inoculation. Samples of pork and lamb chops bought from suburban butcher shops were screened for Toxoplasma cysts by pepsin digestion and mouse inoculation. One of the 30 pork and none of the 30 lamb chops contained viable cysts. Experimental studies determined that the sensitivity of this method of detection was about 1 cyst/100 g of tissue. The relative likelihood of each route of Toxoplasma infection for members of an urban community in a developed country is discussed.

[Congenital heart defects as a cause of still birth and infant mortality].

Among 4,686 still-births and children died from 0 to 1 a in the German Democratic Republic in 1979 there were 307 (6.6%) isolated congenital heart diseases. We only analyzed 294 died infants. The first 5 diagnoses were: Hypoplastic left heart (all infants died within the first 28 d) 9.9%, Ventricular septal defect 9.9%, Transposition of great vessels 8.8%, Coarctation aorta 7.1%, Common truncus arteriosus 5.8%. The most important results are an interesting frequency with advancing years of mothers (3.4% in 10,000 mothers of live-births aged 16 a and less to 20.6% at the age of 40 a and over (see Fig. 1), 13% premature children (under 37 weeks of gestation), 23% children under 2,500 g birth weight, 55% boys and 45% girls. 80% of the above 294 infants died within the first 3 months.

Motor nerve conduction velocity for assessment of gestational age in twins.

During the period from October 1, 1979 to March 31, 1981, 40 maturity tests were made on 20 pairs of twins at the Department of Obstetrics at Berlin-Neukölln Hospital. We therefore used the motor nerve conduction velocity (NCV) of the ulnar nerve combined with the Farr system. Only those children whose gestational age was absolutely certain were examined. A strong correlation emerged between the NCV level for twins and singles (r = 0.8 at p less than 0.0001), so that it was possible to establish the maturity age of twins to an accuracy of +/- 1 week. The levels of the NCV measurement within a pair of twins showed a high degree of agreement (r = 0.9 at p less than 0.0001) in which the average fluctuation amounted to 0.5 m/s = 2-3 days. Even when there was a large variation in the weight of the twins (greater than 300 g or greater than 15%), there were only minimal differences (r = 0.9 at p less than 0.0001). On the other hand, the Farr system deviated significantly in a false-positive direction up to the 38th week of gestation. Measuring the motor NCV of the ulnar nerve proved to be a simple and precise method to assess the neonatal gestational age in twins.

[A new ECG electrode concept for the conduction of fetal heart action potentials without penetration of the skin].

Internal cardiotocography is an important method for reliable supervision of the fetus during labor. The main task is the prevention of fetal hypoxia. However, there is a considerable disadvantage as the electrodes used penetrate the fetal skin, creating a possible entry point for organisms. The concept we have developed forms a new way of decreasing the risk of infection during labor by conducting the fetal heart rate potentials without penetrating the skin. The electrode is fixed to the skin of the presenting part by tissue adhesive and electrical contact between the fetal skin and the wire of the electrode is established through using electrolyte fluid.
