PCA after subarachnoid block for cesarean section.

We studied the pain control, narcotic side effects, and PCA utilization with intravenous PCA morphine during 24 hours post cesarean section period. Fifty-two consecutive women were included in the study. Each received subarachnoid block with hyperbaric bupivacain with addition of fentanyl. After surgery, the patient received diclofenac suppository and IV-PCA commenced in the recovery room. Severity of pain and sedation was assessed hourly, and maximum pain and sedation scores for each 6-hour period were recorded. Two-third of the patients felt mild to moderate pain during transition from spinal analgesia to IV-PCA. Pain severity steadily improved during four 6-hour periods (p-value <0.001). Highest mean sedation score was noticed during the third six-hour postoperative period. Mean morphine consumption was 50 mg. The ratio between number of time PCA activated and dose received and pain score helped in managing the postoperative pain. Morphine IV-PCA, adequately replaces post cesarean section spinal (bupivacain-fentanyl) analgesia with fewer side effects.

Quality of handover to the postanaesthesia care unit nurse.

I surveyed 276 patients admitted to the postanaesthetic care unit (PACU). The quality of handover to the PACU nurse was assessed by scoring four indicators: the quality of verbal information about the patient, the condition of the patient on admission, the professional behaviour of the anaesthetist and the nurse's satisfaction with the handover. Maximum scores were attained by 32.6% of anaesthetists for the quality of condition of the patient. Professional behaviour was judged satisfactory or good in 87% of anaesthetists and almost half of the anaesthetists (48%) scored full marks for the nurse's satisfaction with the handover. The scores of all four indicators were aggregated to give an overall impression of quality of handover; this was good in fewer than half the number of patients brought to the PACU.

Progression of first degree atrioventricular block to second degree Mobitz type I block during spinal anesthesia associated with induced hypertension.

A case is presented in which an elderly patient with preexisting first degree atrioventricular (AV) block progressed to second degree Mobitz Type I AV block during spinal anesthesia and associated with hypertension induced by a pure alpha 1 agonist. Second degree AV block caused by increased vagal tone was transient, which resolved as the blood pressure normalized. Hypotension due to spinal anesthesia treated with pure alpha 1 agonist can increase AV block in patient with pre-existing first degree heart block.
