Arthroscopic all-inside meniscus repair.

The all-inside arthroscopic meniscal repair technique allows placement of vertically oriented sutures, which have the strongest pullout strength, across difficult-to-access posterior horn tears. These sutures are placed perpendicular to the tear, rather than obliquely, and without entrapment of the posterior capsule, resulting in a balanced anatomic repair. The posterior cannula allows excellent tear preparation and minimizes dissection. This may improve postoperative pain management and permit outpatient surgery. The all-inside technique minimizes neurovascular injury. This method has limited application and, therefore, requires that the surgeon learn more than one repair technique to repair tears anterior to the posterior horn. The main disadvantage is that advanced instruments (suture hooks and 70-deg lens) are required, as well as the surgical expertise to use them.

Swedish arthroscopic system.

Three anterior joint line portals, coupled with proficient use of angled arthroscopes, yield access to the entire knee joint and a complete arthroscopic examination. The arthroscope is inserted in the joint midline, through the patellar tendon, allowing passage through the intercondylar notch and direct visual access to the posterior compartments. This arthroscope location, along with instrument portals to both sides of the central scope location, allows bimanual anterior surgery and several options for operation in the posterior compartments. Some surgical maneuvers are best accomplished by the Swedish method. Arthroscope insertion in the midline through the patellar tendon has been performed thousands of times and is a safe, comprehensive approach to arthroscopy, both for diagnosis and for percutaneous surgery. It can be a primary method (as it is for me), or it can be used to solve specific problems after complete diagnosis is established.

Fracture-dislocation of the hip.

This report reviews two groups of cases: 194 fracture dislocations of the hip reviewed in 1952, and a follow-up of 222 additional fracture dislocations of the hip seen since then. The report describes the methods of treatment used in terms of the various classifications of fracture dislocations of the hip and analyses the results, the complications and the comparative results in these two groups of patients. Recommendations as to the proper form of management for each type of fracture dislocation are presented.
