Determination of cyclic AMP in heat-separated human epidermal tissue.

Heat separation is introduced as a simple, reliable and quick method for obtaining pure epidermis for cyclic AMP analysis. Heating at 60 degrees -65degrees C for 10 min resulted in a distinct separation of epidermis from dermis. The level of cyclic AMP in epidermal tissue from 9 patients was 3.93 +/- 0.31 pmol per mg dry weight. The inter- and intra-assay variations were 10.3% and 8.8%, respectively. The coefficience of variation in eight biopsies from the same piece of skin was 16.2% The sensitivity of the assay was 0.07 pmol. Recovery analyses revealed that 80.5% of labelled cyclic AMP was refound after extraction and purification procedures. Addition of unlabelled cyclic AMP resulted in a recovery of approximately 100%. The specificity of the assay was high without interference from several nucleotides and the validity was confirmed by zero values obtained after addition of phosphodiesterase or charcoal.

DNA interstrand crosslinks in normal human skin visualized by electron microscopy.

Normal human skin was investigated for DNA interstrand crosslinks by a denaturation electron microscopic method. Skin biopsy materials from the thighs of 8 healthy volunteers were separated into epidermis and dermis. DNA from each layer was isolated, purified and studied in the electron microscope. The presence of DNA interstrand crosslinks in normal human skin was demonstrated. The number of crosslinked DNA molecules and the density of crosslinking within the molecules were determined. Crosslinks were found in 0.8% of the scored molecules and were located almost exclusively in epidermal DNA. The intramolecular density of crosslinking showed a wide range of variation in each volunteer and between the volunteers.

Electron microscopic cytochemical demonstration of adenyl cyclase activity in psoriatic epidermis.

Isoproterenol and sodium fluoride stimulated adenyl cyclase activity was detected in epidermal tissue from 2 patients with untreated psoriasis by an electron microscopic cytochemical technique. Adenyl cyclase activity was present on the outer surface of the cell membranes, predominantly in the basal cells and in the 4-5 lower Malpighian cell layers, while the superficial layers, stratum granulosum and stratum corneum showed no activity. The precipitates (lead-PPi complexes) after isoproterenol stimulation were larger and fewer in number than those seen after sodium fluoride stimulation. Isoproterenol stimulation was abolished by propranolol. Neither the uninvolved epidermis from the 2 patients with psoriasis nor the normal skin from 2 volunteer individuals showed any difference from the psoriatic epidermis.

Asymptomatic male and female gonorrhoea.

Three hundred and ninety-nine patients attending the out-patient clinic of the Department of Dermatology and Venereology, Rigshospital, Copenhagen, for various venereal complaints were examined during a three-month trial. Neisseria gonorrhoeae was found in 75 female and 89 male patients. 43 women (57%) and 13 men (15%) were asymptomatic at the first visit. The duration of the asymptomatic period exceeded 3 weeks in 25 female (33%) and 7 male (8%) patients. The asymptomatic period in the male group varied from 15 to 90 days and from 4 to 180 days in the females. The majority of the patients with asymptomatic gonorrhoea had been exposed to a sexual contact with proven gonococcal infection, thus emphasizing the importance of contact investigation.

Incidence and prognostic significance of skin disease in patients with HIV/AIDS: a 5-year observational study.

Mucocutaneous diseases are common in HIV-infected patients. The aim of the present study was to determine the observed frequency of skin diseases and their prognostic significance for the progression of HIV infection. In a cohort of 150 HIV-infected patients diagnosed from 1986 to 1987 the number of mucocutaneous diseases, the CD4+ cell count, time to development of AIDS and survival time were registered for a period of 5 years. Eight patients were lost for follow-up. In the remaining 142 patients the number of mucocutaneous findings increased markedly during the observation period. The most frequent registered diseases and these with the highest increase were oral candidiasis, hairy leukoplakia, seborrhoeic dermatitis and herpes simplex. On average, 5.2 different diagnoses were registered per patient at the end of the follow-up period. The study showed that the total number of mucocutaneous diseases and the CD4+ cell count were significantly correlated to survival time and time to development of AIDS. The number of mucocutaneous diseases, like the CD4+ count, is an indicator of the immune system and the prognosis of HIV infection.

Ceftriaxone/metronidazole is more effective than ampicillin/netilmicin/metronidazole in the treatment of bacterial peritonitis.

In a prospective, open, controlled clinical study, 190 consecutive patients who were thought to have bacterial peritonitis before operation, were randomised to antibiotic treatment during and after operation with either ceftriaxone 1 g plus metronidazole 1.5 g once daily (n = 94) or ampicillin 2 g plus netilmicin 150 mg twice daily plus metronidazole 1.5 g once daily (n = 96). Incisional and deep surgical wound infections, postoperative pneumonia and urinary tract infection as well as deaths caused by infection were recorded. Ceftriaxone-metronidazole was significantly more effective than ampicillin-netilmicin-metronidazole, 6/94 wound related infections (6%) compared to 18/96 (19%) (p = 0.02). In patients with peritonitis caused by a perforated colon or appendix the rates of clinical failure were 6% and 28%, respectively. We consider ceftriaxone plus metronidazole an efficient and easily administered antibiotic regimen in patients with bacterial peritonitis, and both the wide range of activity against Gram-negative aerobic rods and the long half life of ceftriaxone seem to be beneficial.
