[Ureterocele. Tailored treatment. Report of 45 cases].

The advancements in the treatment of ureterocele have prompted early diagnosis of this condition. We report on 45 cases of ureterocele (6 simple and 39 ectopic) that had been treated at our service from 1974 to 1992. The present series comprised 43 patients (32 females, 13 males), with ages ranging from 20 hours to 9 1/2 years; three cases had been diagnosed prenatally. All cases were evaluated by IVP and cystography. There were 16 cases of left ureteropelvic duplicity, 11 right and 10 bilateral. There was one case with right triplication and 6 cases with single system. Fourteen cases had reflux in the lower ipsilateral kidney, 5 in the contralateral kidney and 4 were bilateral. In some cases, patient evaluation included the use of isotopes, US, CT and cystography. Two cases required no surgical procedure; 2 required endoscopic incision; 6 resection and reimplantation; 1 heminephrectomy; 19 resection and heminephrectomy; 1 resection, reimplantation and ureteropyelostomy; 8 resection and nephrectomy; aspiration, nephrectomy and resection of the ureterocele was required in 1 case. Control evaluation at 6 and 18 months following treatment was satisfactory in 88.3%; of these, 41.8% had pyelonephritic scars, 16.2% had residual reflux and 4.6% urinary incontinence. There was only one death.

[Fibrin adhesive: a new therapeutic alternative in the treatment of cystic lymphangioma].

We present three cases of cystic lymphangioma of the neck, treated with a new therapeutic method which consists of an injection of fibrin adhesive into the lesion following aspiration of the liquid contents of the lymphangioma. Two of the cases had been treated surgically and were faced with the recurrence of the tumor, the aforesaid treatment was applied. In the third case, it was the only therapy applied. We believe that the results obtained were excellent in all three cases.

[Avoidable complications in the management of a patient with an artificial urinary sphincter].

All the patients who underwent implantation of an artificial urinary sphincter can have three types of complications: those coming from the own patient, mechanical complications and other aleatory ones. J. L. C. A., a 13-year-old boy with secondary neurogenic bladder, received the implantation of a model AUS 800 artificial urinary sphincter for urinary incontinence. After 15 months without problems, he was admitted in our hospital with pump erosion in the scrotum. He was operated on and the pump system was preserved. No plain roentgenograms were made neither before nor after surgical procedure. However, the balloon pressure reservoir was not seen in a control pielogram performed before his discharge from the hospital. Three months later the entire device should be removed because of infection. In the operation a cut off tube was found. A new artificial urinary sphincter was reimplanted and the cuff was placed around the bulbous urethra one year later. Retrospective analysis of this case can help to review the possible complications and their management.

[Behavior of the contralateral ureter following surgery for unilateral antireflux surgery].

We present our experience of 44 cases with unilateral vesicorenal reflux that had been treated by ureteroneocystostomy using the Cohen technique. The reflux disappeared in 32 cases, it appeared in the contralateral ureter in 7 patients, it was ipsilateral in 2 and bilateral in 3. Of the 10 cases with contralateral reflux, 2 required surgery, while 8 were amenable to medical treatment. In our series, contralateral reflux appeared at random. Since the spontaneous cure rate was 80%, we believe that bilateral antireflux surgery for unilateral reflux is unwarranted. Urodynamic studies can be important in the indication for conservative treatment.

[Psoas fixation of the bladder. An efficient aid in cases of repeat surgery of the uretero-vesical junction].

We reviewed our experience with 6 patients (5 males, 1 female) aged 3 to 10 years in whom psoas hitch of the bladder was performed in the course of repeated surgery of the vesicoureteral junction. All patients had previously undergone an operation on the distal ureter and presented an evident shortening of its length. After ureteral reimplantation in 4 patients (3 were Hendren's procedure and 1 Cohen's reimplantation), the reflux either persisted or became worse and the remaining 2 patients (posterior urethral valves) underwent cutaneous distal ureterostomy. In all cases the psoas hitch procedure successfully resolved the ureteral defect, provided the bladder with a new surface for a submucosal tunnel of suitable length and diameter, and permitted reimplantation of the ureter without traction or possibility of bending. The psoas hitch procedure is a simple and effective alternative in the management of distal ureter length defects. Although it has precise indications, it can have broad applications when combined with other techniques (TUU). However, the approach should be one of caution in those cases that also require surgery of the contralateral distal ureter.

[Brunn's nests as a cause of benign tumor of the bladder in children].

This report concerns the case of a male patient admitted for an operation due to vesicoureteral reflux. During the operation a sessile tumor was discovered in the interior of the bladder at the level of the neck. The tumor corresponded histologically to a mucous polyp with Brunn's nests in the interior. The following are commented on: the extreme rarity of the case, the histopathological characteristics of this abnormal epithelial proliferation which is produced as an exaggerated response to an irritative stimulus and its non-malignant nature.

[Ureterocalycostomy].

We present the case of a eight-year old girl suffering from severe left hydronephrosis, who was treated in a second operation after failure of a primary pyeloureteral anastomosis, with a direct suture of the ureter to the lower renal calyx, with good result.
