Paraquat poisoning.

A fatal case of paraquat poisoning is described. Postmortem concentration of paraquat in different tissues reveals that treatment in this case could not prevent lethal tissue accumulation. Although accumulation was more pronounced in renal tissue, lung toxicity caused death. The formation of enormous fecaliths and the appearance of hypercalcemia are reported. Both were most likely connected with Füllers earth therapy. In spite of the fact that the exact nature of the equilibrium between plasma levels and tissue accumulation of paraquat (static or dynamic) is not understood, aggressive treatment must be recommended, even after the distribution phase and despite likely "fatal" plasma levels.

Unipuncture membrane plasmapheresis.

A simple method of plasma exchange is presented that uses a hollow-fiber plasma filter together with conventional unipuncture dialysis equipment. The procedure was well tolerated for eight biweekly sessions in a patient with metastatic breast cancer.

Autogenous saphenous vein AV fistulae for haemodialysis: eight years experience with 30 patients.

Since 1973, 41 autogenous saphenous vein (ASV) fistulae in the arm or the leg have been constructed in 30 chronic haemodialysis patients. The mean patency rate for the arm fistulae was 36.8 months and 21.9 months for leg fistulae. Eight patients received a leg fistula, after their arm fistula thrombosed: in this group the mean patency rate for the arm and leg fistulae was 15.4 and 20 months respectively. If a conventional fistula is not feasible, an ASV fistula is the method of choice for some patients.
