[Dentomaxillary and -facial problems in cleidocranial dysplasia].

A 14-year old boy with cleidocranial dysplasia was referred because of an almost complete presence of the deciduous dentition. Diagnostics by means of a panoramic radiograph, lateral headfilm, CT scan and 3D computer images of the dentomaxillary complex showed 7 supernumerary permanent teeth: 13', 11', 21', 23', 28', 35' and 45'. A combined orthodontic-surgical treatment was started. The supernumary teeth were removed surgically and the impacted teeth are exposed. The closed eruption technique was used to extrude these teeth orthodontically.

Brown tumour: presenting symptom of primary hyperparathyroidism.

The skeletal lesions of primary hyperparathyroidism, including brown tumour, are rare nowadays, with the practice of checking serum calcium levels leading to an earlier diagnosis of hyperparathyroidism. Clinical, laboratory, radiographic and histological investigations can lead to a correct diagnosis. Treatment of brown tumour focuses on the hyperparathyroidism, and is usually followed by a regression of the brown tumour. The diagnosis of hyperparathyroidism and brown tumour should be considered in patients with hypercalcaemia and an osteolytic expansive bone lesion. We present a patient where a brown tumour of the mandible was the presenting symptom of primary hyperparathyroidism.

Treatment of hemifacial microsomia in a growing child: the importance of co-operation between the orthodontist and the maxillofacial surgeon.

The treatment of patients with hemifacial microsomia (HM) always requires an interdisciplinary approach including at least maxillofacial surgery and orthodontics. Co-operation not only within the team, but also with the patients and their family is essential in order to achieve the best results. In the case history of the 10-(1/2) year old female patient reported here, three surgical interventions (two with costo-chondral bone grafts) and a 3-year orthodontic treatment have taken place. A harmonious facial and occlusal result was finally reached.

Cephalometric evaluation of maxillary advancement with an internal distractor in an adult boxer dog.

OBJECTIVE: The aim of the study was to evaluate cephalometrically the effects of distraction of the maxilla over a 1-year period by means of an internal distractor applied in a boxer dog. DESIGN: Internal distractors were placed bilaterally in the internal cavity of the maxillary sinus of a 2-year-old boxer dog after a Le Fort I osteotomy. Distraction was started 5 days after surgery and activations were continued for 14 days at a rate of 1 mm/day. Standardized lateral cephalograms were taken with an extemal source of 90 kV, 60 mS immediately pre-surgery, at day 1 after the start of the distraction (dp) and at day 5 dp, day 7 dp, day 10 dp, day 14 dp (end of distraction), at 14 days of consolidation period (cp), at 28 days cp and at 56 days cp (removal of the distractors). One year after the removal of the distractor, a final lateral cephalogram was taken. Cephalometric analysis was performed and superimpositions were used for the evaluation of the sagittal position of the maxilla. RESULTS: The linear measurements, as well as the superimposition showed evidence for a significantly advanced position of the maxilla, which was stable 1 year after removal of the distractor. CONCLUSION: The application of the internal maxillary distractor in a boxer dog resulted in a sagittal advancement of the midface that was still present after 1 year.

Gonial angle changes after bilateral unidirectional mandibular distraction in sheep.

It was the aim of this study to investigate radiologically the distraction site in the gonial angle area after three different protocols of unidirectional mandibular distraction in sheep. Bilateral mandibular distraction was performed in nine sheep, and one other sheep served as a sham-operated control. In the nine experimental sheep, three different distraction protocols were used. In three sheep, an oblique corticotomy line was applied and the distractors were positioned perpendicular to the corticotomy line. In three other sheep, a horizontal corticotomy line was applied and the distractors were also positioned perpendicular to this corticotomy line. In the remaining three sheep, again, a horizontal corticotomy was performed, but with an angular positioning of the distractors relative to this corticotomy line. Distraction of 20 mm was performed over 20 days at a rate of 1 mm per day. After 3 weeks of retention the sheep were sacrificed, the mandibles were dissected and hemisectioned, and standardized hemimandibular contact radiographs were made of the specimens. The distracted gaps were traced from the radiograph and the gonial angle was measured. The displacements of the distracted parts were measured, the gonial angles were determined, and the values were compared with four additional control non-distracted sheep mandibles. The results show that there was a very large variability in the distracted gaps as well as in the gonial angle changes after distraction, and it was not possible to recognize any of the three different distraction protocols radiographically after distraction. Although performed under standardized conditions it is difficult to reproduce intended mandibular corticotomy lines in sheep.

Maxillary distraction using a trans-sinusal distractor: technical note.

In this pilot study, the principle of distraction osteogenesis was used to advance the midface of a boxer dog. A modified high Le Fort I-type osteotomy was performed. Following a latency period of 5 days the maxilla was distracted 14 mm in 14 consecutive days at a rate of 1 mm per day. Ten weeks after the completion of the distraction, multiple biopsies were taken across the distraction gap. Histological observation showed bone deposition in the osteotomy sites. Soft and hard tissue formation resulted in complete healing across the distraction gap. The maxillary sinus was used to accommodate the distraction device. Superimposition of the standardized lateral cephalograms taken at the end of distraction and 14 months after the removal of the distractors showed no sign of relapse in the achieved sagittal advancement of the maxilla. This small, intraoral trans-sinusal placed distractor has a completely new conceptual design, and may be helpful in distraction of maxilla in children and adults with midfacial hypoplasia.

Hormone-dependent fluctuations of pressure pain threshold and tactile threshold of the temporalis and masseter muscle.

In order to better objectify reported pain and disturbed sensation, psychophysical parameters as pressure pain threshold (PPT) and tactile threshold (TT) have been introduced in pain research. The present study evaluated fluctuations of these parameters, obtained using an algometer (PPT) and von Frey hairs (TT), in relation to fluctuations in female reproductive hormones. Ten students not using oral contraceptives and eight students using oral contraceptives participated in the study. In addition, seven male students served as a control group. During two menstrual cycles, the PPT and TT of the masseter and temporalis muscles, as well as from the hand, were obtained twice per week. At the end of the study the values were evaluated in relation to the different phases of the menstrual cycle, by means of a linear mixed model. The TT appeared very reproducible within subjects and did not fluctuate over time. Scores on the left temporalis muscle appeared significantly higher than that on the right side. In all subjects the PPT tended to be higher on the left side, while in students using oral contraceptives, the PPT values of the temporalis muscle were significantly higher in the menstrual phase, and lower in the follicular phase. A significant correlation between the PPT of the temporalis muscle and the TT of the overlying skin was observed.

Tactile threshold and pressure pain threshold during treatment of orofacial pain: an explorative study.

The aim of the present study was to evaluate the pressure pain threshold (PPT), tactile threshold (TT) and pain intensity as measured on a Visual Analogue Scale (VAS) in patients with pain in the orofacial region. Twenty-six patients with myogenous pain (7 ss), myogenous and capsular pain (11 ss) or neuropathic pain (8 ss) participated in the study. At baseline, the PPT was evaluated using an algometer and the TT using von Frey hairs. In addition, the pain intensity was rated on a VAS scale and the psychological profile made using the SCL-90 questionnaire. In some of the patients, the measurement procedure was conducted I week later, immediately before active treatment started. As active treatment, patients with myogenous or capsular pain received physical therapy in addition to conscious relaxation of the masticatory system, while patients with neuropathic pain were treated using tricyclic antidepressant medication. After 4 to 6 weeks, the measurements were repeated. Statistical evaluation using a linear mixed model revealed no significant changes in TT. In patients with myogenous and capsular pain, as well as in patients with neuropathic pain, the PPT increased after active treatment, while it decreased in the group with pure myogenous pain. The VAS score decreased in all groups. Regarding psychological profiles, a variety of characteristics were observed and the scale for sleeping problems was especially elevated in a majority of the patients.

Bone healing in porous implants: a histological and histometrical comparative study on sheep.

Tissue integration in four types of porous implant materials (Interpore 200 or Corallin hydroxyapatite, hydroxyapatite blocks, hydroxyapatite granules and polymethylmethacrylate) was evaluated in vivo. Porous blocks measuring 20 mm x 10 mm x 8 mm were implanted in mandibles and iliac crests of sheep. Bone healing in porous blocks was studied at 2 and 6 months after implantation. The behavior of the material itself was also analyzed. Histological and histomorphometrical analysis revealed bone healing depending upon healing time and material. On the basis of analysis of variance, differences in amounts of bone ingrowth at 2 and 6 months were statistically significant (p=0.0039 in mandible; p=0.0351 in iliac crest). The longer the time span, the more mineralized tissues were observed in the specimen. Our data confirmed that hydroxyapatite has osteoconductive capacities. Porous PMMA was found to be biocompatible, but it showed less bonegrowth within the pores. Interpore 200, which had the highest surface to volume ratio was found to display the highest level of osseointegration and biodegradation.

Management of necrotizing fasciitis in the neck.

Necrotizing fasciitis is a soft tissue infection, usually polymicrobial, characterized by necrosis of fascia and subcutaneous tissue. Although it more commonly involves the groin, abdomen, and extremities, it may also occur in the head and neck. We report a case of cervical necrotizing fasciitis arising from a dental infection and review the cause, pathophysiology, diagnosis and treatment of this potentially lethal entity. Early detection and accurate intervention are emphasized. Extensive surgical debridement completed with hyperbaric oxygen therapy and antibiotics are the mainstay of treatment.

[The use of tongue flaps for oral tissue repair].

Pedicled tongue flaps have proven to be an effective method of repairing defects due to tissue loss in the oral cavity. Their rich vascular supply, mobility and close proximity to the lesion (lips, palate, floor of the mouth, cheek mucosa, oro- and hypopharynx) make the use of these flaps feasible in a variety of sites. After a review of the vascularity of the tongue, the possibilities of reconstruction will be discussed. The surgical procedure will be briefly documented with some clinical cases.

[A psychiatric key: the day hospital].

After being the first structure of this type in Belgium, the day hospital "La Clé" from the University of Liège exhibit the modalities of its functioning with some comments. It is considered as a dynamic structure in the view of a intergrated psychiatry. The specificity of the day hospital is discussed.
