Use of non-pneumatic antishock garments for ruptured abdominal aortic aneurysm.

Two cases of near-fatal aortic abdominal aneurysm (AAA) are described which were successfully stabilized by a new principle for exerting abdominal compression, the non-pneumatic medical antishock trousers (NP-MAST). Obviously, the compression obtained with the NP-MAST suffices for this dramatic emergency. Moreover, compared with pneumatic antishock garments (PASG), the NP-MAST has the advantage of being light, independent of flight height and unable to produce dangerously high pressures. Moreover, it contains no manometers, which could detract the doctor's attention from more important measures. The need for endotracheal intubation in advance to application of any kind of abdominal compression (PASG or NP-MAST) is stressed.

The Solar Acoustic Spectrum and Eigenmode Parameters

The Global Oscillation Network Group (GONG) project estimates the frequencies, amplitudes, and linewidths of more than 250,000 acoustic resonances of the sun from data sets lasting 36 days. The frequency resolution of a single data set is 0.321 microhertz. For frequencies averaged over the azimuthal order m, the median formal error is 0.044 microhertz, and the associated median fractional error is 1.6 x 10(-5). For a 3-year data set, the fractional error is expected to be 3 x 10(-6). The GONG m-averaged frequency measurements differ from other helioseismic data sets by 0.03 to 0.08 microhertz. The differences arise from a combination of systematic errors, random errors, and possible changes in solar structure.

Differential Rotation and Dynamics of the Solar Interior

Splitting of the sun's global oscillation frequencies by large-scale flows can be used to investigate how rotation varies with radius and latitude within the solar interior. The nearly uninterrupted observations by the Global Oscillation Network Group (GONG) yield oscillation power spectra with high duty cycles and high signal-to-noise ratios. Frequency splittings derived from GONG observations confirm that the variation of rotation rate with latitude seen at the surface carries through much of the convection zone, at the base of which is an adjustment layer leading to latitudinally independent rotation at greater depths. A distinctive shear layer just below the surface is discernible at low to mid-latitudes.

Omeprazole and visual disorders: seeing alternatives.

In the WHO data base, visual disorders reported spontaneously with omeprazole, ranitidine and cimetidine, are very rare in the context of the widespread use of these drugs. There is a maximum reporting rate of severe visual impairment possibly ascribed to i.v. omeprazole of 0.94 reports per million treatment days in one year and in one country, Germany. This gives the worst quantitative case scenario for omeprazole by a single route of administration, to be compared with the worldwide reporting rate of all severe visual disorders by all routes of administration--0.008 reports per million treatment days. Moreover, the reported visual abnormalities have a varied pathophysiological aetiology and their number increased in Germany after the first signal was raised in that country. Thus, apart from a direct causal relationship, solicited reporting artifact is one alternate plausible explanation for the apparent excess of cases of visual disturbance to omeprazole compared with cimetidine and ranitidine. That reporting rates of clinical events on newly marketed drugs are generally higher than with older drugs is a second factor for higher reporting rates with omeprazole. Vasculitis has been suggested as an aetiological factor, but the even lower reporting rate of this reaction makes this an unlikely hypothesis without any other supporting evidence. The authors are unaware of any drug that has caused a vasculities solely affecting the eye. Information on the prevalence of relevant visual disorders in the community would have been of considerable help in interpreting this signal, and a case control study of visual events in relationship to severe illness would be of public health interest, since no data seems to exist concerning this.

Major interventions in the field stabilization of trauma patients: what is possible?

Previous studies on prehospital care are mostly hampered by a large number of less-urgent missions and lack of utilization of the possibilities which blur the effect of an advanced medical service. The current analysis of the most aggressive trauma care on-scene concludes that largely all vital stabilization can be carried out prehospitally, except performing an X-ray (with its possible consequences) and an operation. The latter measure does, however, also represent the current limits of field stabilization: circulatory unstable patients with penetrating bleeding trauma or presumed internal haemorrhagia will require a minimal stabilization--concentrating on securing the airway--and then fast transport to the nearest hospital with capability to carry out an acute operation (the 'scoop and run' principle). Novel principles and pathophysiological understanding indicate that new principles of field stabilization may actually offer further support and thereby increase the chances of survival for these patients, provided the necessary new techniques acknowledge the need for speed. In this development, the principles in prehospital care often exceeds what is actually offered in the hospitals' emergency rooms. Since the majority of advanced prehospital methods depends on comparatively safe anaesthesia and analgesia techniques, the particular responsibility for anaesthetists in this field is stressed.

Sexual function in patients with symptomatic benign prostatic hyperplasia.

This study evaluated the sexual function in an unselected population referred to hospital for treatment of clinical BPH. 401 patients filled in the DAN-PSS questionnaire including 3 questions concerning sexuality (erection, ejaculation volume and pain/discomfort during eja culation). The patients were divided into four age groups (< 50; 50-59; 60-69 and > 69 years). The answers were analyzed for severity of symptoms and the related bothersomeness. The proportion of patients with erectile dysfunction increased with increasing age, and the related bother was much higher in the young males. Fiftyfour percent noticed that the amount of semen had decreased, and a correlation between increasing age and smaller ejaculation volume was observed. However, the related botherfactor was low. There was an age independent occurrence of about 15% of patients experiencing pain/discomfort during ejaculation and the bother factor is high. This study shows that the questions concerning sexuality in the DAN-PSS score system are formulated in a way that most patients can understand, and are willing to fill in the form.

Validation of the self-administered Danish Prostatic Symptom Score (DAN-PSS-1) system for use in benign prostatic hyperplasia.

OBJECTIVE: To validate the Danish Prostatic Symptom Score (DAN-PSS-1), a self-administered quality-of-life questionnaire comprising 12 questions related to voiding problems and the perceived bother of each individual symptom. METHODS: Using published results from several comparisons of other symptom scoring systems with DAN-PSS-1, the test-retest reliability, internal consistency, construct and content validity, and responsiveness of the DAN-PSS-1 system were assessed. RESULTS: The system was internally consistent (Cronbach's alpha = 0.73), the median test-retest reliability of answers to each question was 83.5% (range 0-99.7%) and the questionnaire was well understood by the patients. The DAN-PSS-1 system demonstrated a high degree of construct validity, correlating with the extensively used Madsen-Iversen score system (Spearman's correlation coefficient, rs = 0.51) and with the patients' answers to questions about how bothersome their symptoms were (rs = 0.71). The DAN-PSS-1 system discriminated clearly between patients with benign prostatic hyperplasia (BPH) and control subjects (an area under the receiver operating characteristic curve of 0.94). Finally, the DAN-PSS-1 was sensitive to changes following intervention, with scores decreasing from a median of 20 to zero 4 months after patients underwent transurethral prostatectomy and from a median of 11.5 to 7.5 (65%) after patients had received 4 months treatment with an alpha-blocker. CONCLUSIONS: The DAN-PSS-1 system is reliable, valid and responsive, and therefore can be recommended for assessing the severity of symptoms among patients presenting with lower urinary tract complaints suggestive of BPH and in the follow-up after intervention.

Subjective and objective evaluation of patients with prostatism and infravesical obstruction treated with both intraprostatic spiral and transurethral prostatectomy.

In order to evaluate the effect of intraprostatic spiral and subsequent transurethral prostatectomy (TURP) on obstructive benign prostatic hyperplasia, 9 men were evaluated with symptom scores and urodynamic studies at inclusion after 4 months of spiral treatment and 4 months after TURP. Six patients in a control group were evaluated at inclusion after an observation period of 4 months and 4 months after TURP. After treatment with spiral the symptom score decreased in 7 patients, increased in 1, and was unchanged in 1. After the observation period in the control group the symptom score decreased in 4 patients, increased in 1, and was unchanged in 1. After TURP the symptom score decreased even further in all patients, but one. After spiral treatment the urethral resistance decreased in 8 of 9 patients, but according to Abrams and Griffith's [(1979): Br J Urol 51:129-134] definition, only 2 were unobstructed. Five patients in the control group were unchanged obstructed after the observation period, while one was unobstructed. After TURP, the patients in both groups were all unobstructed. Intraprostatic spirals decrease the severity of symptoms and reduce the urethral resistance, but TURP in the same patient was much more effective.

The anatomy of a prostate waiting list: a prospective study of 132 consecutive patients.

OBJECTIVE: To report the results of a standard evaluation programme performed prior to transurethral resection of the prostate (TURP) for benign prostatic hypertrophy (BPH). PATIENTS AND METHODS: All 132 patients with symptoms of prostatism on the waiting list for surgery were invited to attend the hospital to undergo a physical examination, symptom evaluation and routine blood sampling. Of these, 117 attended. If the suspicion of BPH was sustained a transrectal ultrasound examination of the prostate and a urodynamic evaluation, including a pressure-flow study, were performed. RESULTS: Urodynamic evaluation was carried out in 80 of the 117 patients who attended the clinic. Infravesical obstruction was present in 61 patients while in 19 (24%) there was no obstruction. An operative procedure to relieve obstruction was performed in 65 patients (49%). CONCLUSIONS: In a population of patients scheduled for TURP, 24% were found not to have an obstruction. This is in accordance with other reports. As the patients with few symptoms and those who did not have an obstruction were treated conservatively only 49% of the referred cases underwent prostatic surgery.

[The volume of the prostate in benign hyperplasia and evaluation of infravesical obstruction].

In a prospective study group of 22 patients we analyzed the relationship between the volume of benign prostatic hypertrophy (BPH) and infravesical obstruction. We diagnosed obstruction with urodynamic measurement (pressure flow studies) in 16 patients (72.7%). Volume of BPH was investigated by the transrectal ultrasound. We found obstruction in 11 from 14 patients (63.6%) with BPH > or = 30 ml and in 5 from 8 patients (36.4%) with BPH < 30 ml. The diagnostic accuracy of prostatic volume measurement in the diagnosis of infravesical obstruction was only 63.6%. The volume of BPH did not correlate with the infravesical obstruction. (Tab. 3, Fig. 1, Ref. 14.)
