[Estimation of blood pressure, selected biochemical parameters and indices of left ventricular heart function in patients with mild or moderate essential hypertension treated with potassium losartan].

This study was conducted in 19 patients with mild or moderate essential hypertension randomised open trial to evaluate the influence of potassium losartan dosed 50 mg per day on blood pressure, metabolic processes and left ventricular heart function. The following intervals of examination were accepted-at entry (before therapy with losartan) and then after 10 days; after 30 days and after 60 days of treatment. Mean values of arterial blood pressure and heart rate measured by conventional technics and obtained from 24-hour ambulatory monitoring data, biochemical results (serum levels of glucose, uric acid, creatinine, sodium, potassium, total cholesterol, HDL-cholesterol, LDL-cholesterol, triglycerides, bilirubin and serum activity of aminotransferases) and selected indices of left ventricular heart function were evaluated as intragroup differences using Student's paired t test. Afterwards the body mass index was compared. Statistically significant decrease of systolic arterial pressure after 60 days of therapy was revealed and it concerned both diurnal period and sleep time and during whole 24 hours. During treatment with losartan mean creatinine serum level was increased and the serum level of bilirubin was decreased in serum after 10 days of therapy. No significant changes were noted in any other laboratory parameters. After 30 and 60 days of treatment with potassium losartan thickness of interventricular septum end-diastole (IVS-D) diminished and E/A ratio (transmitral flow during early ventricular diastole to peak velocity of late filling) augmented statistically significantly after 30 days of treatment. In addition body mass index was increased after 10 days of therapy with losartan when compared with this index at the entry. This study confirmed profitable antihypertensive losartan effect as significant decrease of systolic blood pressure and demonstrated improve diastolic function of left ventricle during therapy in patients with mild or moderate essential hypertension.

[Usefulness of doppler ultrasonography in diagnosing changes in atherosclerotic lesions in lower extremity arteries in hyperlipoproteinemia].

The purpose of this study was to establish the correlation between the intensity of atherosclerosis of the lower extremities found in the course of the Doppler examination, and type of hyperlipoproteinaemia. An attempt was undertaken to analyse the relation of the risk factors of obliterative atherosclerosis to the condition of the arteries in the lower extremities. 53 men at the age of 37 to 63 years (mean age 51 years) were examined. HLP type IIa, IIb, IV, was found in 16, 19, 18 patients respectively. The control group consisted of 30 healthy non-smoking men. The examination was performed using ACUSON 128 with a linear probe 5 and 7 MHz. The Doppler spectrum was registered from the external iliac, common femoral, superficial femoral, deep femoral and popliteal arteries. Simultaneously, a two-dimensional color picture of contrasted vessels, and the Doppler spectrum of blood velocity were recorded. Blood velocity spectrum was quantitatively assessed according to the classification suggested by Jager et al. The results of the Doppler examination confirmed the unfavourable influence of the so called risk factors on the progress of atherosclerosis, especially low serum concentration of HDL cholesterol and cigarette smoking. Besides, the examination has proved the possibility to diagnose even asymptomatic atherosclerotic lesions of the arteries of lower extremities.

[Doppler ultrasonography of arteries of the lower extremities].

We examined 90 male nonsmokers aged 22 to 76, without any symptoms of arterial occlusive disease. They were in the I class of the NYHA classification. The subjects were divided into 2 groups according to age: group I 22-49, group II 50-76. The study was performed using an Acuson 128 with linear probe 5 MHz. We examined arteries of the lower extremities from external iliac artery to popliteal artery. On the B-mode image the diameter of the artery and wall irregularities were evaluated. Doppler spectra were evaluated according to 3 criteria. Doppler waveforms differed significantly between two groups. The third wave of spectrum (the diastolic wave toward the probe) was significantly lower in group II. This finding may be caused by decrease in vessel wall compliance in elderly patients. Moreover we found spectral broadening in the presence of atherosclerotic plaques (38% in group II). The authors' examination showed that Doppler sonography is useful in the evaluation of abnormality of an asymptomatic areas. Our study also indicated that subject's age has to be taken into consideration when Doppler waveforms are evaluated.

[Measurement of blood flow of the portal system in liver cirrhosis using the Doppler method].

Portal and splenchnic blood flow was studied in a group of 31 patients with hepatic cirrhosis and splenic veins. The authors used Acuson 128 with sector head 3,5 MHz enabling a simultaneous Doppler examination by means of impulse wave. A control group consisted of 50 healthy volunteers. Two patients had regressive flux in the portal vein, one patient had a reflux in the splenic vein. It was found that portal blood flow was significantly lower in the group examined than in the control group, and the splenic one was significantly higher. The sensitivity and specificity of the method of measuring the blood flow in the portal vein was 80%, and 88.2% respectively. 67.7% of patients had a different shape of the Doppler spectrum in the portal vein. That spectrum did not show the normally seen waves due to the heart beats. The authors' examinations show that the measurement of blood flow in the portal and splenic veins by means of the method using Doppler phenomenon provides valuable information on the haemodynamics of the portal system.

Effect of cigarette smoking on the tidal carbon dioxide profile.

Four indices have been tested for quantitative description of the form of the expiratory partial pressure carbon dioxide curves within a group of 80 male smokers with normal routine spirometry. The control group consisted of 60 healthy nonsmokers whose age and residence matched those of the smokers. The tests demonstrated statistically significant differences between smokers and the control group. Gamma angle index and planimetric index displayed high sensitivity to the quantitative description of the capnographic curves, whereas the concomitant cough and sputum production or the smoking index did not. The data produced show that capnographic indices provide valuable data on ventilation-perfusion ratio dysfunction and also, indirectly, on small airways on smokers with normal conventional spirometry.

[Doppler measurements of blood flow in the portal system of healthy volunteers].

A group of 50 healthy volunteers underwent the measurement of blood flow in the protal vein and in the splenic vein by means of the Acuson 128 sector ultrasound probe 3.5 MHz connected with the Doppler pulsation wave. The authors strictly obeyed the principle on directing the ultrasound beam at the vessel examined at an angle not larger than 25 degrees. The frequency spectrum obtained was used for calculating the medium speed of flow, and the diameter of the vessel found in the tw-dimension picture was used to calculate the surface of the cross section of a vessel. The blood flow was calculated by multiplying the medium flow speed by the surface of the cross-section of a vessel. The flow in the portal vein amounted to 853 +/- 130 ml/min., and the flow in the splenic vein amounted to 327 +/- 141 ml/min. The flow in the portal vein was best correlated with the body mass, and the flow in the splenic vein with the volume of the spleen.
