[Hartmann's resection of the large intestine--a method still useful].

UNLABELLED: Hartmann's procedure is an operation indicated especially in emergency cases for left-sided colonic pathology where primary anastomosis is considered unsafe. METHODS: Retrospective study 95 patients who underwent Hartmann's procedure over a 5 year period in the Surgical Clinic of the University Hospital in Pilsen. RESULTS: 81 (85%) operations were emergency procedures. 45 (47%) operations were done for the malignant disease, the remaining 50 (53%) for benign disease (33 for complications of diverticular disease). Median age of patients was 69.4 years. The 30-day mortality rate was 16.8%. 37 patients had colostomy reversed, after a mean of 5.6 months with no mortality. CONCLUSIONS: Hartmann's procedure is a safe method. Our results are similar to results collected by foreign clinics.

[Esophageal stents].

The method of oesophageal stents application using a surgical procedure via gastrotomy for otherwise surgically unmanageable malignant stenoses of the oesophagus and the gastric cardia, has been used for several decades. Once the self-expandible endoprostheses, applied under either the endoscopical or radiological (RTG) control, or both, and following a change in the oesophageal malignities treatment strategy, the spectrum of indication groups of the patients, who would undergo the stent implantation, have enlarged. Recently, thank to the endoscopically removable endoprostheses, the authors broaden the spectrum of use to benign findings of the oesophageus and the gastric cardia (strictures, perforations, insuficiency of anastomoses), as well. In their work, the authors present a group of patients who have had the oesophageal stent, using the endoscopic method, implanted at the Surgical Clinic of the Faculty Hospital in Plzen since 1992.

[Conversion of laparoscopic cholecystectomy].

Laparoscopic cholecystectomy is nowadays the method of choice in treatment of symptomatic cholecystolithiasis, despite the fact that some operations cannot be completed laparoscopically and are converted. In a group of 431 patients operated during the last two years the authors evaluate the causes and predispositions of conversion which was performed in 7.4%. A significantly higher risk was found in subjects above 65 years of age, in those with ultrasonographic signs of thickening of the gallbladder wall and patients with acute cholecystitis. The risk of conversion is not significantly influenced by the patient's sex, obesity, the condition after endoscopic retrograde cholangiopancreaticography, pancreatitis and a previous abdominal operation.

[Carcinoid of the ileocecal junction with symptoms of acute abdomen].

The authors describe a group of six patients (two men and four women) operated at the Surgical Clinic in Plzen during 1995-2000 on account of a carcinoid of the GIT. The mean age of the patients was 48.6 years, the range 14-77 years. In five instances a carcinoid of the appendix was detected. In a separate case-record they evaluate a carcinoid of the ileocaecal transition with symptoms of acute abdomen, early surgery, its extent incl. the postoperative procedure.

[Injuries of the diaphragm].

The author presents an analysis of 27 injuries of the diaphragm treated at the surgical Clinic of the Faculty Hospital in Plzen in 1969-1992. There was a total of 22 closed injuries, all part of multiple injuries. In five instances an open injury was involved. The total mortality was 22.2%. The authors analyze the causes of injuries of the diaphragm, affections of different parts of the body in relation to multiple injuries and discuss the problem of assessment of the diagnosis. In the diagnosis in addition to common X-ray methods, sonography and CT and in exceptional cases angiography of the hepatic artery proved useful. The authors draw attention to the difference between prolapse of internal organs and post-traumatic diaphragmatic hernia and emphasize the importance of early operation after the diagnosis was established. They discuss approaches to surgical repair, treatment of injured internal organs and the diaphragm.

[Nosocomial infections in surgical intensive care of multiple injuries].

The authors present an abridged version of the final report of a State research programme. By introduction of a series of provisions and expedient use of funds available for research it proved possible to reduce the incidence of nosocomial infections in surgical patients from 14% to 12% and in a group with multiple injuries in the surgical intensive care unit from 18% to 10%.

Disseminated pulmonary adiaspiromycosis identified in bioptic material from the lung.

A disseminated form of pulmonary adiaspiromycosis was estimated in a 48 year old miner. The origin of a hypersensitivity mainly of an immediate allergic type became evident during an immunological examination and was partly indicated also by the tissue reaction and by elevated level of serum IgE (480 I.U.). Of great interest was the finding of a coagulation necrosis in the bioptic material. An examination of functions disclosed signs of a moderate obstruction which was latent when first observed with normal value of lung diffusion capacity. Its activation during stress was indicated by an increase in the value both of the residual volume and the IGV to the upper border of their limits. No apparent changes occurred in the disease during our two-year-investigation. Neither the total state of the patient nor the x-ray picture reflected the effect of a prolonged treatment with antituberculotics.
