[Allogenic transplantation of hematopoietic stem cells in the treatment of children with high-risk acute leukemia].

BACKGROUND: Most children with acute lymphoblastic leukemia (ALL) and increasing number of children with acute myelogenous leukemia (AML) are currently cured with conventional chemotherapy. Despite of this success there is a subset of patients with high-risk features at diagnosis who are predisposed to a very high risk of relapse. Relapse of AML and early bone marrow relapse of ALL can not be cured by conventional chemotherapy. Allogeneic hematopoietic stem cell transplantation (HSCT) is therapeutic option in these children with very high-risk acute leukemia. METHODS AND RESULTS: Between XI/1989-XII/1996 33 children with acute leukemia (ALL: 22, AML: 11) underwent an allogeneic HSCT from HLA identical related donors (HLA-identical sibling: 30, twin: 1, other HLA-identical relative: 2) at the 2nd Dept. of Pediatrics, University Hospital Motol. Median age of our group was 9 years (1.5-19 y.), boys (n = 23) clearly dominated over the girls (n = 10). The resource of stem cells was bone marrow in 31 children, bone marrow and peripheral blood progenitor cells (PBPC) and PBPC in one child respectively. Myeloablative conditioning regimen varied, consisting of total body irradiation and chemotherapy in 21 children and chemotherapy in 12 children. HSCT was performed in first complete remission of acute leukemia in 9 children (AML: 7, ALL: 2), in second remission in 14 children (AML: 2, ALL: 12), in third remission in 4 children (ALL: 4). Six children underwent HSCT in first partial remission (n = 1) and in second (n = 4) or third (n = 1) chemoresistant relapse. Seven (21%) children died due to post-transplant complications. Nine (28%) children suffered from clinically significant acute graft-versus-host reaction (GVH) and 15% (4/27) children who survived 100 days post-transplant suffered from chronic GVH disease. Relapse of leukemia was diagnosed in 39% (12/31) children. Fourteen (42%) children are alive and well in continuous remission with median follow-up 42 months. CONCLUSIONS: Allogeneic HSCT can cure children with very high-risk acute leukemia in the situations where conventional chemotherapy fails. Relapse of leukemia and GVH reaction are most important causes of post-transplant morbidity and mortality.

[A registry of potential bone marrow donors].

Transplantation of bone marrow is relatively successful treatment in some haematological diseases. The most suitable donor of bone marrow is a HLA genotypic identical sibling; however, roughly only 30% of patients have a suitable related donor. In order to find a HLA identical non-related donor for a maximum number of patients it is necessary to create as extensive registers as possible of potential bone marrow donors. The authors mention the possibility to obtain these donors from the ranks of regular blood donors and they mention the advantages of obtaining a register from this group of subjects.

[The effect of thrombocytapheresis on hematologic, biochemical and immunologic indicators].

The authors investigated in 33 separate donors the influence of thrombocytapheresis on haematological, biochemical and some immunological parameters. The paired t-test revealed a statistically significant increase of values of total proteins, aspartate aminotransferase, immunoglobulin A, alpha 1 antitrypsin, transferrin and the C4 component of complement seven days after apheresis and a significant increase of thrombocyte, total protein, alkaline phosphatase, immunoglobulin A, orosomucoid, and transferrin levels two weeks after apheresis. A significant decrease of haemoglobin values was recorded 7 and 14 days after apheresis. As the revealed changes of the investigated parameters are within the physiological range of the healthy adult population, the authors conclude that thrombocytapheresis does not have an adverse effect on the health status of donors.

[The significance of the thrombocyte crossmatching test].

The authors investigated the dependence of the results of the crossover test with thrombocytes and lymphocytes of the donor and serum of the recipient on the rise of thrombocytes 1 and 24 hours following administration of a thrombocyte concentrate. The clinical condition of the patient at the time of thrombocyte administration was taken into account. The donor of thrombocytes isolated in a blood element separator was selected from a panel of cca 800 subjects with previously assessed HLA antigens. A total of 36 administered concentrates was evaluated. From the assembled findings it may be concluded that the result of thromboconcentrate transfusion is significantly influenced by adverse clinical factors in the patient such as splenomegaly, haemorrhage, febrile conditions. A positive crossover lymphocytotoxic test can predict reliably the ineffectiveness of thrombocyte administration. A positive crossover test with the donor's thrombocytes does not affect significantly the success of the transfusion. It seems thus that for the fate of administered thrombocytes the presence of HLA antibodies is decisive and not the presence of specific thrombocytic antibodies.

[Therapeutic effectiveness of thrombocyte concentrates in relation to the results of the lymphocytotoxicity test].

The authors analyze the effect of transfusions of 211 thrombocyte concentrates to polytransfused patients in relation to the results of the crossover test between donor and recipient, using the lymphocytotoxic test (LCT). In negative LCT tests the adequate and inadequate effect was 50%. After elimination of non-immune influences (splenomegaly, febrile temperature) the percentage of adequate effects rose to 70%. In LCT positive results the effect of administration of thrombocyte concentrates declined to 30%. The authors discuss practical possibilities and ways to select suitable donors to achieve an optimal therapeutic effect of substitution therapy.

Examination of serum ferritin and erythrocyte ferritin--its role in the blood transfusion service.

Blood donors were examined for serum ferritin values and concentration of ferritin in the erythrocytes. The group of male and female donors without previous donations showed average values of 102.27 ng and 51.75 ng of ferritin per one ml of serum, respectively. Males with over 20 donations had 68.04 ng ferritin per one ml, females 37.14 ng of ferritin per one ml. The reduced serum ferritin values in multiple male and female donors is statistically significant. Serum ferritin values in women of the two groups are lower than those of males, the difference also being statistically significant. In male and female blood donors, irrespective of the number of donations, average values of 13.74 ag and 12.07 ag of ferritin per erythrocyte, respectively, were established. The difference in ferritin concentration in the erythrocytes between males and females is statistically insignificant. The correlation coefficient failed to demonstrate any dependence between erythrocyte ferritin concentration and concentration of ferritin in the serum. The object of serum ferritin determination in blood donors is to detect the earliest stage of storage iron deficiency in the organism. For the latter purpose, the determination of erythrocyte ferritin is ineffective.

[Comparison of the Haemonetics V50, Fenwal CS 3000 and IBM 2997 blood separators].

Haemonetics V 50 (HV-50), Fenwal CS 3000 (FCS-3000) and IBM 2997 separators are compared according to the results of donor plateletaphereses. The mean platelet yield of HV-50, FCS-3000 and IBM-2997 was 464 X 10(9), 551 X 10(9) and 468 X 10(9) respectively (the difference both between FCS-3000 and HV-50 and between FCS-3000 and IBM-2997 being significant - p less than 0.01). The median leukocyte contamination of platelet concentrates was 0.3 X 10(9), 0.2 X 10(9) and 5.3 X 10(9) respectively (the difference between FCS-3000 and HV-50 being not significant). The separators are further compared on the basis of platelet concentrate volume, erythrocyte contamination of thromboconcentrates, processing time, donor and staff convenience, our experience concerning product reliability and service quality, and finally the price of both the separators and the disposables.

[The Apple II e microcomputer as a databank for registering special donors].

The authors introduced in the Institute of Haematology and Blood Transfusion in Prague records of special donors, voluntary donors of bone marrow and voluntary donors for the separator using the microcomputer Apple II and the system ArchTex. These provisions make it possible to select a suitable donor much more quickly than by the hitherto used system of cards which, however, still are essential for the work of the special donor station.

Serum ferritin estimation in blood donors subject to apheresis on IBM 2997 cell separator.

The serum ferritin concentration was tested in blood donors selected for apheresis on IBM Cell Separator. Of 41 donors, 28 were males and 13 females. All donors exhibited normal hemoglobin and RBC. The mean value of serum ferritin was 90.93 ng/ml in males and 48.38 ng/ml in females. In males--often with repeated blood donations--a low value of serum ferritin suggesting reduced Fe stores in organism was found in one individual only. In contrast, reduced serum ferritin levels were observed in 4 females who, before apheresis, regularly donated blood or had several pregnancies in their anamnesis. The obtained results point to prelatent or latent sideropenia. Serum ferritin concentration was measured in 18 donors selected for apheresis. The examination of ferritin was performed prior to, immediately after, and one week post separation. No significant changes in serum ferritin concentrations due to the separation procedure were observed. Preventive tests of serum ferritin in multiple blood donations and in women with previous pregnancies are recommended.
