Discordant anatomically corrected malposition of the great vessels and Ebstein's malformation: fetal and neonatal findings.

Discordant anatomically corrected malposition of the great vessels (ACM) is a rare congenital cardiac malformation that is often associated with other cardiac lesions. To our knowledge, a fetal diagnosis of this lesion has not yet been reported in the literature. We present a case of ACM associated with Ebstein's malformation, diagnosed by fetal echocardiography.

Tranquilliser use as a risk factor for falls in hospital patients.

This study looked at associations of tranquilliser use and falls risk in a hospital population of confused and nonconfused patients. In a prospective observational study in a rehabilitation hospital for elderly patients, we followed 1025 consecutive patients. The number of fallers, recurrent fallers and total falls was recorded. Confused patients (p < 0.0001) and patients on tranquillisers (p = 0.001) were significantly more likely to fall than nonconfused patients and patients off tranquillisers. Confused patients on tranquillisers were significantly more likely to have recurrent falls (p = 0.026) when compared with confused patients off tranquillisers. The risk was apparent from admission, persisting throughout the first 30 days of stay. This was not noted for nonconfused patients. We identified a stratification of risk for falls with nonsignificant trends for confused and nonconfused patients on tranquillisers to be fallers and to have more falls compared with patients off tranquillisers. These data are associational and do not necessarily imply causality. There is however no evidence to recommend the routine withdrawal of tranquillisers from all patients. Any future research needs to include confused patients.

Electrochemical organization of monolayer protected gold nanoclusters on single-walled carbon nanotubes: significantly enhanced double layer capacitance.

This paper reports a novel electrochemical route for anchoring monolayer protected gold nanoclusters (size 8 +/- 0.2 nm) on single-walled carbon nanotube bundles, resulting in the formation of hybrid materials. Monolayer protected gold nanoclusters prepared by modified Brust synthesis route were organized on SWNT bundles by cycling the potential in dichloromethane between -1 to +1 V at a scan rate of 50 mV/s. Monolayer protected nanoclusters in electrolyte solutions possess ionic space charge around them (double layer charging), making them suitable for organization on nanotube bundles, by tuning the electrostatic interactions. More significantly, analysis of the double layer capacitance of these hybrid materials shows almost ten times increase in capacitance compared to that of bare SWNT bundles. We believe that these hybrid materials are potentially useful in nanoelectronics.

Variability in the coat protein gene of Papaya ringspot virus isolates from multiple locations in India.

The coat protein (CP) sequences of eleven Papaya ringspot virus (PRSV) isolates originating from different locations in India were determined, analysed and compared with the sequences of other isolates of PRSV. The virus isolates from India exhibited considerable heterogeneity in the CP sequences. The CP-coding region varied in size from 840-858 nucleotides, encoding protein of 280-286 amino acids. Comparative sequence analysis revealed that the PRSV isolates originating from India were divergent up to 11%. Though the PRSV isolates were differentiated in to two clusters, yet the sequence variation could not be correlated with the geographical origin of the isolates. Implication of the sequence variation in the coat protein derived transgenic resistance in papaya is discussed.

The impact of cognitive impairment on upper body dressing difficulties after stroke: a video analysis of patterns of recovery.

OBJECTIVE: to study the underlying cognitive deficits influencing a stroke patient's ability to relearn to dress. The aim was to investigate how recovery had occurred and whether the nature of cognitive impairment was the reason for persistent dressing problems. METHODS: the dressing performance of 30 stroke patients was compared at the sub-acute stage and three months later. Standardised cognitive and physical tests were carried out, together with a video analysis of patients putting on a polo shirt. RESULTS: thirteen patients with preserved power in the upper limb used both arms to put on the shirt. Despite visuospatial impairment or apraxia in some cases, all were successful given sufficient time. Out of 17 patients with arm paresis, 12 were dependent putting on the shirt. Amongst the five who were independent, significantly fewer cases of cognitive impairment were seen on tests for apraxia (p<0.05) and visuospatial perception (p<0.05). Video analysis confirmed the importance of cognitive problems such as neglect or apraxia. Three patients who failed shirt dressing showed neglect or apraxia at follow up and had persistent arm paresis. Test failures also occurred amongst those who were independent. DISCUSSION: cognitive impairment affected patients attempting to relearn to dress with one hand, but did not affect patients who used both hands. The three patients who remained impaired on cognitive tests at follow up were unable to adapt or learn any compensatory strategies. The influence of cognition on a person's ability to learn compensatory strategies has implications for the design of rehabilitation therapies.

Secondary bone grafting in cleft lip and palate with eruption of tooth into the graft: a case report.

Secondary bone grafting in cleft lip and palate patients is performed preferably before the eruption of permanent canine in order to provide adequate periodontal support for eruption and preservation of the teeth adjacent to the cleft. Presented here with is a case of unilateral cleft lip and palate, which was followed up from birth to 15 years of age. The role of an orthodontist in the team approach for management of such anomalies is described. Also discussed in detail is the entire range of treatment procedures the child underwent, especially the role of secondary bone grafting.

Hydrocephalus in a hydropic fetus with Turner syndrome: a rare association.

We report a rare case of antenatally diagnosed Turner syndrome associated with hydrocephalus. In addition, the fetus had cystic hygroma, hydrops fetalis and hypoplastic left heart syndrome. Pregnancy was terminated because of the poor outcome of the above-mentioned anomalies. A possible mechanism for the development of hydrocephalus is discussed.

Sleep disorders in Parkinson's disease.

Sleep quality is one of the major sources of dissatisfaction among patients with Parkinson's disease (PD). Insomnia, parasomnia and daytime sleep disorders are all common. The motor problems accompanying PD are well studied and documented, yet little is known about sleep and the other non-motor problems. Dopaminergic medications, the neurochemical and neurodegenerative changes may all contribute to the pathogenesis of sleep disorders in PD. Subjective or objective sleepiness assessment should routinely be performed by physicians looking after PD patients. Patients should be informed of the risks associated with excessive daytime sleepiness. Management is difficult and should be targeted to the specific sleep disorder and its likely cause. Simple sleep measures such as sleep hygiene should be tried first before pharmacological treatment is initiated.

Berry syndrome with trisomy 13.

We report on 2-day-old neonate with trisomy 13 with coexistent distal aortopulmonary septal defect, aortic origin of the right pulmonary artery, interrupted aortic arch, intact ventricular septum, and a patent ductus arteriosus diagnosed by two-dimensional and color Doppler echocardiography. Review of the literature reveals that this patient is the 24th reported case of Berry syndrome and the first case of this unusual combination of cardiovascular defects associated with trisomy 13 syndrome.

Kinetics of phase separation in polymer-solvent mixtures.

We investigate the kinetics of phase separation in systems with a strong asymmetry in the mobility. This simple model system mimics the segregation kinetics of polymer-solvent mixtures, where the polymer-rich phase forms a low-mobility gel. We obtain detailed numerical results for this model, both without and with thermal noise. In the absence of thermal noise, we find that domain growth is slowed down drastically when the low-mobility phase percolates the system. However, thermal noise restores rapid domain growth through a Brownian coalescence mechanism.

Specific roles of NMDA and AMPA receptors in direction-selective and spatial phase-selective responses in visual cortex.

Cells in the superficial layers of primary visual cortex (area 17) are distinguished by feedforward input from thalamic-recipient layers and by massive recurrent excitatory connections between neighboring cells. The connections use glutamate as transmitter, and the postsynaptic cells contain both NMDA and AMPA receptors. The possible role of these receptor types in generating emergent responses of neurons in the superficial cortical layers is unknown. Here, we show that NMDA and AMPA receptors are both involved in the generation of direction-selective responses in layer 2/3 cells of area 17 in cats. NMDA receptors contribute prominently to responses in the preferred direction, and their contribution to responses in the nonpreferred direction is reduced significantly by GABAergic inhibition. AMPA receptors decrease spatial phase-selective simple cell responses and generate phase-invariant complex cell responses.

Congenital adrenal hyperplasia presenting as hematuria and acute renal faliure.

We present a neonate who presented with hematuria and acute renal failure. Classical 21 hydroxylase deficiency was diagnosed on the basis of features of salt wasting, response to treatment with corticosteroids and mineralocorticoids and a positive ACTH stimulation test. Renal vein thrombosis secondary to hemoconcentration due to salt wasting was attributed as the cause of hematuria. Follow-up revealed clinical improvement and normalization of renal parameters. This is the first report of congenital adrenal hyperplasia presenting as hematuria and renal failure to the best of our knowledge.

Anti-inflammatory activity of Dalbergia sissoo leaves.

The possible anti-inflammatory activity of the 90% ethanolic extract of Dalbergia sissoo leaves (DSELE) was studied in different models of inflammation in rats after oral administration at doses of 100, 300 and 1000 mg/kg. DSELE significantly inhibited carrageenin, kaolin and nystatin-induced paw oedema, as well as the weight of granuloma induced by a cotton pellet. It also inhibited dye leakage in acetic acid-induced vascular permeability test in mice. DSELE was devoid of ulcerogenic effect on the gastric mucosa of rats in acute and chronic tests. In acute toxicity studies, it was found to be safe up to 10.125 g/kg, p.o. in the rat. It was concluded that the D. sissoo leaf extract possessed significant anti-inflammatory activity (in acute, sub-acute and chronic models of inflammation) without any side effect on gastric mucosa.

Effect of nutrients supplementation on anaerobic sludge development and activity for treating distillery effluent.

Startup of laboratory anaerobic reactors and treatment efficiency were investigated by supplementing the distillery effluent feed with macronutrients (Ca, P) and micronutrients (Ni, Fe and Co) under mesophilic conditions. Calcium and phosphate were deterimental to the treatment efficiency and sludge granulation. Traces of salts of iron, nickel and cobalt, individually and in combinations improved the COD removal efficiency and sludge granulation process.

Induction of visual orientation modules in auditory cortex.

Modules of neurons sharing a common property are a basic organizational feature of mammalian sensory cortex. Primary visual cortex (V1) is characterized by orientation modules--groups of cells that share a preferred stimulus orientation--which are organized into a highly ordered orientation map. Here we show that in ferrets in which retinal projections are routed into the auditory pathway, visually responsive neurons in 'rewired' primary auditory cortex are also organized into orientation modules. The orientation tuning of neurons within these modules is comparable to the tuning of cells in V1 but the orientation map is less orderly. Horizontal connections in rewired cortex are more patchy and periodic than connections in normal auditory cortex, but less so than connections in V1. These data show that afferent activity has a profound influence on diverse components of cortical circuitry, including thalamocortical and local intracortical connections, which are involved in the generation of orientation tuning, and long-range horizontal connections, which are important in creating an orientation map.

Anomalous colloidal stability of protein coated polystyrene latex beads studied by small angle light scattering.

Results of small angle laser light scattering experiments carried out on polystyrene latex beads coated with lysate of Plasmodium falciparum (Pf) antigen (Ag) and human seropositive sera (10:1 ratio) are reported for various NaCl concentrations (0-300 mM). The protein coated beads showed time-dependent coagulation. The normalised intensity of scattered light I(s)(t)/I(0) showed I(s)(t)/I(0)=1+(Gammat)(delta) behaviour with the coagulation rate, Gamma and exponent, delta showing anomalous dependence on NaCl concentration. The coagulation rate exhibited strong increase up to NaCl concentration of 50 mM, above this and up to 300 mM the coagulation rate was found to remain independent of NaCl concentration yielding non-DLVO behaviour. The same was true for delta which increased from 1.04+/-0.06 to 6.94+/-0.07 as NaCl concentration was raised from 0 to 50 mM. Above 50 mM it remained constant with delta=6.94+/-0.07. Results are discussed through Smoluchowski aggregation kinetics and theoretical construction of interparticle interaction potentials relevant to our problem.
