Aldose reductase and sorbitol dehydrogenase distribution in rat kidney.

The sorbitol pathway catalyzes the conversion of glucose to fructose via the intermediate sorbitol. It consists of aldose reductase (AR) and sorbitol dehydrogenase (SDH). In adult (44 day) kidney zones, AR was highest in the outer medulla. In substructures AR was highest in distal convoluted tubule. The AR was greatest in newborn and 8-day zones of developing rat kidney. Acute alloxan diabetes was associated with decreased AR in small arteries, but not glomeruli. The SDH was lowest in outer medulla. It was most active in glomeruli and distal convoluted tubules. The diabetic state leads to no change of SDH in arteries but an increase in glomeruli. SDH increased with development. This study demonstrates AR and SDH in substructures of the kidney. The pathway is present in developing kidney. In diabetes the enzymatic changes would tend to decrease accumulation of sorbitol.

Non invasive continuous anterior fontanel tension monitoring in neonates.

Anterior fontanel tension (AFT) has been shown to reflect the intracranial pressure (ICP). A new transducer was designed for AFT measurement which overcomes the problems of replacement in subsequent use. Using the transducer AFT was monitored noninvasively over extended periods in about 200 normal and sick neonates. The results indicate that the technique can be used to identify elevated pressure due to different disease conditions and it has a potential to help in understanding physiological and pathological mechanisms causing alterations in the normal cerebrospinal dynamics. Further studies are underway to establish the full potentials of the technique.

Repolarization abnormalities in children with a structurally normal heart and ventricular ectopy.

In adults, increased QT dispersion has been shown to predict arrhythmic risk as well as risk of sudden death in several clinical settings. It is controversial whether QT and JT dispersion are increased in children with ventricular ectopy and a structurally normal heart. We studied two groups of children: 25 patients with ventricular ectopy and 25 healthy children as controls. Standard electrocardiograms were reviewed and dispersions of both corrected QT (QTc) and JT (JTc) intervals were compared. We conclude that QTc and JTc dispersions are significantly increased in children with ventricular ectopy compared to control subjects.

Heterotopic ossification in moyamoya disease: a case report.

Moyamoya disease is a rare disorder of cerebrovascular circulation. A review of the literature failed to reveal the association of heterotopic ossification in patients with this disease; such a case is now presented. The patient described had atraumatic intracranial hemorrhage, was in a coma for a period of time, underwent ventriculostomy and shunt placement, and was left with residual spastic hemiparesis. Evidence of heterotopic ossification around the hemiparetic shoulder and hip was subsequently noted on radiographic studies and was managed conservatively. Heterotopic ossification may be a rare complication of the neurological deficits associated with moyamoya disease and needs careful consideration when joint stiffness persists, despite usual conservative measures.

Septic induced abortion claiming life of a Nepalese woman.

This is a case report of septic induced abortion done at 16 weeks of gestation outside Kathmandu valley who presented to the emergency department of Tribhuvan University Teaching Hospital (TUTH) with peritonitis and septic shock. The case underwent emergency laparotomy and was treated surgically for perforated ileum and uterus by resection and end to end anastomosis of bowel and subtotal hysterectomy. The patient died after 16 hours of operation due to multiple organ failure (MOF) as a sequelae of septic shock. Unsafe abortion remains one of the major causes of maternal mortality in Nepal. Increasing public awareness about hazards of septic abortion and the provisions of law and decentralizing the trained manpower throughout the country would play a pivotal role in decreasing the incidence of septic induced abortion.

Infectious complications of vasectomy: a study of 200 cases.

Semen and urine were cultured prior to vasectomy. Infectious complications only occurred in patients with positive semen and urine cultures and the offending organism was the same as that found in the semen and urine cultures. This evidence for an endogenous cause of postoperative vasectomy infection suggests that semen and urine cultures should be obtained prior to vasectomy. In this manner, the correct antimicrobial agent can be applied as an aid to rapid solution of a postvasectomy infection.
