This report describes the development and characterization of monoclonal antibody EG2.3. Although produced from a fusion that used splenocytes from donor mice immune to bovine salivary mucin (BSM), EG2.3 bound selectively to a number of human tumor cell lines including colon adenocarcinoma LS174T. Therefore, EG2.3 was compared to B72.3, another mucin (TAG-72) binding monoclonal antibody that also binds to LS174T. Like B72.3, EG2.3 reacted with an epitope on TAG-72. However, these two MAbs differed in a number of ways. Treatment of mucin or TAG-72 with periodate did not reduce the binding of EG2.3 to either antigen. In contrast, B72.3 did not react with either periodate treated antigens. Removal of sialic acid from either BSM or TAG-72 compromised the reactivity of both EG2.3 and B72.3. It was concluded that the EG2.3 binding site was distinct from the carbohydrate structure detected by B72.3.
A double blind study compared the efficacy of metronidazole in two doses (20 mg/kg, 10 mg/kg) with placebo in patients with Crohn's disease. One hundred and five patients participated but only 56 completed the 16 week study -21 were withdrawn for deterioration of symptoms, 17 for adverse experiences, and 11 for protocol violation. Significant improvement in disease activity as measured by the Crohn's disease activity index (metronidazole 20 mg/kg, 97 units; metronidazole 10 mg/kg, 67 units; placebo -1 unit, p = 0.002) and serum orosomucoid (metronidazole 20 mg/kg/day, 49; 10 mg/kg/day, 38; placebo, -9, p = 0.001)) were detected. Changes in C reactive protein concentrations did not achieve significance when all three groups were considered but were significant when all metronidazole treated patients were grouped and compared with the placebo treated patients (0.8 v -0.9, p less than 0.05). Although patients receiving metronidazole 20 mg/kg/day had a greater improvement in disease activity than those receiving 10 mg/kg/day (difference 30 units (95% confidence intervals -27-87), the small sample size may have precluded the detection of statistical significance. Preliminary analysis suggests that metronidazole was more effective in patients with disease confined to the large intestine or affecting both small and large bowel than in those with small bowel disease only. There were no differences in remission rates between metronidazole and placebo treated patients. We conclude that metronidazole warrants further assessment in the treatment of patients with active Crohn's disease.
Two hundred and one whiptail lizards, Cnemidophorus spp., from Texas and Colorado (USA), were examined for Mesocestoides sp. tetrathyridia. Eleven (5%) were infected, including three of 58 (5%) C. dixoni, six of 70 (9%) C. gularis septemvittatus, one of 35 (3%) C. marmoratus, and one of 34 (3%) C. tesselatus; four C. inornatus heptagrammus were not infected. In addition, 41 non-cnemidophorine lizards from the same study area were not infected. Free tetrathyridia were found in the body cavity of lizards and encapsulated tetrathyridia were observed in the heart, liver, stomach, mesenteries, ovaries, intestines, and lungs. None of the Mesocestoides sp. exhibited any evidence of asexual proliferation such as multiple scoleces or buds. This note, the fifth in a series of reports on helminths of Cnemidophorus spp., represents the first time Mesocestoides sp. has been reported from these four taxa, and Colorado is a new geographic locality record for this parasite.

Monoclonal antibodies with specificity for the diethylenetriaminepentaacetic acid (DTPA) portion of the GYK-DTPA (glycyl-tyrosyl-lysine-DTPA) linker structure used to chelate metals to immunoglobulins have been prepared. A significant proportion of these antibodies were lambda light chain isotype. Competition assays demonstrated that DTPA, rather than GYK, was the binding site of the antibodies tested. These monoclonal antibodies should be useful reagents for use in assays specific for the presence of the common linker structure used to chelate metallic radioisotopes to monoclonal antibodies. The antibodies tested did not discriminate between chelated and unchelated DTPA.
Quality of life (Q/L) measurements are used to justify or refute different forms of medical treatment; to identify the sequelae of disease or treatment which are resolved by other interventions including nursing and to provide a basis for allocating resources to those treatments judged to be most cost-effective. This paper reviews some of the difficulties in defining and measuring Q/L and the ethical issues raised in relation to their use in allocation of treatment resources. It is suggested that Q/L tests should include a range of dimensions known to contribute to Q/L and that they are applied at different times during the progression of illness and incorporate information obtained from patients. It is essential that weightings by individuals of dimensions investigated in the test are included. Further research is necessary to determine the effects of coping strategies and personality variables on Q/L.

Standardized proportional mortality ratios (PMR) were computed for a population of highway workers. Hazards of highway maintenance work include exposure to solvents, herbicides, asphalt and welding fumes, diesel and auto exhaust, asbestos, abrasive dusts, hazardous material spills, and moving motor vehicles. Underlying cause of death was obtained for 1,570 workers who separated from the California Department of Transportation between 1970 and 1983, and who died in California between 1970 and 1983 (inclusive). Among 1,260 white males, the major findings were statistically significant excesses of cancers of digestive organs (PMR = 128), skin (PMR = 218), lymphopoietic cancer (PMR = 157), benign neoplasms (PMR = 343), motor vehicle accidents (PMR = 141), and suicide (PMR = 154). Black males (N = 66) experienced nonsignificant excesses of cancer of the digestive organs (PMR = 191) and arteriosclerotic heart disease (PMR = 143). Among 168 white females, deaths from lung cancer (PMR = 189) and suicide (PMR = 215) were elevated. White male retirees, a subgroup with 5 or more years of service, experienced excess mortality due to cancers of the colon (PMR = 245), skin (PMR = 738), brain (PMR = 556), and lymphosarcomas and reticulosarcomas (PMR = 514). Deaths from external causes (PMR = 135) and cirrhosis of the liver (PMR = 229) were elevated among white males with a last job in landscape maintenance. White males whose last job was highway maintenance experienced a deficit in mortality from circulatory diseases (PMR = 83) and excess mortality from emphysema (PMR = 250) and motor vehicle accidents (PMR = 196). Further epidemiologic and industrial hygiene studies are needed to confirm the apparent excess mortality and to quantify occupational and nonoccupational exposures. However, reduction of recognized hazards among highway maintenance workers is a prudent precautionary measure.
In 20 normal subjects (N) and 20 asthmatic patients (A) using bronchodilators as needed, the PD40, PD10, PD15, PD20, PD10T, and PD20C were measured (PD being provocation dose, subscript being -% delta in Gaw/VL for PD40, lowest FEV1 for PD20C, and best FEV1 for the remaining PDs; 10T means delta FEV1 greater than -10 percent). For discriminant analysis we used an ad hoc graphic (best case) method, a ceiling method based on highest PD in A and two methods (logistic and linear) which considered PDs in both N and A (PDN and PDA, respectively). The distribution of PDN and PDA had substantial overlap and appeared log normal. The PD15, PD20, and PD20C displayed the smallest mean misclassification error followed by PD10T, PD40, and PD10. The linear and logistic methods produced balanced sensitivity and specificity but, predictably, a misclassification error higher than that of the graphic method. The ceiling method proved unsatisfactory with 100 percent sensitivity but approximately equal to 60 percent specificity. Using linear and logistic methods, the posttest likelihood of asthma could be expressed as a function of its pretest likelihood and level of PD recorded. We concluded that: (1) the most discriminant PDs are PD20, PD15 and PD20c; and (2) new normative data for diagnostic bronchoprovocation are needed, because: (a) when PDA and PDN overlap, the currently used ceiling method leads to a high misclassification rate, while the linear and logistic method based on mathematical model have a better discriminant ability; (b) to separate PDA from PDN and allow the application of the ceiling method, "as-needed bronchodilators" is not a reproducible criterion.

This article presents a case study of the implementation of a continuous quality improvement (CQI) effort in a school of nursing. By using the process of CQI reported in the literature and used in clinical settings, the school has transformed its governance and structure to address issues of faculty, student, administrative, and staff performance and satisfaction, curriculum development, and organizational change. An annual "Report Card" monitors all CQI-related activities and provides structure for the feedback and change that are integral to the process.
