[Antibodies against Cytomegalovirus and Herpes simplex virus in alcoholics and drug addicts].

Anti-CMV and anti-HSV type 1 of the IgG class were tested in 123 consecutive outpatient alcoholics and 95 consecutive drug addicts admitted to a detoxification ward. The prevalence of both viral markers in these groups was not different from the prevalence found among controls, however, antibody titres were significantly higher. Only weak correlation was found between titres of anti-CMV and anti-HSV in tested subjects. The reasons responsible for high titres of anti-CMV and anti-HSV remain to be determined.

[T4 lymphocytes (helper cells) and T8 lymphocytes (cytotoxic and suppressor cells) in patients with asymptomatic and symptomatic HIV infection].

250 determinations of lymphocyte T subsets in 130 HIV infected patients (79 asymptomatic carriers or with lymphadenopathy, 31 ARC- and 20 AIDS-patients) were analyzed as to the percentage, number, and ratio of T4 (helper) and T8 (cytotoxic/suppressor) lymphocytes in sequential clinical stages of HIV infection. Asymptomatic HIV carriers or patients with lymphadenopathy were found to have statistically significant higher counts of erythrocytes, platelets, total lymphocytes, percentage and number of T4 lymphocytes and T4/T8 lymphocyte ratio than the ARC-patients. Persons with ARC in comparison with AIDS-patients were found to have significantly higher values of erythrocytes, platelets, leucocytes, total lymphocytes, T4 lymphocytes, percentage and count of T8 lymphocytes and T4/T8 lymphocyte ratio. In AIDS patients a statistically significant correlation was seen between number of T4 lymphocyte and number of erythrocytes, platelets, total number of lymphocytes and value of T4/T8 lymphocyte ratio.

[Prospective studies of T4 and T8 lymphocyte subpopulations and beta 2-microglobulin in patients with HIV infection].

Sequential changes of percentage and number of T4 (helper) and T8 (cytotoxic/suppressor) lymphocyte subsets and serum beta 2 microglobulin (beta 2m) level were observed in 23 HIV infected patients for a period of 4 months to 3 years. 10 subjects were asymptomatic HIV carriers, 9 had already AIDS at the beginning of the study and 4 patients developed clinically overt disease during the observation period. T4 lymphocyte number remained in normal range in asymptomatic HIV carriers but in 4 patients its decrease preceded the development of clinical symptoms of infection. A rapid decrease of T4 lymphocyte number was generally observed in AIDS patients. Serum concentration of beta 2m remained at relatively low and constant level in asymptomatic HIV carriers but in 4 cases its increase preceded the appearance of clinical symptoms. In patients with clinically overt disease beta 2m levels were significantly increased and reached the highest values just before death. A correlation between clinical stage of infection and the number of T4 lymphocytes and beta 2 m level was found.

[Influence of therapy with glucocorticosteroids on clinical status, t4 lymphocyte count and t4/t8 lymphocyte ratio in people infected with AIDS].

The influence of glucocorticosteroids (GKS) therapy on clinical status, T4 lymphocyte count in peripheral blood and T4/T8 lymphocyte ratio was investigated in five HIV infected patients: two were asymptomatic, while three had clinically overt disease. The reasons for GKS therapy were: thrombocytopenia in two patients, pancytopenia in two and sepsis with severe endocarditis in one. No influence of GKS on the clinical course of HIV infection was observed. The prospective determinations of T4 lymphocyte count and T4/T8 lymphocyte ratio were relatively constant in two cases, in one case increased and in two patients, one of whom had AIDS, a decrease in both parameters was observed. It seems that GKS therapy does not seriously influence the course of HIV infection in most patients and can be instituted without risk of severe worsening of the immune status.

[Occurrence of HIV antigens in patients with acquired immunodeficiency syndrome].

A total of 74 serum samples from 37 AIDS patients were analysed for HIV antigen and results were correlated with duration of the disease, predominant symptoms, T4 lymphocyte count in peripheral blood, and antiviral therapy. The prevalence of HIV-Ag increased with duration of AIDS and T4 lymphocyte depletion. It was also higher in patients who were not treated with antiviral therapy than in treated persons. No correlation was found between HIV-Ag and clinical picture of AIDS. HIV antigenemia indicates a poor prognosis and can be regarded as an additional parameter of monitoring of AIDS patients.

[Liver damage in alcoholics].

The epidemiology of alcohol abuse and liver disease among alcoholics, with special emphasis on Poland, was reviewed and liver morphological changes attributable to the action of alcohol discussed. Furthermore, possible mechanisms leading to steatosis, alcoholic hepatitis and cirrhosis were discussed in detail followed by brief review of clinical and biochemical abnormalities accompanying alcoholic liver disease and therapy of this disorder.

[Titer of anti-P24 and anti-GP41 antibodies as a diagnostic parameter in patients with asymptomatic and symptomatic HIV infections].

Anti-p24 and anti-gp41 antibody titers were determined in sera from 20 asymptomatic HIV carriers and 15 AIDS patients. Anti-p24 titer was found to be significantly lower in AIDS patients than in asymptomatic HIV carriers and anti-gp41 titer was similar in both investigated groups. A correlation was found between T4/T8 lymphocyte ratio and anti-p24 antibody titer in patients with AIDS.

[Delta virus infection among drug addicts].

Altogether 240 unselected intravenous drug addicts were tested for the presence of infection with delta virus (HDV). Anti-delta were found in 16 (16%) out of 100 drug addicts tested in the years 1988-1989 and in only one drug addict (0.7%) out of 140 tested in the years 1985-1986. It seems that delta virus has been introduced to Warsaw drug community in the mid-eighties and has spread enormously since that time.

Affinity of anti-HIV antibodies in patients with asymptomatic and symptomatic HIV infection.

A novel method based on the dose-response curve of antibodies was used for the assessment of the affinity of anti-HIV antibodies in 15 males with asymptomatic or symptomatic HIV infection. The subjects were followed up for a period of 1 to 5 years. No significant correlation between antibody affinity and the clinical stage of infection was observed, however, the computed affinity values increased slightly in most of the asymptomatic HIV carriers and remained relatively constant in AIDS patients. A distinct decrease of anti-HIV affinity was observed in 2 patients with advanced AIDS.

Acute non-A, non-B hepatitis in a hospital population in Warsaw, Poland: clinical and epidemiological aspects.

In the years 1986-1988, 530 consecutive cases of acute viral hepatitis were admitted to an infectious disease hospital in Warsaw. Hepatitis A was diagnosed in 34 cases (6.4%), hepatitis B in 436 (82.3%), CMV infection in five (0.9%), and 55 cases (10.4%) were classified as hepatitis non-A, non-B. In 47 cases (85%) of non-A, non-B hepatitis, there was evidence of parenteral transmission, whereas in eight (15%), the source of infection was unknown. Chronic liver disease developed in 50% of patients with parenteral exposure, but there were no cases with known exposure. Seroconversion to anti-HCV was observed only in patients with parenteral exposure, and 60% of followed up cases developed antibodies within 6 months.

[Non-A, non-B hepatitis--epidemiologic and clinical aspects].

In the years 1986-90 530 consecutive patients with acute hepatitis were hospitalized in the Department of Hepatology of the Institute of Infectious and Parasitic Diseases in Warsaw. Hepatitis A was diagnosed in 34 cases (6.4%), hepatitis B in 436 (82.3%), CMV infection in 5 (0.9%) and 55 cases (10.4%) were classified as hepatitis non-A, non-B. In 47 cases (85%) of non-A, non-B hepatitis in 8 (15%) the source of infection was unclear. In 50% of patients with parenteral exposure transaminases remained elevated for longer than 6 months, while all patients with no known exposure recovered.

[Application of DNA molecular hybridization for detection of hepatitis b virus in diagnosis of the infection].

The principles of new method of detection of hepatitis B virus DNA in serum and liver tissue from infected patients, the molecular hybridization test, are presented in this paper. It also contains data concerning comparison of this method with other serological markers in patients with acute of chronic viral hepatitis, liver cirrhosis and hepatoma, or asymptomatic carriers of HBV.

[Serological markers of hepatitis b and hepatitis d among individuals with symptomatic and asymptomatic HIV infection].

Serological markers of present or past HBV infection were looked for in 69 persons infected with HIV. 40 out of them were clinically asymptomatic, while 29 showed evidence of clinically overt disease related to HIV infection. Most of them were homo- or bisexual men, prostitutes or intravenous drug abusers. Control group consisted of 247 anti-HIV negative persons with the same behavioural patterns. Serological markers compatible with present or past HBV infection were found in 18 out of 40 (45%) asymptomatic HIV carriers, in 23 out of 29 (79.3%) patients with clinically overt HIV infection, and in 82 out of 247 (33.2%) anti-HIV negative persons from the control group. Presented data show that HIV-infected clinically overt individuals have a higher prevalence of HBV markers than asymptomatic carriers of HIV or persons without anti-HIV belonging to risk groups. Therefore, HBV could be a factor influencing outcome of HIV infection.

[Beta-2 microglobulin as a non-specific marker of immunodeficiency in individuals infected with human immunodeficiency virus (HIV)].

Beta-2-microglobulin (beta 2m) is a polypeptide composing HLA antigens on the surface of nucleated cells. Its serum concentration is increased mainly in patients with lymphoproliferative disorders and during viral infections, and reflects probably accelerated activation and turnover of T cells. In this paper we report on abnormalities of beta 2m serum levels in HIV-infected patients. 17 asymptomatic HIV-carriers and 16 persons with clinically overt disease were examined and compared with 20 healthy controls. Patients with confirmed AIDS or ARC were found to have significantly raised beta 2m levels. We found elevated values of beta 2m in asymptomatic HIV-carriers even with normal T4/T8 lymphocytes ratio. There were statistically significant differences in mean beta 2m values between these groups and healthy individuals. beta 2m can be considered as an early, non-specific marker of HIV infection, even in the absence of clinical manifestations of AIDS.

A follow-up study of an outbreak of non-A, non-B hepatitis in a plasmapheresis unit.

A cluster of acute non-A, non-B hepatitis comprising 12 blood donors was diagnosed in a plasmapheresis unit. Nine cases were followed-up for 2-5.5 years and seven out of them progressed to chronicity, as judged by biochemical abnormalities. In six, liver biopsy was performed 1 year after the acute disease revealing chronic active hepatitis in two, chronic persistent hepatitis in two, chronic lobular hepatitis in one and normal liver in one. Repeated biopsies showed progression to cirrhosis in one case of chronic active hepatitis, and resolution of the disease in another one, while in the remaining patients liver morphology remained unchanged. Circumstantial epidemiologic evidence suggests a single agent being the cause of the outbreak, which resulted in a broad spectrum of liver disease.

Liver disease among Polish alcoholics. Contribution of chronic active hepatitis to liver pathology.

A group of 70 chronic alcoholics--65 men and 6 women--has been studied. HBsAg was found in 11 (16%), and at least one marker of HBV infection was present in sera from 31 (44%) persons, these results being significantly higher than in a matched control population. The commonest single histological patterns were: steatosis (18 cases), and alcoholic hepatitis (18 cases), followed by normal liver (14 cases) and chronic active hepatitis (12 cases). Cirrhosis was diagnosed in only 4 cases. Five cases of chronic active hepatitis could be attributed to infection with HBV or HDV; in the remaining 7 cases the etiology was unclear. Infection with HBV seems to play an important role as the cause of liver disease among alcoholics in Poland, and chronic active hepatitis of various etiology may be an important form of liver pathology among them.
