[Long term observation of children with Giardia intestinalis invasion treated with metronidazole and furazolidone].

The results of a long-term (3-year) follow-up of children infected with G. intestinalis treated with metronidazole and furazolidone are presented. Therapy was effective in 84-95% of cases, depending of the duration or the follow-up. Repetition of therapy with tinidazole was indicated in 5% of children after 6 months and in 15% of children after 1 and 2 years of observation because of persisting symptoms of the infection or recurrence. More frequent the treatment was necessary in children under 3 years of age. Some (14%) children required milk free diet and in a few cases (underlying disease--coeliac) gluten-free diet or a diet free of main allergens.

[Evaluation of selected parameters in calcium and phosphate metabolism in acute liver failure following poisoning with Amanita phalloides].

Serum calcium, phosphate and 25-OHD8 levels as well as calcium and phosphate urine excretion were determined in 21 children between 5 and 7 days after poisoning with Amanita phalloides. Hypophosphatemia was seen in children with the symptoms of moderate intoxication whereas hypocalcemia in severely poisoned patients. Phosphates deficit should be corrected early in all patients poisoned with Amanita phalloides.

Value of bile acid determination for the diagnosis of neonatal jaundice.

Serum concentrations of bile acids and bilirubin, and activity of alanine transferase and alkaline phosphatase as well as bile acid and bilirubin levels in duodenal contents were determined in 90 infants aged 1-44 weeks (including 49 under 10 weeks of age) admitted to hospital for prolonged jaundice. Infants with extrahepatic cholestasis were found to have statistically higher serum bile acid and bilirubin concentrations. Oral administration of cholestyramine produced a statistically significant decrease in serum bile acids and bilirubin in infants with intrahepatic cholestasis under 10 weeks of age. In 24 out of the 30 infants with biliary tract obstruction total absence of bile acids in the duodenal contents was demonstrated while in the others the concentration did not exceed 0.2 mmol/l. The mean bile acid concentration in infants with intrahepatic cholestasis was 2.81 mmol/l while in 8 infants out of the 60 bile acids were either absent or present in trace amounts. The method had an 84.4% sensitivity.

[Humoral response of the immunologic secretory system of the small intestine in children infected with Giardia intestinalis].

Local humoral response of the intestinal mucosa was determined with secretory IgA levels and secretory component activity in enterocytes and duodenal content of 15 children infected with G. intestinalis. The obtained results were compared to those in 5 children with coeliac disease and 12 children with diarrhoea without lambliasis. Secretory IgA was increased in about 50% of children with lambliasis (in the remaining groups in 25% of children) to the values higher than that in the comparative groups. Secretory component activity was relatively high in the intestinal epithelium. Secretory component activity in the duodenal content was high in about 40% of children independently of the examined group. No correlation between the said variables was noted except positive correlation of secretory IgA levels and secretory component activity in the bile.

["Munchausen syndrome by proxy" as an example of diagnostic error].

A case of the girl who underwent multiple hospitalizations is presented. Gastrointestinal disorders were seen in the infancy together with skin rash of allergic type, hypoglycaemia without any clear reason in the fourth year of age, and polyuria with hyponatremia and hypokalemia since the sixth year of age. Mother's lack of concern was unexplainable in view of the deteriorating child's health. Samples of the urine and faeces supplied by the mother have shown the laboratory findings suggesting that potassium chloride was added to the faeces and natrium hydrocarbonate--to the urine. Urine collected during polyuria contained large quantities of furosemide. Long-term follow-up, numerous examinations and performed tests have led to the diagnosis of the particular form of the ill-treated child syndrome, so-called "Munchausenn by proxy" syndrome.

Campylobacter pylori in upper digestive tract diseases in children.

Results of studies on the frequency of the occurrence of the bacteria, Campylobacter pylori, in the mucous membrane of the gastric antrum are presented in this paper. The study was carried out on 61 children treated for chronic abdominal pain. The diagnosis was established on the basis of flexible endoscopy and histology of antral biopsies. The presence of Campylobacter pylori was determined using the CLO-test. The positive CLO-test was obtained in 87.5% of children with gastritis, in 75% of children with duodenal ulcer and in only 17% of children from control group (p less than 0.001). Four-week therapy with De Nol eradicated Campylobacter pylori in 60% of treated children, and reduced infection in the next 25%. These bacteria seem to play an important role in the pathogenesis of chronic gastritis and peptic ulcer disease.

[Autoimmune chronic hepatitis in children].

Autoimmune chronic hepatitis is a clinical syndrome of unknown aetiology with serious prognosis. The disease poses also important problems. During 4 years 11 children with autoimmune chronic hepatitis were observed, among them 10 girls and 1 boy aged from 8 to 16 years. The diagnosis was based on histological examinations, clinical symptoms and biochemical investigations. In all cases the level of gamma-globulin was raised over 20 g/l, with increased concentrations of immunoglobulin G and bile acids, and increased A1AT activity. Abnormal results of the antipyrin test were found in 4 children, and presence of tissue antibodies was demonstrated in 8 children. All children were treated with prednisone. After 2 years of treatment control histological examinations were done in 5 cases. Despite the use of glycocorticosteroids full remission was not obtained in any case. This observation agrees with literature data indicating that autoimmune chronic hepatitis is a long-standing process leading to cirrhosis.
