The relative influence of distant and local (DEW-line) PCB sources in the Canadian Arctic.

Soil PCB contamination has been delineated at 18 of 21 Distant Early Warning Line (DEW-line) stations being cleaned up by the Canadian Department of National Defence (DND). As a result, detailed surface soil delineation data has been reported for contamination exceeding 1 microg/g (dw total Aroclor), which is the remedial criteria for PCB contaminated soil under the DEW-line cleanup project. The results of this delineation work has allowed us to estimate the mass of PCB contained in surface soil at these sites and to quantify the DEW-line as a source of PCBs to both local and Arctic wide contamination. Our analysis of DEW-line cleanup delineation reports suggests that pre-cleanup surface soils (top 10 cm) with over 1 microg/g PCB constituted a source of PCBs that ranged from 0.8 to 43 kg with a mean of 18 kg. The total mass of PCB at all 18 sites was 119 kg. Previous studies have described a "halo-effect" that surrounds DEW-line sites, whereby PCB signatures in soil and plants up to 10 km from source areas were attributed to the local source. At Cambridge Bay (CAM-M), Nunavut, our inventory of PCB sources and redistribution suggests that up to 3.4 kg of PCB were exported from the site to the surrounding tundra prior to cleanup. The primary mechanism of transportation appears to be wind borne particulate. Potential vapour phase emissions of PCB from contaminated soil at DEW-line sites appears to have been negligible.

[An unusual case of the otogenic purulent thrombosis of the sigmoid sinus and internal jugular vein].

The authors present a case of 51 years old female suffering from the purulent thrombophlebitis of the sigmoid sinus and internal jugular vein. The disease developed in the course of chronic otitis media. During first 2 weeks she was treated with antibiotics because of the septic fever without significant otogenic symptoms. An incidental otolaryngologic examination showed the reason of the illness. The authors emphasise the thrombophlebitis process extended from transverse sinus to the lower end of internal jugular vein. Nowadays the otogenic thrombophlebitis complications are rare, but the knowledge of them is indispensable in the everyday practice. The authors emphasise the necessity of cooperation otolaryngologists with other specialists. Especially it is important in the Polish new model of health service, which place the main role of the treatment to general practitioners.

[Teratoma in the middle ear and throat of an infant].

The authors present a case of an 8-moth-old female infant treated for prolonged otitis media. The inflammatory process was connected with teratoma of tympanum and throat. The authors discuss presumable connection of the both masses. The child was completely cured by surgical treatment.

[Histiocytosis X (Langerhans cell histiocytosis) of temporal bone. Role of scintigraphy in disease progress evaluation].

The authors present a case report of a 7-year-old girl with Langerhans cell histiocytosis of temporal bone. Edema and bone destruction involved zygomatic process of temporal bone and temporal fossa without ear occupation. The role of picturing diagnosis in evaluation of lesion extent with special importance of scintigraphy is discussed. Treatment methods of Langerhans cell histiocytosis lesions in temporal bone are presented. In our case locally Depo-Medrol injection was given and radiation was left as last resort management.

[Adenolymphoma (Warthin's tumor) of the larynx: coexistence with the bilateral laryngocele. Contribution to differential diagnosis with oncocytic papillary cystadenoma].

Adenolymphoma is a benign monomorphic lesion composed of epithelial tissue. Usually it is found in parotid gland; however, its laryngeal occurrence was rarely described. Extraparotid Warthin's tumour as scanty lymphatic tissue. In cases when there is no lymphatic tissue in subepithelial layer it is called oncocytic cystadenoma. The authors describe the case of bifocal adenolymphoma in bilateral laryngocoele and indicate classification difficulties of oncocytic lesions in the larynx.

Accelerated phase of chronic myeloid leukemia presenting with hypercalcemia and a mediastinal mass.

A patient with chronic myeloid leukemia developed hypercalcemia as a presenting sign of the accelerated phase of the disease. Ultrasound of the neck showed a large hypodense mass connected to the thyroid gland, which was thought to be a parathyroid tumor and the cause of the hypercalcemia. Histology of the surgically removed mass revealed a chloroma. The patient's course was complicated by respiratory failure and metastatic calcinosis of the lung, an unusual finding in hypercalcemia of short duration.

[Head and neck lymphomas in the material of the ENT department of Municipal Hospital in Gdynia in 1977-1996].

The authors present data relating to patients with malignant lymphoma of the head and neck diagnosed at the ENT Department, Municipal Hospital in Gdynia, during the years 1977-1996. The material is analysed in relation to the sex and age of the patient and the localisation of tumour. Some cases are presented in detail depending on localisation or difficulties in diagnosis (lymphoma of the larynx and ethmoidal sinus).

The effect of desmopressin on massive gastrointestinal bleeding in hereditary telangiectasia unresponsive to treatment with cryoprecipitate.

A patient with hereditary hemorrhagic telangiectasia and reduced levels of von Willebrand factor had severe recurrent gastrointestinal bleeding. Treatment with either desmopressin acetate or cryoprecipitate normalized both bleeding time and kaolin-activated partial thromboplastin time. Hematochezia (five episodes) continued, however, for a week despite daily treatment with cryoprecipitate, whereas treatment with desmopressin at the onset of three later episodes of hematochezia resulted in the prompt cessation of bleeding. After an episode of life-threatening gastrointestinal bleeding, desmopressin administration allowed a total colectomy without excessive bleeding. During the 3 years after colectomy, no further gastrointestinal bleeding occurred. In the past year, however, three episodes of severe epistaxis responded promptly to intravenous infusion of desmopressin. We conclude that desmopressin may be useful in the management of bleeding in patients with hereditary hemorrhagic telangiectasia with or without von Willebrand factor deficiency.
