The influence of medroxyprogesterone acetate (MPA) on hormone parameters in prostatic carcinoma patients.

In ten patients suffering from previously untreated, advanced, non-metastatic prostatic carcinoma, the effect of MPA on the hormonal parameters of testosterone and luteinizing hormone (LH) was examined. The dosage of MPA amounted to 500 mg i.m. daily for a week. Thereafter 1000 mg i.m. every four weeks were applied. Full castration could be achieved in all patients, and this effect could be kept up during the time of observation. The tolerance was good, there were no side effects.

Prognostic significance of immunohistochemical parameters in breast carcinomas.

136 breast carcinomas were investigated immunohistochemically for alpha-lactalbumin (ALA), lactoferrin (Lfr), human milk fat globule membrane antigen (HMFG-2), transferrin receptor (TrfR), Ki-67 and epidermal growth factor receptor (EGFR). The relationships of pathologic features and steroid receptor status previously shown to be of prognostic significance and the immunohistochemical parameters were examined. It was found that estrogen receptor (ER) was inverse related to TrfR, Ki-67 and EGFR whereas progesterone receptor (PgR) was inverse correlated to Ki-67 solely. Tumor grading revealed significant correlations between TrfR, Ki-67 and HMFG-2 (inverse correlation). Tumor diameter showed correlation between Ki-67 solely. Moreover there were significant relationships between lymphoid infiltration and TrfR, Ki-67 and HMFG-2 (inverse correlation). The comparison of the immunohistochemical parameters showed correlations between Ki-67 and TrfR, Ki-67 and HMFG-2 (inverse correlation) as well as HMFG-2 and ALA. Therefore it is suggested that functional and tumor kinetic properties determined by HMFG-2, Ki-67 and TrfR may be an additional indicator with prognostic significance. On the other hand immunoreactivities of ALA in conjunction with HMFG-2 possibly indicates a subpopulation of breast carcinomas that may have to be investigated further. Moreover these results showed that lymphoid infiltration was correlated with Ki-67 reactivity as well as TrfR expression.

[Correlation of maternal androgens in early and late pregnancy. A correlation with fetal size and sex].

In the course of prospective investigations blood was withdrawn for steroid analysis in 50 gravidae between 12th and 16th and between 26th and 30th week of gestation. The levels of androstenedione, testosterone, dehydroepiandrosterone sulfate, 17-hydroxyprogesterone and sex hormone binding globuline were determined by radioimmuno-assay evaluating a possible correlation between the androgen values and growth retardation or the sex of the fetus. Neither during the early nor during the late period of pregnancy significant differences concerning the androgen levels could be detected among both sexes. Worth mentioning is the significant decrease of the DHEA-S levels during the late period of gestation, which occurred in both groups, whereas the androstenedione, testosterone and especially the 17-OH-P levels showed a significant increase. The SHBG values almost did not change at all.

[The medroxyprogesterone acetate serum level following various medroxyprogesterone acetate dose schedules in gynecologic oncology].

Medroxyprogesterone acetate (MPA) is used as an adjuvant hormonal medication in patients with different kinds of carcinomas. Since adequate serum levels are thought to be essential we determined the individual, postoperative MPA levels in 36 patients with endometrial carcinoma over a period of 12 weeks. The patients received either an oral dose of 3 X 100 mg MPA or a weekly changing scheme with 2 X 10 mg Tamoxifen and 3 X 100 mg MPA. An additional small group of 4 patients with ovarian carcinoma was enrolled receiving an oral dose of 1000 mg MPA daily. The peripheral serum levels of MPA exhibit enormous inter- and intraindividuell variations and only the high dosage schemes yield levels above 90 ng/ml which are claimed necessary by some authors. The cortisol concentration measured at the same time were within the normal range and did not correlate with the MPA values.

Pharmacokinetics of cyproterone acetate and its main metabolite 15 beta-hydroxy-cyproterone acetate in young healthy women.

Cyproterone acetate (CPA) and 15 beta-hydroxy-cyproterone acetate (15 beta-OH-CPA) have been quantitated in human plasma by a selective, automated HPLC assay in 8 young female subjects. Subjects received a single oral dose of 100 mg CPA and four weeks later, a single intramuscular dose of 300 mg CPA. Plasma levels in seven subjects could be evaluated pharmacokinetically. After intramuscular administration maximum CPA serum levels of 191 +/- 49 ng/ml were reached between 2 and 3 days. Postmaximal levels declined with a half-life of 4.3 +/- 0.7 days. After oral administration maximal serum levels of 255 +/- 110 ng/ml were measured between 2 and 3 h. CPA levels decayed biphasically with a terminal half-life of 3.6 +/- 1.3 days. Bioavailability after oral administration was nearly complete (88 +/- 20%). Metabolite levels were below CPA levels until 4 days (i.m.) and 6 h (p.o.) and exceeded CPA levels later on. 15 beta-OH-CPA levels declined with half-lives similar to those of corresponding CPA levels, indicating that terminal half-life of the metabolite in serum reflects its rate of formation rather than its rate of elimination. Areas under the metabolite serum level curves after intramuscular and oral administration were 10 to 15% higher than the corresponding areas for unchanged CPA. Since CPA and 15 beta-OH-CPA contribute to the antiandrogenetic efficacy there is good reason to assume that the intramuscular administration of CPA might be equally effective in the therapeutical use as the oral administration.

[High-dosage medroxyprogesterone acetate treatment in advanced prostatic cancer].

32 patients suffering from previously untreated, advanced metastatic or non-metastatic prostatic carcinoma, were studied in an assessment of the effect of high-dose medroxyprogesterone acetate (MPA) on the hormonal parameters testosterone and the luteinizing hormone (LH). The dose used was 500 mg intramuscularly daily for a week, followed by 1000 mg monthly until progression or for a minimal period of 12 months. Full castration was achieved and this effect was maintained during the observation period. Stable disease was seen in 20 patients (63%) whilst progression of disease was found in 12 patients (37%). 4 patients were non-responders. The incidence of side effects was 31%.

[Steroid hormone receptors in struma].

Oestradiol and progesterone receptors occur in different hormonally dependent tissues. The presence and number of steroid receptors in malignant breast tumours is well known. Endocrine treatment is effective. An attempt was made to quantify the oestradiol and progesterone receptors in human goitre. The behaviour of steroid receptors was studied in the cytosol of tissue samples from 33 surgically-treated patients. Neither oestradiol nor progesterone receptors were found in human goitre tissue.

Prognostic significance of steroid hormone receptors and histopathological characterization of human breast cancer.

Since breast cancer is a heterogeneous disease, evaluation of prognosis is an obvious problem. The prognostic significance of several pathological criteria including tumor grading according to WHO, steroid hormone receptor status, histological tumor type, tumor size, and axillary lymph node involvement have been investigated. The single parameters with the strongest prognostic significance were axillary lymph node status, tumor grading, and estrogen receptor status. A significant direct correlation exists between tumor grading and steroid hormone receptors, but nevertheless on stratified analysis both factors independently affected the risk of death. Tumor size had weak prognostic importance and histological tumor types failed to show any statistically significant differences with regard to survival.

["Immunosuppressive acidic protein" (IAP)--an improvement for tumor marker diagnosis in epithelial ovarian cancer?].

Aim of this prospective study was to examine the value of the "Immunosuppressive Acidic Protein" (IAP) as a tumour marker for epithelial ovarian carcinomas and to compare the results with these of the established tumour marker CA-125. In 75 patients with malignant ovarian tumours and in 36 patients with benign ovarian tumours and in 68 healthy women the serum IAP and CA-125 values were determined. In dependence of the threshold for the IAP (500 or 800 microgram/ml) a pronounced lower specificity (70.2% or 97.1%) or sensitivity (88% or 40%) could be achieved in comparison with the CA-125 (95.2% specificity and 81.3% sensitivity). In only 4% of all ovarian carcinomas the CA-125 was false negative and the IAP right positive. Whereas the serum CA-125 level correlated in 86.9% of the patients with the clinical course of disease, the serum IAP level correlated only in 43.3% of the patients with their clinical course of disease. We therefore concluded, that the IAP is less suitable as a tumour marker in ovarian carcinomas than the CA-125 and even the combination of both markers is only beneficial for a very small number of patients.

[Significance of 17-hydroxyprogesterone for the diagnosis of ovulation].

In 30 sterility patients with normal course of the cycle, 17 OHP already rises significantly one day before the LH-peak. It influences not only progesterone and E2 but also hypothalamic opiate activity and LHRH and LH secretion. 17 OHP is on the one hand discussed as a further trigger for the LH peak and is on the other hand regarded as an initiator for incipient luteinization.

[Endocrine profile of patients with post-pill amenorrhea].

"Post-pill amenorrhea" is still a much-debated subject in gynecological endocrinology. The authors therefore studied the reproductive history and endocrinological parameters of 145 patients with the presumptive diagnosis of post-pill amenorrhea, seen in their department during the last two years; 166 cases of secondary amenorrhea served as a control group. The incidence of late menarche (20%), previous oligomenorrhea, and elevated androgen serum levels was high in both groups. There were no significant differences between the two collectives. These findings suggest that although oral contraceptives may have an unfavorable influence on the menstrual cycle, there is no causal relationship between such agents and the development of secondary amenorrhea. It seems that in many cases of presumptive post-pill amenorrhea the common condition of juvenile hyperandrogenemic ovarian insufficiency might be the underlying problem.

Estrogen receptor immunocytochemistry for preoperative determination of estrogen receptor status on fine-needle aspirates of breast cancer.

Fine-needle aspirates (FNAs) of 84 primary breast carcinomas were analyzed immunocytochemically for estrogen receptor (ER) using (ER-ICA) monoclonal antireceptor antibodies. ER-ICA in FNAs was concordant to ER-ICA in histologic biopsies in 87% (P less than 0.0001). In most of the carcinomas, biochemically determined ER status also correlated to ER-ICA. There was no false positive ER-ICA in FNAs compared with ER-ICA in histologic biopsies. In 11 FNAs, ER-ICA was negative, whereas it showed positivity in histologic specimens. The most frequent contributing factors to false negative ER-ICAs of FNAs were ER-ICA-low results in histologic biopsies, a prominent stroma component in these tumors, and low cellularity of FNAs. The biochemical ER values in these cases never exceeded 90 fmol/mg protein. In a minority of cases, false negative results were inexplicable.

Parenteral and oral cyproterone acetate treatment in severe hirsutism.

In the present study a parenteral treatment regimen with cyproterone acetate was compared with the high dose peroral administration in patients with severe hirsutism. Two groups consisting of 10 patients each performed treatment with either 100 mg cyproterone acetate perorally for the first 10 days of the menstrual cycle or received a monthly implant of 300 mg cyproterone acetate intramuscularly applied on the first day of each cycle. In addition, contraception was performed with Diane in both patient groups. 9 treatment cycles were followed by a posttreatment period of 3 months. Hair parameters and serum androgens were monitored regularly. No significant differences of testosterone, androstenedione, dehydroepiandrosterone-sulfate and prolactin serum levels became evident between both cyproterone acetate regimens. Measurements of facial hair diameters revealed a better reduction for the parenteral application. Also the improvement of dermatological parameters was more prominent in the parenteral treatment group. The good effects of a medium dose parenteral application against higher dose peroral treatment thus was documented. The lack of significant differences of androgenic suppression in both regimens turns up the question if different metabolism at the cellular level may be responsible for that phenomenon.

Efficacy of low-dose oral contraceptives containing levonorgestrel, gestoden and cyproterone acetate.

The efficacy of low-dose oral contraceptives containing 30 micrograms or 35 micrograms ethinyl estradiol in combination with Levonorgestrel, gestoden (delta 15-Levonorgestrel) and cyproterone acetate was studied. Borderline doses for inhibition of ovulation were found to be approximately 50 micrograms for Levonorgestrel, 40 micrograms for gestoden and 1 mg for cyproterone acetate. These results were derived from the assay of luteinizing hormone, follicle-stimulating hormone, 17 beta-estradiol and progesterone serum levels during the daily treatment of a total of 47 female volunteers from day 5 through day 25 of their cycles. A combination of 30 micrograms ethinyl estradiol plus 75 micrograms Levonorgestrel or 75 micrograms gestoden resulted in complete inhibition of ovulation in each of the 20 subjects studied. Monitoring of hormone serum levels in 6 female volunteers with normal cycles during the administration of 35 micrograms ethinyl estradiol plus 2 mg cyproterone acetate showed inhibition of ovulation in all subjects studied during the first and third therapy cycle. One subject exhibited follicular maturation during the third treatment cycle as judged from the patterns of 17 beta-estradiol serum levels. Present data suggest that low-dose oral contraceptives containing Levonorgestrel, gestoden and cyproterone acetate are highly efficient in providing great contraceptive safety and do not support the previous notion that the kind of progestagen used is relevant for the degree of central inhibition.

Multi-centre study of a new CEA enzyme immunoassay using monoclonal antibodies.

The use of a new monoclonal enzyme immunoassay (EIA) for the carcinoembryonic antigen (CEA) (Enzymun-Test CEA) was evaluated in a multi-centre study. Fifteen different laboratories [participated in the study. Data from the investigation were analysed in terms of precision, sensitivity, specificity and correlation with other test methods. The intra-assay coefficient of variation was between 1.3% at 23.0 microg/l CEA and 13.9% at 1.3 microg/l CEA. Inter-assay reproducibility ranged from 3.6% to 19.2%. The apparent sensitivity of the new EIA for CEA was approx. 0.5 microg/l CEA. The findings indicate that lipaemic and haemolytic sera and samples taken from icteric, rheumatic and dialysis patients did not have any influence on the results. There was no evidence that drugs commonly used in the treatment of carcinoma patients have any influence on the assay results. A good correlation between the new EIA for CEA and six other CEA enzyme immunoassay or radioimmunoassay methods was registered. These results seem to be of significance in particular for the monitoring of therapy for carcinoma patients. The new EIA for CEA exhibits a high degree of sensitivity, specificity and reproducibility.
