[Registration of hospital infections: effects of 5 years observation of peritonitis].

In connection with a great availability of antibiotics, many negative side effects have been observed. A registration program of hospital infections' occurrence and drug-sensitiveness has been conducted for five years in all wards of Mining Hospital in Sosnowiec. This paper presents our first data and conclusions after examinations of the biological materials coming from patients suffering from different kinds of the peritonitis.

[Histopathological verification of the cervical pap smears based on material of the obstetrical-gynecological department of the provincial hospital in Kielce].

The results of the cervical Pap. smear taken before cold knife conization and the histopathological results of the cones removed in 336 women, were compared. In cytological examination we found 10.8% (13/120) false negative results, which did not confirm of the existence the precancerous lesion within cervical epithelium. The mild dysplasia according to Pap. smears was established among 15.9% (15/94) of women with CIN III and CIN II diagnosed pathologically. In part of the women the histopathological results of the postoperative specimens and bioptic cervical material were compared. Clinical value and correlation between histopathological and cytological results in the estimation of CIN (cervical intraepithelial neoplasia) were discussed with particular regard to CIN I according to Bethesda system.

Applications of modelling studies.

There is increasing use of economic evaluation to support decision-makers in health services for allocation of scarce healthcare resources. However, information necessary for economic evaluations cannot always be derived from well-conducted prospective clinical studies, and decision-analytical models can be used to provide some of the missing information. A number of applications of decision-analytical models are discussed, including submissions to authorities for reimbursement purposes, pharmacoeconomic programme development, portfolio investment decisions, marketing and communication strategies, and applications in disease management.

[Acute renal failure during pregnancy, labor and puerperium in women].

The reasons and course of acute renal failure during pregnancy, labour and puerperium were presented in 30 women treated in Provincial Hospital in Kielce. The most frequent reason was haemorrhage--15 (50%) women, sepsis--10 (33,3%) women and preeclampsia--eclampsia--2(6, 6%) women. 15 women died as the consequence of multiple organ failure. Among 15 women who survived renal function has returned completely in 13 (86, 6%) ones. In remaining 2 (13, 4%) women chronic renal failure persisted.

[Level of neopterin in blood serum in selected thyroid diseases].

The aim of our study was to estimate the levels of neopterin (NTP), anti thyroglobulin antibodies (ATG), anti thyroid microsomal antibodies (MAB), immunoglobulins (IgG, IgA and IgM) and creatinine in the serum of patients with thyroid diseases. The clinical material consisted of 71 patients (64 women and 7 men) with non-toxic nodular goitre (30 persons), neoplasms (33 persons: 22 with thyroid adenoma and 11 with thyroid carcinoma) and thyroiditis (8 persons). The control group consisted of 30 healthy women. In studied groups the mean serum levels of NTP and creatinine were in normal range. The highest levels of NTP were found in patients with thyroid carcinoma (7.14 +/- 5.95 nmol/l) and with non-toxic nodular goitre (5.13 +/- 3.57 nmol/l). It was statistically significant in comparison with control group (p < 0.05). Anti thyroglobulin antibodies (ATG) were detected in serum of 36 patients (50.7%) and anti thyroid microsomal antibodies (MAB) were in serum of 28 patients (39.4%). Mean levels of these autoantibodies were significantly higher than in control group (< 0.05). In patients with autoantibodies (ATG and/or MAB) mean serum level of NPT was significantly higher than in patients without these autoantibodies (p < 0.05). We did not find a significant correlation between the level of NPT and the levels of ATG and MAB.

[Thoracic sympathectomy using thoracoscopy].

In the years 1996-1997 9 dorsal sympathectomies (DS) were performed in the authors Department using VATS technique. There were 6 right dorsal sympathectomies and 3 left DS in the group of 7 patients. Indication for dorsal sympathectomies was idiopathic hyperhidrosis in 4 cases and vascular disorders in 3 cases. Operating time for the single procedure was 30'. Chest drain was never used. There were no major complications (Horner's syndrome-one patient, intercostal neuralgia, which disappeared in a few days-2 patients). Patients left hospital the day after operation (only those with fingers ulceration stayed longer). The results of dorsal sympathectomies in patients with hyperhydorsis were excellent, less impressive in those suffered from vasospastic and vascular disorders.

[Late results of laparoscopic surgery for inguinal hernia (TAPP)].

Late results of TAPP have been assessed in a group of 79 patients operated for inguinal hernia- in the years 1994-1996. Altogether 87 TAPP were performed in this group (8 bilateral herniorrhaphies) all of them classified according to ASA scale from I degree-III degree. Following Schumpelick hernia classification there were: M = 41, L = 35, F = 11, I degree = 31, II degree = 27, III degree = 21. 31 hernias were recurrent and 11 were irreducible. The patients have been followed up for at least 1 year (on average 16 months). TAPP was lasting in this group of patients from 40-120 min (on average 70 min) including bilateral repairs. There was 1 intraoperative bleeding which had been the cause of conversion to the open operation. No wound infection was noticed. Postoperative inguinal hematoma which resolved spontaneously was noticed in 4 cases. The patients left hospital the day after TAPP. 2 recurrences were noticed so far (2.3%), 3 patients complained of transient inguinal neuralgia. Late results of TAPP in the rest of the patients have been very good.

[Usefulness of standard clotting and fibrinolysis tests for evaluation of hemostatic balance in women during the III trimester of pregnancy with slight and moderate preeclampsia].

Usefulness of the standard test to control hemostatic balance has been evaluated in 22 pregnant women in III trimester with preeclampsia. Unpregnant and pregnant healthy women there were the control group. Increase of the fibrinogen concentration and the fibrinolysis time prolongation have been confirmed as well as increase of the degradation fibrin/ogen products (FDP/fdp) concentration (significantly the highest in the women with moderate preeclampsia). The remaining parameters of hemostasis in women with preeclampsia there were within laboratory standards.

[Influence of fetal weight on labor induction with intracervical PGE2].

A hundred and forty four pregnant women between 35 and 42 weeks of pregnancy with unfavourable cervices were given intracervical prostaglandin E2 gel for cervical priming and labour induction. Successfully inductions of labour were achieved in 102 (70.8%) women and failed inductions were in 42 (20.2%) women. Cesarean section was performed in 15.6% women in group with successfully inductions of labour and in 19% in group where unsuccessfully inductions were continued by traditional methods. All infants were delivered in good condition. There was no correlation between fetal weight and efficiency of labour induction with intracervical PGE2.

Surgical treatment of vulvar cancer.

Two hundred and seventy two women with vulvar cancer were submitted to standard surgical treatment. In 127 women (46.7%) regional lymph nodes were involved. Out of 181 women (66.5%) who survived 5 years, 132/145 women (91%) were with negative nodes and 49/127 women (38.6%) were with positive nodes. Surgical management of vulvar cancer is discussed, especially inguinal lymphadenectomy.

[Usefulness of determining serum levels of prolactin in women with menstruation disorders].

In 173 women with oligomenorrhea or amenorrhea, serum level of prolactin (PRL) was determined by radioimmunoassay method. In 22 cases (12.7%) the concentration of PRL was above 15 ng/ml. From the above women in 9 cases (5.2%) of all examined patients pituitary microadenoma was diagnosed. In 1 patient with pituitary microadenoma, coexisting partially empty sella was diagnosed. The mean basal PRL concentration in women with menstruation disturbances without hyperprolactinemia (group I) was found to be 7.1 +/- 3.1 ng/ml while the women with hyperprolactinemia without pituitary microadenoma (group II) was 32.9 +/- 5.8 ng/ml. In women with diagnosed pituitary microadenoma (group III) PRL concentration was 131.8 +/- 47.7 ng/ml. The group II and III demonstrated statistically significant increase of serum prolactin level in comparison to group I. Moreover in comparison with group I, statistically significant reduction of serum level of FSH, LH and estradiol 17 beta was confirmed in group with pituitary microadenoma. In group II significant decrease of serum concentration of LH and estradiol 17 beta was stated as well as lack of significant difference in serum level of FSH, in comparison to group I.
