[Spinal epidural abscess: an interdisciplinary emergency].

On the basis of fifteen own observations in the years 1978 to 1984 as well as the literature, clinic diagnostic and therapy of epidural spinal abscess are represented. Myelography and CT are the leading methods after clinic and neurology must already lead to a hospital admission in case of the slightest signs of an incipient transverse lesion of te cord with paraplegia. Laminectomy as well as suction and through-drainage are methods of choice, conservative treatment is an exception.

Abscess

Ultrastructural findings after the use of a CO2 laser in carpal tunnel surgery.

The laser beam enables tissue cutting and care of the cut edges, which prevents an increased scar tissue formation. For this reason we have used the laser beam in carpal tunnel surgery for transection or sealing of the transverse carpal ligament since 1981 because scar tissue formation is for the most part the cause of recurrence. Scanning-electron microscopic studies of the carpal ligament show the difference between the laser and traditional cutting techniques. After using the laser beam, there is no normal tissue structure; the cut surface appears as uneven and rough in contrast to the cut surface of the cold knife.

Carpal Tunnel Syndrome

Scanning electron microscopic findings on the rabbit meninges after local lavage with Polyvinylpyrrolidon-Jod-Komplex.

The microbicide Polyvinylpyrrolidon-Jod-Komplex (P.J.K.) was experimentally evaluated for its use in local lavage of the epidural space. In 20 rabbits a lumbar laminectomy was performed followed either by local lavage with P.J.K. or with NaCl. After one month the meningeal covering of the operated spinal cords revealed no signs of fibrosis or arachnoid adhesions when studied macroscopically or by scanning electron microscopy (SEM).

Animals

The intra-operative application of povidone-iodine in neurosurgery.

Over a period of 6 months 250 patients were operated on for lumbar disc prolapse, 125 patients each receiving prophylactic antibiotic therapy or only local, intra-operative administration of povidone-iodine. The analysis of variance showed a significant difference between the patient group treated with the antiseptic and the prophylactic antibiotic group. More abscesses and signs of infection occurred in the antibiotic treated group. Animal studies are also described.

Adult

[The craniocervical transitional area: a diagnostic problem].

The case histories of six patients with spinal C1/C2 region lesions are presented. All cases were misinterpreted by clinical features and radiological findings over a long period. Radiological evaluation documents the significant improvement in diagnosis by supplementary use of computer assisted myelography.

Aged

[Brain injuries in skiers].

During the last skiing season (November 1981 to March 1982), 33 patients were treated for craniocerebral injuries caused by skiing accidents. Based on the classification made by Tönnis und Loew, 11 patients had grade I, 11 patients grade II and 11 patients grade III (including 3 open craniocerebral injuries). Four patients underwent operation for intracranial hematoma. Out of 33 patients 79% recovered completely. One patient has a permanent neurologic defect, two patients have an apallic syndrome and four died.

Adolescent

[Carpal tunnel syndrome as an unrecognized disease in the aged].

Although the symptoms are very characteristical (in our patients 91,5% pain, 69,2% disturbed sensibility and 29,2% weakness) and the neurological diagnosis is typical (82,9% disturbed sensibility, 30,6% weakness, 57,1% atrophy of the thenar muscle) faults in diagnosis are possible. Particularly in old age it is mistaken for vertebral or vascular diseases. If conservative treatment is ineffective or the compression of the nerve is enlarging fast, surgical treatment will be indicated. In this case you can get freedom of pain in 66,6% in applying the right method of operation (5 different methods in micro-neurosurgery are available). Since May 1981 we use laser-surgery with great success. We checked (in average 13 months after the operation) 111 of 133 patients (33 male, 100 female, age of 16-84 most of them in the 6. decade) and got those results: 58,7% very good, 14,8% good, 15,7% satisfying and 5,8% unchanged.

Aged

Survival after traumatic ponto-medullary tear.

Two children (aged 10 and 12 years) were admitted unconscious to the neurosurgical department after traffic accidents. Both developed a 6th nerve paralysis on the next day. One patient was able to communicate from the 2nd day and died on the 8th day in an anuric state without major neurological deficit. The second patient remained deeply comatose, tetraplegic, and required intermittent artificial respiration: She died of pneumonia on the 26th day. Neuropathological examination revealed a ponto-medullary rent in each case: additionally there was avulsion of small arteries over the pyramids, haemorrhage and small focal infarcts in the distribution of perforating arteries in the medulla and pons, and abundant retraction balls in longitudinal fibre tracts of the brain stem. The cases show for the first time that traumatic ponto-medullary tears are not always rapidly fatal, and demonstrate that primary focal brain stem trauma may occur in the absence of diffuse trauma of the white substance.

Cerebral Arteries

[Supratentorial arachnoid cysts].

In 11 of 13 patients with arachnoid cysts in the region of the cerebrum, diagnosis and indication for surgery could only be established by computer tomography. In one patient, a chronic subdural haematoma combined with a cyst was found, in one case a cyst could also be diagnosed by angiography and pneumoencephalography. In II patients there was a space-occupying effect of the cyst with compression of cerebral tissue and pronounced neurological symptoms, so that a cyst resection or removal was carried out in 10 patients and a shunt operation in one case.

Adolescent

[Cauda equina tumor].

The importance of the myelography in comparison to CT (CT-myelography) is demonstrated in a patient with an intradural neurinoma.
