Small intestinal manometry in healthy elderly subjects.

Small intestinal manometry was performed on ten healthy elderly subjects (mean age, 72 years) and ten healthy young-adult subjects (mean age 25 years). All three phases of the migrating motor complex were observed in both groups during fasting. There were no differences in the motility index between the two groups, the frequency and the mean amplitude of contractions of phases 2 and 3, or the duration and the velocity of propagation of phase 3. After a meal the motility index and the frequency of contractions were significantly lower (P less than 0.05) in the elderly group. The results suggest that intestinal motility after a meal is less in the elderly than in young adults.

Comparative anatomy of the myenteric plexus of the distal colon in eight mammals.

The behavior of the most distal part of the colon in a variety of species suggests that the innervation of this part may differ from that of more proximal parts. Silver impregnation was used to demonstrate the arrangement of the myenteric plexus of the distal colon in eight species (rat, guinea pig, rabbit, Australian possum, American opossum, cat, dog, and monkey). A distal zone, approximately 5%-20% of the total length of the colon above the anal verge in the nonrodents, was characterized by a plexus of very irregularly disposed intersecting nerve bundles of highly variable size with few and small ganglia; this zone was absent in the three rodent species. A next most distal zone, approximately 10%-65% of the total colonic length, contained a stellate plexus of large, regularly disposed ganglia interconnected by small nerve fiber bundles upon which were superimposed large dark-staining nerve bundles; these bundles began to be seen at the level of the irregular rectal plexus and ran cephalad, bypassing some ganglia but giving off branches to others. These, called shunt fascicles, contained many myelinated nerve fibers. Above this zone, the plexus was a stellate plexus throughout the remainder of the colon.

Isolation and characterization of a major tandem repeat family from the human X chromosome.

We report the identification and characterization of a family of repeated restriction fragments whose molecular organization is apparently specific to the human X chromosome. This fragment, identified as an ethidium bromide-staining 2.0 kilobase (kb) band in BamHI-digested DNA from a Chinese hamster-human somatic cell hybrid containing a human X chromosome, has been cloned into pBR325 and characterized. The 2.0 kb repeated family has been assigned to the Xp11 leads to Xq12 region on the X by Southern blot analysis of somatic cell hybrids and is predominantly arranged in tandem clusters of up to seven 2.0 kb monomers. Homologous DNA sequences, not organized as 2.0 kb BamHI fragments, are found elsewhere on the X chromosome and on at least some autosomes, but are not found on the Y chromosome. From a dosing experiment using various amounts of the cloned repeat, we estimate that there are 5,000-7,500 copies of the 2.0 kb BamHI repeat per haploid genome. Since the vast majority, if not all, of these are confined to the X chromosome, this repeated DNA family must account for 5-10% of all X chromosome DNA and must constitute the major sequence component of the pericentromeric region of the X.

Busulfan versus hydroxyurea in long-term therapy of chronic myelogenous leukemia.

Survival of patients with chronic myeloid leukemia seen in the University of Colorado Leukemia Clinic were reviewed with respect to therapy. A total of 55 patients seen consecutively through mid 1980 were included in this study. Of these patients 30 were treated with busulfan, 14 were treated with hydroxyurea and 11 received under modalities. Busulfan treated patients who are now deceased, have had a median survival of 35 months (range, 13-108) and actuarial analysis shows the total busulfan treated population to have an expected median survival of 48 months. Hydroxyurea treated patients who are still alive have been followed for a median period of 69 months (range, 25-136 months) and a projected median survival for periods of 56 and 90 months, respectively. These data suggest that hydroxyurea may be an important treatment modality in the treatment of chronic myelogeous leukemia.

Treatment of advanced malignant melanoma with high-dose chemotherapy and autologous bone marrow transplantation. Preliminary results--Phase I study.

Thirteen patients with advanced (Stage III) malignant melanoma have been treated with high-dose chemotherapy (nitrogen mustard or a combination of BCNU and melphalan) combined with autologous, nonfrozen, bone marrow transplantation. Three patients (24%) achieved a complete remission and are currently alive and free of disease without further therapy at 26, 60, and 73 weeks. Five patients (38%) achieved partial remissions and five patients (38%) had no response. There was no difference in the response rate to nitrogen mustard and the BCNU-melphalan combination. Severe side effects to nitrogen mustard, however, precluded its further use in this study. The major cause of death in patients was intracerebral metastases, raising the question of prophylactic brain irradiation in future studies. Studies of the recovery rate of peripheral blood neutrophil, platelet, and peripheral blood and bone marrow CFU-C suggest that autologous bone marrow infusion may be of benefit in shortening hematopoietic recovery following intensive chemotherapy.

Causes of death in patients with non-Hodgkin's lymphoma.

The causes of death and postmortem findings in patients treated for non-Hodgkin's lymphoma at a single institution over a 13-year period were reviewed. Postmortem examination (70% of the entire sample) revealed evidence of lymphoma in 67 of 80 patients. The most frequent extranodal sites of involvement were the respiratory tract, bone marrow, liver, kidney, and gastrointestinal tract in that order. The most common cause of death was infection (33% of cases). Predisposing factors for infection included the underlying disease, (i.e., lymphomatous infiltration of organ systems) and granulocytopenia secondary to combination chemotherapy. Other causes of death included hemorrhage and respiratory failure secondary to lymphomatous infiltration of the lung. Despite advances in therapy and supportive care of patients with non-Hodgkin's lymphoma, many patients still die of this disease or of sequelae related to its treatment.

Physicochemical-activity relationships in asymmetrical analogues of methoxychlor.

Compounds of the general formula 2-aryl-2-(p-methoxyphenyl)-1,1,1-trichloroethane have been prepared and tested for toxicity toward houseflies, pretreated for 1 h with 2mug of piperonyl butoxide. The majority of the compounds synthesized were chosen with the aid of computer programs designed to ensurewell-spread sets of minimally correlated physicochemical parameter values. A nonlinear two-dimensional representation was used to map the active region of physiochemical parameter space and a regression equation was obtained relating the observed toxicity to a combination of these physicochemical parameters. The equation indicates that toxicity increases with the hydrophobicity of the molecules but is decreased markedly by the introduction of bulky substituents into the ortho positions of the benzene ring and less markedly by bulky substituents in the meta and para positions. Substituents which donate electrons to the benzene ring by the "resonance" effect favor high toxicity. The equation performs well in forecasting the toxicity of further members of the series.

The experimental production of hyaline membranes in premature rhesus monkeys.

When premature rhesus monkeys are ventilated with oxygen or air, hyaline membranes are formed within 5 minutes in alveolar ducts and respiratory bronchioles. Studies with fluorescein-labeled antiserum to porcine pepsinogen provide evidence that the monkey fetuses have aspirated gastric fluid. The findings suggest that some component of gastric fluid plays an important role in the formation of the hyaline membranes, either directly by causing necrosis of susceptible bronchiolar epithelium or secondarily by sensitizing the bronchiolar epithelium to another necrotizing agent such as oxygen.
