[The effectiveness of combined treatment for laryngeal cancer in Silesia region during the years 1987-1996].

Between 1987-1996, 484 patients with laryngeal cancer were treated by surgery and adjuvant radiotherapy. The 25-30% therapeutic gain was noted when to compare the first period of the study (1987-1992) with the second (1993-1996). The increment of effectiveness of combined treatment for larynx cancer is probably the result of better quality of surgical procedures and individualization of radiation treatment.

Sequential combination of radio-chemotherapy (5-fluorouracil, cis-platinum and irradiation) in the management of locally advanced head and neck cancers.

Thirty-seven previously untreated patients with advanced, inoperable head and neck were treated with a sequential courses combining hypofractionated irradiation with chemotherapy (5-fluorouracil and cis-platinum). Each course was repeated every 4 weeks. Tumor response was evaluated and for 15 patients (41%) with a partial or complete regression after 3 radio-chemotherapy courses conventional radiotherapy was added. Eleven percent of all patients were in complete remission at the end of a treatment. This tumor response rate and the 50% rate of pain subside after first course for symptomatic patients contributed for a good palliative effect in the present study. However, the median survival of 7.2 months was considered unsatisfactory.

[Prognostic factors in radiotherapy of supraglottic cancer].

In 971 patients with squamous cell carcinoma of supraglottic larynx several clinical and physical prognostic factors were evaluated. There were 55% of patients with advanced primary tumours and 71% of patients with no regional neck metastases. All patients were irradiated radically using external megavoltage beam. The total dose was in range 60-70 Gy for 95% of patients. The 3-year local control rate and 3-year overall in whole group were 41% and 50% respectively. Clinical stage, haemoglobin level at the end of treatment, overall treatment time, sex and total dose were estimated as an independent and important prognostic factors for the outcome in radiotherapy of supraglottic larynx cancer.

[Epidemiological risk factors of larynx cancer among natives and immigrants in the Upper Silesian region].

A case-control study has been used to analyse risk factors such as tobacco smoking, pollution exposure in the microenvironment of the place of work, ambient air pollution in the living place among natives and immigrants for larynx cancer in Upper Silesia Region. There were tested 399 cases (269 natives and 130 immigrants) and 942 controls (557 and 385 respectively). Tobacco smoking and exposure to air pollution in the place of work is connected with increased incidence of larynx cancer risk among natives and immigrants. It was shown that there is no connection between larynx cancer and the degree of air pollution in the living place among natives.

Larynx cancer risk factors.

The larynx cancer relative risk has been evaluated (328 cases) in the stationary-native Upper Silesians (63.7%) and migrating (36.3%) male population. The particular control groups (656 men) included 418 and 238 men respectively not suffering from malignant neoplasms. The essentially higher larynx cancer incidence risk has been shown for men manual labor and exposed to the influence of various dusts, gases and vapors and other pollutants in their place of work. The increased substantial incidence risk is also true for men who smoke tobacco and are professionally exposed to the pollution. The results in regard to the consumption of raw and boiled vegetables suggest that these may be inhibitors of the neoplasmatic process in the area of larynx (although not only). The frequency of the consumption of alcohol--vodka or beer--increases the larynx cancer incidence risk both among autochthons and immigrants. Nevertheless, the data concerning the amount of alcohol consumed are less valuable here; they do not allow to establish fully reliable quantitative levels of the beer or vodka consumed. In comparison with the literature of the subject, divergent data have been obtained as far as the joint influence of drinking alcohol with simultaneous smoking of tobacco and professional exposure are concerned.
