Influence on the depth of inhalation on the effect of the beta 2-receptor-stimulating drug fenoterol hydrobromide (berotec).

Study of the influence of different modes of administration of 200 microgram of fenoterol showed that in a selected group of patients, two thirds of the maximal obtainable decrease in airway resistance could be achieved within 3 min of local application of 200 microgram fenoterol onto the nasal mucosa, without inhalation into the lower airways. Aspects of the potential pathways by which inhaled substances reach their receptor or target sites are discussed.

Adjuvant therapy with levamisole in resectable lung cancer.

In view of the discouraging results that have been obtained so far with the use of cytotoxic chemotherapy as an adjunct to surgery, a double-blind placebo-controlled evaluation of the adjuvant use of levamisole was conducted in 211 resectable lung cancer patients, following these patients for 2 years after their operation. Levamisole (or the placebo) was given for 3 days every 2 weeks and the dose level ranged 1.1--3.8 mg/kg per day (a fixed dose of 3 x 50 mg was given to all patients). It appeared that recurrences and carcinomatous deaths had occurred significantly less often in patients who had received a high dose (i.e., 2.1--3,8 mg/kg: patients weighing 70 kg or less) but not in the patients who received a lower dose. Patients who had more advanced cancers at the time of surgery seemed to have profited more from the treatment, but the results did not seem to depend upon the histologic type of the tumor or on the immune status of the patients as estimated from the skin test reactivity at the start. There was also suggestive evidence that levamisole may be more effective in preventing hematogenous dissemination than in inhibiting recurrences in the lung or the mediastinal tissues. Levamisole, if dosed adequately, appears to be a very suitable adjuvant treatment in resectable lung cancer patients as judged from its efficacy and its lack of troublesome side-effects.

Late presentation of primary pulmonary lymphangiectasis.

Pulmonary lymphangiectasis can occur either as a primary change or secondary to chronic congestion. Primary lymphangiectasis is generally considered to occur exclusively in young children. This report describes three male patients, aged 13, 16, and 19 years, in whom the changes are limited to one lobe or to two lobes and the mediastinum. So far as could be ascertained these patients are the oldest reported. Lymphangiectasis should be considered in the clinical differential diagnosis of "pulmonary cysts" in children and young adults.

Cell-bound IgE on human mast cells and basophilic granulocytes in atopic and nonatopic subjects.

In previous studies basophil-bound IgE was measured by means of quantitative immunofluorescence miscroscopy. But mast cells are of particular importance in local atopic reactions, we thought it necessary to investigate the IgE-load on both cell types. The amount of mast cell-bound IgE can be estimated in a semiquantitative way by end point titration with a fluoresceinated sheep antihuman IgE (FlaIgE). The amount of mast cell-bound IgE was estimated this way in tonsils, adenoids, bronchial and tracheal tissues, and was compared with the amount of IgE on the basophils. A correlation was found between the IgE-load on mast cells and on basophils. Atopic patients had a higher mast cell fluorescence titer (MCFT) than nonatopic controls. It was demonstrated that, as on basophils, the IgE level of the serum contributes to this difference.

Comparative studies of six nebulizers with Congo Red millipore technique.

If the particle size is the important factor to be considered in case of droplet size spectrum then from our results it can be said that Gauchard with 6 1/min. flow and Bird Asmastik with 95 1/min. are producing the particle size classes which can reach most easily the lower respiratory passage. The nebulizers which are producing droplets of a mass median diameter bigger than 10 mu, send only an unpredictable portion of their liquid output into the lower respiratory system. Smaller particles are representing very little weight and carry only small quantities of liquid material unless present in vast majority in the aerosol. To recommend an ideal nebulizer is yet a long way to go but considering the very big quantitative influence of a few big droplets, a small mass median diameter (1-7 mu) is an important factor on the credit of any nebulizer.

A model of the pathways of inhaled medicaments to their receptor sites.

From controlled inhalation experiments with bronchotropic substances, it becomes clear that the 'dust model of the lungs' is inadequate in explaining the 'facts and findings' of drug inhalation. Two pathway flow charts are worked out, permitting a more specific description of the transport processes involved, in order to meet the need for a more appropriate model.

Open lung biopsy as a diagnostic adjuvant in the diagnosis of pulmonary diseases.

In a survey of the possible means of establishing a diagnosis in diffuse pulmonary diseases, the usefulness of the open lung biopsy technique is discussed in more detail. In the opinion of the writers the open biopsy offers advantages over the other biopsy methods since: 1) a greater number of and more extensive biopsies can be taken; 2) the hazard of complications is slighter; 3) differences in biopsy specimens, for example, from the upper lobe and the lower lobe can be studied; 4) multiple biopsy specimens permit the carrying out of many examinations such as virological examination, bacteriological examination, histochemical examination, electron microscopic examination and immunofluorescence examination. The method under discussion carries no risk. There was no mortality in this series. The hazard of complications is slight and certainly slighter than in other methods; our experience has shown that it is slighter than in other methods of biopsying the lung.
