Rosacea: current thoughts on origin.

Rosacea is a clinical pattern beginning and evolving in the genetically susceptible individual in response to a host of exposures. It produces a variety of clinical presentations, which vary over time and with age. Recently, many specific mediators of rosacea development have been described. A primary genetic cause for rosacea is suggested as single genes often control such mediators: enzymes, neuroendocrine transmitters, and cytokines are found in pathways to rosacea signs and symptoms. Currently, neither a specific cause nor a laboratory indicator of rosacea has been suggested. However, broadening interest in rosacea portends future increase in knowledge.

Effect of treatment of Helicobacter pylori infection on rosacea.

OBJECTIVE: To evaluate the clearing and intensity of symptoms of rosacea 60 days after the treatment of Helicobacter pylori infection. DESIGN: Randomized, double-blind, placebo-controlled clinical trial. SETTING: The dermatology section of a large multispecialty clinic in the North Central United States. PARTICIPANTS: Men and women older than 25 years with active signs of rosacea who tested positive for H pylori with both the rapid whole blood test and the urea breath test. INTERVENTION: Treatment of H pylori infection with 14-day therapy using clarithromycin. 500 mg orally 3 times a day, and omeprazole, 40 mg orally once a day. MAIN OUTCOME VARIABLES: The extent and intensity of rosacea as measured by the number of papules and pustules and the extent and intensity of erythema and telangiectasia. RESULTS: Three hundred twenty patients presented with rosacea. For 50 patients, the results of a urea breath test were positive for H pylori, and 44 patients were enrolled in the study. Rosacea abated in almost all patients, but none were cured. Notably, lessening of rosacea for patients treated for H pylori was not significantly better than for the control cohort. CONCLUSIONS: Rosacea abated in most participants in this study, whether they were in the treatment or the control cohort. There was no statistical difference when the results of active treatment were compared with those of placebo. Treating H pylori infection has no short-term beneficial effect on the symptoms of rosacea to support the suggested causal association between H pylori infection and rosacea.

Atopic eczema unresponsive to evening primrose oil (linoleic and gamma-linolenic acids).

This study was designed to look at the effect of evening primrose oil (linoleic and gamma-linolenic acids) as an oral supplement for patients with atopic eczema. We used a double-blind, blocked crossover design with random assignment of patients to treatment groups. We used Wilcoxon's signed-ranks method of comparing changes during the trial. We observed no significant effect on erythema, scale, excoriation, lichenification, or overall severity in 123 patients with atopic eczema of average severity while they took oral doses of evening primrose oil (2 or 4 gm in children, 6 or 8 gm in adults).

Treatment of tinea versicolor with sulfur-salicylic shampoo.

One week of nightly application of sulfur-salicylic acid shampoo as a lotion was evaluated for the treatment of tinea versicolor. One half of the randomly allocated patients used the active preparation and the other half used a bland oil-in-water lotion as a placebo. Three months after completing treatment, nineteen of twenty-two using sulfur-salicylic acid shampoo were still negative by KOH examination, whereas only one of sixteen controls was negative. Skin irritation occurred in a few patients but did not reduce the effectiveness of the treatment.

Failure of oral gastrointestinal dialysis as therapy for psoriasis.

Ten patients with chronic psoriasis were treated twice weekly with gastrointestinal dialysis in an attempt to clear psoriatic skin lesions with this method alone. In a fasting state in the morning, each patient drank from 3 to 6 L of an electrolyte solution containing 220 mmole/L of mannitol during three hours. The procedure was well tolerated, safe, and effective in causing copious diarrhea. However, after two months of therapy, these patients showed no substantial improvement in their psoriatic skin lesions.

Spontaneous regression of congenital cutaneous histiocytosis X: report of a case with discussion of nosology and pathogenesis.

Spontaneous regression of histiocytosis X is a rare biologic event as judged by the extensive recorded experience in the literature. We present a case of congenital cutaneous histiocytosis X that resolved without specific therapy in the first week of life. Histologic and ultrastructural studies confirmed the interpretation. This child is an example of so-called pure cutaneous histiocytosis, which, like the other unisystem forms of the disorders, has an excellent prognosis.
