Pattern of cancer in Saudi Arabs referred to King Faisal Specialist Hospital.

A total of 7251 histologically confirmed new cases of cancer (4117 males and 3134 females) were seen in the 6-year period 1979 to 1984 at the King Faisal Specialist Hospital and Research Centre in Riyadh, Saudi Arabia. The crude relative frequencies of cancer at various primary sites have been determined with reference to sex, age, geographic origin, and year of diagnosis. The most common cancer sites among males were non-Hodgkin's lymphomas, esophagus, lung, liver, stomach, and nasopharynx. Breast cancer was the most common tumor among the females, followed by non-Hodgkin's lymphomas and cancers of the thyroid, esophagus, cervix, and ovary. The most marked deviations were found in the Southern Region for cancers of the oral cavity (2.4 times higher), bladder (1.8 times higher), and lung (4.3 times lower). Known etiologic factors, such as local chewing, smoking habits, and schistosomiasis are likely to be responsible for these differences. Upward trends in cancers of lung, breast, colon and rectum, and the downward trend in esophageal cancer may reflect the rapid pace of modernization.

Histiocytosis-X of the spinal cord: a case report.

A rare case of histiocytosis-X of the spinal cord with evidence of intramedullary infiltration and epidural spinal cord compression is reported. The clinical, radiological, and histopathological features are discussed, and the pertinent literature is reviewed.

Cauda Equina

Study of the HLA gene and antigen frequency from a Saudi Arabian hospital.

The HLA-A, -B, -C and DR loci antigen frequencies were determined, respectively, on 1145, 558 and 352 healthy nonrelated Saudi family members. B21, CW4, CW7, and Dr7 showed the highest gene frequencies, of 14.6%, 28.3%, 7.4% and 19.5%, when compared to other populations. Haplotypes A2-B5, A32-B51, A26-B8, A2-B21, A28-B35, Aw19-B21 and B21-CW4 showed the highest frequencies when compared to other populations. Gene frequencies of 14.6% and 20.2% for B21 and Aw19 antigens, respectively, are highest among Middle East populations. Gene frequencies for A1 (10.5%), A2 (24.9%), A3 (8.9%), A9 (16.7%) and A28 (7.9%) are similar to the 10.1%, 24.9%, 8.3%, 16.8% and 7.7%, respectively, reported for the Turkish population. Also, gene frequencies for B5 (18.5%), B21 (14.6%) and B35 (10.2%) are very close to 17.1%, 14.0% and 10.2%, respectively, reported in the Yemenite population. The above results suggest some influence of other populations on the 'pure' Arab population.

Kaposi's sarcoma in renal transplant recipients. Ultrastructural and immunoperoxidase study of four cases.

Tissues from four cases of Kaposi's sarcoma developing in renal transplant recipients were studied by light and electron microscopic examination and by immunoperoxidase staining for Factor-VIII-related antigen. Ultrastructurally, the tumors in all four cases contained a variable mixture of cells, including endothelial cells, pericytes, fibroblasts, and myofibroblasts. These findings support the origin of Kaposi's sarcoma from primitive vasoformative mesenchyme. Immunoperoxidase staining for Factor-VIII-related antigen was limited to endothelial cells. In one case intracytoplasmic virus-like tubular complexes were seen. The significance of this finding is briefly discussed.

Pathology of bilharzial bladder cancer.

Retrospective review of bladder carcinoma at this institution has revealed a high incidence of squamous cell carcinoma associated with bilharzia infection as has been found in other Mideast and African countries. Associated inflammatory and epithelial metaplastic changes were commonly noted and apparently represent early changes in the development of carcinoma, particularly in view of the progression from squamous metaplasia to in situ and infiltrating carcinoma observed in both bladder and ureter. The relationship between bilharzia infection and the development of bladder carcinoma has been postulated to be related to several factors; however, as yet the specific etiologic relationship and pathogenesis have not been defined.

Adenocarcinoma

Secretory carcinoma of the breast in adults. Light and electron microscopic study of three cases with review of the literature.

Three adult cases of secretory carcinoma of the breast were studied by both light and electron microscopy. Histologically the tumors revealed three patterns, namely, solid, microcystic and ductal. Ultrastructurally the tumor cells contained large numbers of membrane-bound intracytoplasmic secretory vacuoles. In addition, numerous intracytoplasmic as well as intercellular lumina containing abundant secretory material were noted. Secretory carcinoma in adults is rare with only twenty cases reported previously in the literature. It is known to have a better prognosis than the usual ductal carcinoma. The most appropriate treatment appears to be an extended simple mastectomy.

Rapid cytologic diagnosis of surgical specimens.

The application of a rapid cytologic procedure for examination of tissue submitted for immediate frozen section diagnosis is reported. Its use in various anatomical locations is described briefly. It is apparent that this technic has wide applicability in the cytologic diagnosis of lesions from all organs. It has been used by the author for the past twenty years and has been found applicable to all types of lesions from all organs.
