Aluminium absorption in infancy.

The use of aluminium-containing medications and aluminium contamination of infant formulae is common. We aimed to determine whether aluminium absorption occurs after antacid ingestion. Plasma and urinary levels of aluminium were measured before and after antacid therapy in seven infants whose mean gestational age was 36 +/- 2 weeks and postnatal age 11 +/- 5 days. Antacid therapy (400-800 mumol aluminium) was given with feeds for 2 days. Plasma aluminium levels increased and reached toxic levels (0.64 +/- 0.33 mumol/L vs 3.48 +/- 2.86 mumol/L, P = 0.029). Urinary aluminium: creatinine ratio also increased. These results demonstrate that infants absorb aluminium from antacids and raise the concern of aluminium toxicity.

Delivery of vitamin A from parenteral nutrition solutions in neonates.

The delivery of Vitamin A from parenteral nutrition may be suboptimal. To investigate this sample solutions were prepared and Vitamins A and E concentrations from the solution bags and the efflux from the lines were measured over a 24 h period. The results show that mean Vitamin A levels in the solution bags declined from a range of 57-66% to 17% of expected over 24 h and at the efflux of the lines from a range of 13-29% to 4% of expected, giving a calculated delivery of 10% of expected. There is no improvement with light protection or an ethylene vinyl acetate system. Mixing the vitamin preparation in lipid showed less decline (from 92 to 70% of expected over 24 h), but the delivery was variable. When vitamins were added to a lipid-dextrose-amino acid solution, there was minimal loss from the solution bag and line with a calculated delivery of 94% of expected. The delivery of Vitamin E from all systems was constant with a mean calculated delivery of 74% of expected. It was concluded that the mixing of multivitamins in dextrose-amino acid-electrolyte solutions results in poor delivery of Vitamin A and this is improved by mixing lipid solution. This is important in preterm infants who are prone to become Vitamin A deficient.

Aluminum concentrations in infant formulae.

The aluminum concentrations in breast milk and in 25 commercially available infant formulae were measured. The mean concentration in breast milk was 49 micrograms/L while concentrations in most of the humanized formulae were less than 500 micrograms/L. Higher concentrations were found in Nan, Prem Enfamil and the three soya formulae. We suggest that all formulae have the potential to be contaminated with aluminium, and to varying degrees in different batches. Until it is known whether aluminium toxicity occurs in normal infants fed these formulae, it seems reasonable to expect manufacturers to routinely measure aluminium and keep aluminium contamination to a minimum. This may be especially important for formula fed to infants with compromised gastrointestinal and renal systems.

[Magnification mammography using 0.1 mm microfocus. Comparison of grid and spot-view magnification].

Phantom measurements of resolution and dosimetry of 0.1 mm microfocus spot magnification mammography and 0.3 mm grid technique using different film-screen systems form the basis of this study. The diagnostic efficiency of spot views with these methods is evaluated according to the detection and analysis of microcalcifications, and the assessment of tumor margins.

[5MHz realtime sonography of the breast. 1. Technical equipment studies].

The resolving characteristics (depth dependence of spatial resolution) of different scanning units are tested by a specially designed tissue-mimicking phantom. The phantom includes cyst-like structures (0.8-5 mm) in several layers. Additional measurements of the lateral resolution (-6 dB beam width) are performed on a spherical test object. The results are compared with regard to the requested imaging depth (visualization of the breast muscle) and the optimum focus zone (depth of pathologic breast lesions) in 307 echomammographic examinations. It is shown that the resolution characteristics and penetration depth of a 5 MHz linear-array-real-time-scanner (LS 3000, Picker Inc.) with special near-range-focus best meet the requirements of breast echography.

[5MHz realtime sonography of the breast. 2. Examination technic and diagnostic value].

The diagnostic value of mamma-sonography using realtime technique was evaluated in a prospective study based on the experience of 452 pathomorphologically confirmed sonographic results. Large cysts (greater than 1 cm) were correctly classified as 97%, while small cysts (less than or equal to 1 cm) were recognized in 84%. Eighty-six percent of the fibroadenomas were visualized; 50% of these lesions were classified as "benign" and 34% as "equivocal--probably benign". In 70% of the patients with mastopathic alterations, no abnormalities were detected sonographically, except when cystic lesions (greater than 3 mm) were present. Fifty-percent of the in-situ-carcinomas and small carcinomas (less than or equal to 1 cm) were recognized, while 97% of the larger carcinomas (greater than 1 cm) were demonstrated. The frequency of the different echographic criteria was analyzed with regard to the pathohistological results.

[Rare computed tomographic and sonographic findings in epithelial body tumors. Report on 2 cases].

Carcinomas and adenomas of the parathyroid glands are not always visualised by sonography and via CT as space-occupying lesions of low echogenicity or as dense contrast enhancing tumours. Parathyroid tumours may be present even if ultrasound or CT detect cystic and/or calcified space-occupying growths in the neck or cranial mediastinum. Attention should be regularly paid to enlarged regional lymph nodes.

Sonographic morphology of the breast.

The interpretation of ultrasound mammograms is complicated by the heterogeneous tissue structure of the breast. To improve the diagnostic analysis, a method has been developed which enables us to correlate ultrasound images of mastectomized specimen with the cross-sectional anatomy, the computerized tomography, the X-ray photographs of the specimen and the histopathologic study of the corresponding gross serial sections. Some echophenomena considering the propagation properties of ultrasound are discussed.

Ultrasonic diagnosis of malignant and benign human breast lesions.

Since 1979 445 ultrasound examinations of the breast have been performed. Histological examinations took place in 268 cases. There were 91 patients with 97 carcinomas (5 patients had multicentric carcinomas). In every case x-ray mammography preceded ultrasound examination. The over-all detection rate of malignant breast lesions was 90% by ultrasound examination and corresponded well to the x-ray mammographic findings with 93%. However, false positive results were obtained in 28% of the patients on the basis of sonographic examination. Cysts--with an incidence of 12% in all benign changes in this study--were always correctly diagnosed by ultrasonography. According to these results ultrasonography of the breast may be considered a meaningful additional method to preoperative examination.

[Echomorphology of the breast. Comparative sonographic, radiologic, anatomic and histologic studies of breast specimens].

The interpretation of ultrasound mammograms is complicated by the heterogeneous structure of breast tissue. To improve the diagnostic analysis, a method has been developed which enables us to correlate ultrasound images of mastectomized specimens with the cross-sectional anatomy, the computed tomography, the x-ray photographs of the specimen and the histopathologic study of the corresponding gross serial sections. Some echophenomena considering the propagation properties of ultrasound are discussed. Echodense areas correlate well with inhomogeneous structures of the glandular tissue, i.e. the continuous change of microcystically alterated alveoli, dense and looser connective tissue, dilated ducts and small fat lobules. The histomorphologic equivalent to less echogenic lesions is represented by a homogeneous tissue structure, for example the homogeneous fibrosis of glandular tissue or tumourous tissue. Even fatty tissue appears echopoor. Shadowing phenomena are explained by refraction and absorption effects. The topographic comparison of echograms with identical x-ray photographs of specimen slices emphasizes that the sonographic examination of the breast can enhance the radiographic findings and therefore improve the diagnostic accuracy.

[Comparative studies of various echomammography techniques].

Until now it is not clear which echographic scanning technique is most effective and economic for breast examination. To answer this question we examined in a special study 189 patients with the following scanning units: 1. automated immersion scanning technique, 3 MHz (Octoson, Ausonics) 2. manual compound scanning unit (skin contact), 5 MHz (80 LDI, Picker) 3. linear array realtime scanner (skin contact), 5 MHz (SAL 20, Toshiba). Two examiners evaluated the image quality with regard to the visualisation of the histologically confirmed malignant lesions (n = 78). For testing the spatial resolution, we developed a tissue--mimicking phantom including cyst-like structures of different sizes. The clinical and technical advantages and disadvantages respectively of these three methods are discussed.
