Behavioral treatment of a case of psychogenic urinary retention.

The case involved a thirteen-year-old girl with a history of different urological disorders who since the age of four showed several rituals associated with micturition as well as progressively intense urinary retention. Micturition occupied 2 to 12 hours a day. Treatment consists of systematic desensitization through imagery and in vivo together with progressive response prevention of ritualized behaviours. Cognitive therapy and parent counselling is also used. Normality is attained after 7 weeks treatment and maintained at 18 months follow-up. Psychiatric diagnosis is simple phobia to micturition.

[Compliance with tuberculosis treatment in adults in Santiago, Chile].

A prospective study was conducted to estimate the current magnitude of adherence to short-course tuberculosis treatment, the degree of abandonment, the characteristics of treatment dropouts, and the causes of this abandonment. The study group was made up of tuberculosis patients over the age of 15 who received care at the Western and Southern Health Services of the Metropolitan Region of Santiago, Chile, between 1 October 1987 and 31 January 1988. The percentage abandoning treatment, calculated by the life table method, was 11.5. The profile of patients who dropped out of treatment was as follows: male, under 45 years of age, single, low level of education, no steady work, homeless, and alcoholic. In addition, an opinion survey on the variables associated with abandonment was conducted and it was concluded that the main ones were alcoholism and intolerance to tuberculosis drugs. Awareness of this profile makes it possible to take measures to prevent patients from abandoning treatment, as well as to educate and even hospitalize at the start of treatment those tuberculous patients exhibiting such a profile.

[Pulmonary cryptococcosis].

The authors present the clinical history of a 61 year old woman who was treated in the Metropolitan Hospital Complex Arnulfo Arias Madrid of the Social Security. The patient had experienced weight loss, chills and pain in the left chest for a period of one month. The chest X-ray showed an irregular oval shaped pulmonary infiltrate, 5 x 5 cm, in the upper one third of the left lung field. Fiberoptic bronchoscopy revealed an endobronchial lesion in the superior division of the left upper lobe bronchus. Transbronchial biopsy and bronchial brushings showed Crytpcoccus neoformans. Clinical evaluations did not reveal involvement of the central nervous system, the immunological system or evidence of neoplastic disease. The patient was treated with Amphotericin B and Ketoconazole resulting in a complete cure. Amphotericin B was used on an ambulatory basis for the first time in their institution and in Panama.

The incidence of injuries in young people: I. Methodology and results of a collaborative study in Brazil, Chile, Cuba and Venezuela.

Injuries and accidents are acknowledged as leading causes of mortality among children and adolescents in the developing countries of the world. However, little is known of the extent of non-fatal injuries and of their potential risk factors. The Pan American Health Organization sponsored the first collaborative study to examine morbidity incidence in specified areas of four selected countries in Latin America, and to test the feasibility and practicality of the developed methodology for application in other regions of the world. The study subjects were injured children and adolescents (0-19 years of age) presenting at the study hospitals in the chosen urban centres, as well as injured that were surveyed in households in the catchment area of the hospitals. Falls constituted the most common (40-52%) cause of injury in all areas, and tended to occur in the younger age groups. Motor vehicle injuries were not as frequent (5-24%) as expected from mortality studies. Males outnumbered females 2:1. The home was the most frequent (37-57%) site of injuries, especially for younger ages. Few received medical care at the site of the injury or en route to a hospital, if they went at all. The results found are consistent with those found in other studies in the developed world. The study methodology provides initial valid information for investigating the injury situation in countries with limited resources.

Eating attitudes, sociodemographic factors and body shape evaluation in adolescence.

The Eating Attitudes Test (EAT) and another questionnaire aimed at sociodemographic and body shape information were administered to an adolescent population of 1554 students from Barcelona in the 12-19 age group. The highest scores on EAT were obtained by girls, older adolescents, overweight adolescents, adolescents practising restrictive diets, adolescents considering themselves obese and those wishing to modify their body size. The same characteristics were associated with EAT scores higher than 30 (the cut-off score, established by the authors of the questionnaire, above which the anorexic population is placed). Results are analysed in terms of sociocultural pressures on adolescents, particularly females, who are subjected to the 'culture of slenderness'.

New beta-lactamase-resistant cephem treatment of guinea pigs infected with Legionella pneumophila.

The in vivo antimicrobial effect of seven new beta-lactamase-resistant cephems (cefotaxime, latamoxef, ceftazidime, ceftriaxone, cefotiam, cefbuperazone, and MT-141) on Legionella pneumophila (strain 81-066, serogroup IV) in guinea pigs was compared with that of erythromycin. As the minimal LD100 within one week was about 4.0 X 10(9) CFU/ml by intraperitoneal injection of the strain, the animals were inoculated with 2.0 ml of twofold dilutions of a suspension of this bacterium. The animals developed purulent peritonitis and systemic involvement demonstrated by the development of periangitis, pneumonia and pleuritis in the lungs. Three different doses of antibiotics were administered intraperitoneally immediately after the rectal temperature reached more than 40 C. Erythromycin had a significant therapeutic effect but none of the new cephems tested death of the infected guinea pigs.

[Clinicopathological correlations in the common atrioventricular canal].

20 patients with endocardial cushion defects were studied. Partial forms of common atrioventricular canal and those associated with complex congenital malformations were not considered. The right and left ventricular inflow and outflow tracts were measured and average values of inflow tract/outflow tract were calculated for each anatomical form of the disease. We found this ratio to be significantly smaller in Group B2 compared to Group B1 of Enriquez de Salamanca's classification, the first group being considered to be a more severe congenital malformation than the second. The inflow tract/outflow tract ratio of the right ventricle ranged between 80% and 50%, with an average of 62.3% in Group B1 compared to 62% and 24% with an average of 49% in Group B2. Using Studert's test, this difference was shown to be statistically significant (p = 0.02). On the other hand, there was no correlation between the other parameters or between the severity of the anatomical lesions and the clinical condition. The reduction of the right ventricular inflow tract may give an indication as to the type of atrioventricular valve which may be present.

An outbreak of hantavirus pulmonary syndrome, Chile, 1997.

An outbreak of 25 cases of Andes virus-associated hantavirus pulmonary syndrome (HPS) was recognized in southern Chile from July 1997 through January 1998. In addition to the HPS patients, three persons with mild hantaviral disease and one person with asymptomatic acute infection were identified. Epidemiologic studies suggested person-to-person transmission in two of three family clusters. Ecologic studies showed very high densities of several species of sigmodontine rodents in the area.

[The usefulness of biofeedback in children with encopresis. A preliminary report].

Children with encopresis (costiveness) have a social problem, and (BFB) offers them a valid therapeutic alternative. The present prospective study compares the advantages of this technique with conventional treatment in 21 patients, with average ages of 10.13 and 8.54 years in each group. The patients were studied by clinical, manometric and electromyographic parameters. Those treated with BFB showed clinical improvement, with manometric significant enhancement (p < 0.001) of the percentage of internal anal sphincter (IAE) relaxation, relaxation interval of the IAE and rectal sensation threshold (RST), on the other hand, patients treated by conventional therapy only improved the RST (p < 0.01). Biofeedback seems useful in the treatment of the child with encopresis.
