Estramustine phosphate ( Estracyt ) was used in 32 patients with a mean age of 73 and a half years suffering from oestrogen-resistant carcinoma of the prostate. These carcinomas were advanced and were divided into 26 stage D and 6 stage C. Treatment was given orally at a dose of 600 mg per day. Results were assessed on the basis of reliable subjective and objective selected criteria. Objective responses were obtained in 28,1% of cases and subjective responses in 40.6%. All the patients in whom there was an objective response showed a subjective response. Objective action was more marked on the primary tumour than on metastases. There was a decrease in bone pain, an improvement in general condition and disappearance of dysuria in more than a third of all cases. When there was a response, it always occurred before the end of the 2nd month and was maximal at 3 months. The mean duration of a response was 29.1 months for objective responses and 27.7 months for subjective responses. Survival of patients responding to treatment was markedly longer (by 15 months on average) than in patients who failed to respond. The low level of toxicity of the compound, even after prolonged use, makes its use possible in all patients. Thus Estracyt is felt to have a role in the treatment of the severe forms represented by hormone-resistant carcinomas of the prostate.
The authors study the effects of local instillations of Thiotepa in 22 patients with superficial bladder tumours not involving the basement membrane. Thiotepa is used as an anti-recurrence agent following radical surgical treatment. The procedure consists of instilling 30 to 60 mg of Thiotepa which may be combined with 7,500 units of urokinase, depending on the age of the patient. The product is left in the bladder for two hours. The instillations are performed weekly for the first month and then monthly. The duration of the treatment varies between 18 months and 24 months. The objective responses to the treatment included a complete response in 36% of cases, where no recurrence was detected, and partial responses in 9% of cases, where the annual recurrence rate was reduced by more than 50% in the period following treatment. All the results were confirmed by statistical analysis. The tolerance of the treatment was very good.
Nephroblastoma is a rare tumour in the adult and the diagnosis is only rarely made preoperatively. In the light of a new case, the authors consider the problems of histopathological differential diagnosis with other borderline forms. They study the clinical aspects of these tumours. The prognosis would seem to be extremely gloomy, worse than that of renal adenocarcinoma.

The authors report a new cases of cystic lymphangioma of the adrenal diagnosed at the time of investigation of systemic hypertension which became normal after the lesion was treated surgically. The place of these lymphangiectasis cysts amongst adrenal cystic lesions, their clinical manifestations, methods of diagnosis and treatment are envisaged.
The authors present a homogeneous series of stress incontinence of urine in the woman treated by a method of indirect suspension of the cervico-urethral region. Results were assessed after a follow up of between three months and twelve years, with more than 5 years in 23 patients. After studying the underlying condition in which the disorder developed, they analyse the long term results which are presented using an actuarial technique. The deterioration in results seen during the first year (20% failures) was often related to associated instability of the detrusor. After one year the results remained stable with follow up periods of more than 10 years.
Cis-diamino-dichloro-platinum (DDP) was administered to fourteen patients with a carcinoma of the bladder recurring at the original site and all analysed from a histological standpoint. All the patients had received multiple therapy previously. The tumours treated included the following: eight superficial tumours not infiltrating the chorion, four tumours involving the chorion, one tumour involving the internal muscle layer and one tumour in situ. The age range of the patients was 52 to 81 years with a mean of 69.6 years. Administration was by intra-vesical instillation of 100 mg of DDP each week for the first six weeks, then every two weeks for the next two months and finally once a month. Results were assessed on the basis of endoscopic appearance, cytology and, where applicable, biopsy. Amongst the superficial tumours not infiltrating the chorion, results were as follows: three complete remissions of respective durations of 8 months, 5 months and more than 2 months; three partial remissions of respective durations of 7 months, 6 months and 5 months; and finally two minor remissions of 5 months and 2 months. There were no failures. Amongst tumours invading the chorion, results were as follows: one complete response lasting for 10 months; 2 minor remissions of short duration: 2 months and 2 months; and one failure. Treatment of the one tumour invading the internal layer of muscle was a failure. Finally, the tumour in situ was difficult to evaluate but showed a complete response lasting for 2 months.
The authors have observed this type of fracture in 5 adolescents. They describe the etiology and the mechanism of trauma by internal rotation of the tibia combined with eversion of the foot. These fractures create a tibio-fibular diastasis and disrupt the articular surface. It is concluded that conservative treatment is not satisfactory and that the bone fragments should be fixed accurately by screwing, using an anterolateral approach.
The authors report the results of a trial of the treatment of recurrences of advanced carcinomas of the bladder using Cis-Diamino-Dichloro-Platinum administered by continuous IV infusion over a period of 24 hours at a dose of 100 mg/m2, under cover of hyperhydration and anti-haematizing drugs. The trial involved 10 patients who has had the following initial treatment : - excision for 8 of them (2 total cystectomies for T4, one total cystectomy for T3B, 2 total cystectomies for T3A and 3 partial cystectomy for T1); - preoperative cobalt therapy for 7 of them (2 flashes of 7.5 gray) - postoperative chemotherapy (VM26 and 5FU) for 5 patients. The targets evaluated were : 7 remaining pelvic tumours or recurrences, 3 lung metastases, two cases of venous compression with oedema of the lower limbs, 3 node metastases, 3 bone and 1 sciatic metastases. Results : - 1 complete remission with disappearance for 10 months in the same patient of pulmonary metastases, venous compression and an inguinal node; - 3 partial remissions for 2 to 9 months; - 2 minor remissions of 3 to 5 months; - 3 stabilizations and 1 progression. It is interesting to note the greater efficacy on distant metastases (2/3 complete disappearances of lung metastases, 2/2 disappearances of oedema of the lower limbs) than on pelvic tumours (4/7 regressions). In total, 4 objective responses out of 10 occurring between the 2nd and 6th week after the first course. The authors announce a valuable synergism of DDP with cyclophosphamide and adriamycin.

The authors describe the unusual symptomatology presented by 2 cases of traumatic bronchial rupture in childhood. The first concerns a boy with rupture of the left superior bronchus revealed by a contra-lateral hemopneumothorax. The second cases concerns a young girl with rupture of the right main bronchus: wheezing appearing on the tenth day permitted the diagnosis. They emphasize the eventuality of partial bronchial lesions in cases of moderate and serious thoracic trauma in the child, which should be investigated by bronchoscopy the slightest doubt exists.
