[Automatic re-utilization of capillary dialysis].

The possibility of multiple use of Cuprophan capillary dialyzers reutilized with our own method with a help of an automatic apparatus produced according to our own constructive assumptions was evaluated. The experiments were carried out using capillary dialyzers of the Organon Technika firm, Andante H.F. type with 8 mu thickness of a Cuprophan capillary++ membrane and a dysfusion surface of 0.8 m2. Complete sterility of the regeneration process and full wash of the disinfectant (formaldehyde extract) were observed before reusing of the dialyzer. The clearance of creatinine did not lower when the regenerated dialyzers were used. During the first use of the dialyzer it was 105.8 +/- 8.9 ml/min, and during the sixth one--105.5 +/- 9.8 ml/min. In a group of 24 patients first dialyzed using dialyzers only once and then after 4 months' dialyzing with dialyzers used at least 6 times and again after 4 months' of further dialyzing using a dialyzer only once, remarkable changes of the concentration of urea, creatinine, hydrogen carbonates, fibrinogen, the hematocrit indicator, the amount of hemoglobulin, the number of hematocytes, blood platelets and the weights of the patients in blood serum were not observed. Differences in the before-mentioned indicators between the group of patients dialyzed with a multiple used dialyzer and group of 7 persons dialyzed at the same time with a dialyzer used only once were not observed. During 1075 dialyses made using reutilized dialyzers negative phenomena which might be connected with multiple use of dialyzers were not observed in patients. The authors come to the conclusion that the automatic reutilization of their own method does not lower the efficiency of hemodialysis, lowers the risk of the first use syndrome and is well tolerated by patients which with simultaneous lowering the costs of dialysis as is much for multiple use of dialyzers.

[The role of aluminum in the development of osteomalacia in patients on chronic hemodialysis].

On the basis of a retrospective assessment of aluminium content in the bones and morphometric findings in spongy bones of patients maintained on long-term dialyses a trial was undertaken of demonstration of a possible causative effect of this metal on the development of osteomalacia. The analysis included also the role of aluminium in the development of intravitally occurring fractures of bones in these patients. A biopsy specimen was taken from the iliac ala with a modified Bartelheimer's needle. The study comprised two groups of patients. The first one included 10 patients in a mean age of 38.2 +/- 6.6 years remaining, on the average, since 72.3 +/- 33.6 month who had previously evidence of fractures demonstrated clinically and radiologically. In the second group 12 patients of a mean age of, 41.9 +/- 6.8 years on dialyses since 40.3 +/- 12.2 months with demonstrated subperiosteal resorption, and patients with normal radiological appearance of the skeleton were studied. The patients were dialysed using a dialysate prepared from water not previously adapted and containing Al in a concentration of 0.135 +/- 0.102 mg/l. The diagnosis of osteomalacia was based on the morphometric method of Bordier (6). In both studied groups a highly significant rise was demonstrated in the concentration of Al in the spongy bone, and pathological fractures were closely related to the morphometric markers of osteomalacia. Aluminium cumulation in the bone tissue of patients with fractures exerted a significant effect on an increase of osteoid volume and reduction of mineralization fronts, and demonstrated a directly proportional relationship with the time of treatment with dialyses and overall exposure to aluminium in the dialysing fluid. In the group with osteomalacia a linear trend was observed in the increase of osteoid volume related to aluminium retention in spongy bone.

[Studies of the possibility of multiple use of capillary dialyzers. II. Effectiveness of the elimination of urea and creatinine].

By means of the method divised by the authors two types of capillary dialyzers were used many times: MTS C 1.0 model by Fresenius, membrane thickness 11 mu, and 23-08 model by Travenol, membrane thickness 8 mu. At the fourth use of the Fresenius dialyzers it was found in the studies performed at the start of haemolysis that urea clearanic decreased by 8% on the average. This, with the blood flow of 200 ml/min, was significant statistically (p less than 0.05). In the Travenol dialyzers the decrease was by 18% on the average, while the decrease that was statistically significant was when the dialyzer was used for the second time (p less than 0.05). Creatinine clearance at the fourth use of the Travenol dialyzers decreased by 20% on the average--insignificantly, and in the Fresenius dialyzers it decreased significantly at the second use (p less than 0.01), though at the fourth haemodialysis it decreased by 10% on the average. Clearance of urea and creatinine at the end of haemodialysis decreased more prominently than at the start. This caused an increase in the difference between the start and the end of the dialysis, which in the Travenol dialyzers was made visible as late as at the fourth use of dialyzer. The authors think that the data show the capillary dialyzers can be used three or four times.

[Studies of the possibility of repeated use of capillary dialyzers. I. Renovation method and the evaluation of biological reactivity].

A method devised by the authors of renovation of capillary dialyzers, after rinsing the blood and the liquid systems of the dialyzers is discussed. After rising the blood and the liquid systems of the dialyzers by water at 2 atm.--water being conditioned by the apparatus for reverse osmosis--in the liquid system overpressure of 2 atm was produced by water, the by 0.5% solution of sodium hypochlorite. After 2-3 minutes sodium hypochlorite was rinsed out and both systems were filled in by 1-2% formaldehyde solution. Immediately before using the dialyzer in the same patient the fluid system was rinsed with conditioned water, and the blood system with 21 of sterile distilled water and with 41 of 0.9% NaCl solution. In order to facilitate the renovation process a semi-automatic apparatus was designed, which makes it possible to renovate two dialyzers at once. This method makes it possible to use 80% capillary dialyzers four times with lateral input of the blood system, and four times 100% ones with central input. Experiments on animals and clinical observations and bacteriological studies shoved that the renovation process was atoxic, apyrogenic and sterile. No damage of capillaries and no reactions in subsequent use in patients under dialysis were observed.

[Certain aspects of clinically mild, non-tropical chyluria].

Chyluria despite pseudo- banality may pose several diagnostic and therapeutical problems. Its etiopathogenesis is unclear, too. Prolonged loss of the lymph observed in patients with chyluria due to inherited anomalies of lymphatic system may lead to serious deficits and homeostasis disorders. Basing on these thesis, the authors present two patients with chyluria due to inherited anomaly of extraperitoneal lymphatic system and review available literature. Particular role in the clinical follow-up of these patients plays their immunologic status. The authors did note deficiency of cell-mediated immunity measured with skin tests and lymphocyte T blastic transformation test, and increasing IgG deficit, and particularly annoying IgA deficit in the patient with long-lasting, massive chyluria.

[Urinary tract infections in the first three months following kidney transplantation].

Eighty kidney transplants were examined. During the first three months following transplantation, urinary tract infections occurred in 68 patients (85%). Fifty patients (74%) have had a few episodes of infections in the form of either suprainfection or recurrence whereas in the remaining patients there was only one episode of urinary infection. No difference in infection incidence was noted in both men and women. Ninety percent of urinary infections occurred within the first 4 weeks following transplantation. The most frequent cause of the urinary tract infections were gram-negative bacilli of Enterobacteriaceae family. In case of multiple infections there was a high percentage of gram-positive cocci.
