Jejunal mucosa in marasmic children. Clinical, pathological, and fine structural evaluation of the effect of protein-energy malnutrition and environmental contamination.

Seven children suffering from marasmus were investigated clinically, biochemically and morphologically. The fine structure of the jejunal mucosa obtained by peroral biopsy was evaluated. The mucosal changes noted agree with the only other ultrastructural study reported by Brunser et al. (8) and add information on three additional features: an increase in theliolymphocytes, excessive epithelial cell extrusion and abnormalities in the appearances of the mucosal plasma cells, suggesting possible local deficiency in immune function.

[Calculation of the mean pressure of the pulmonary artery and of the valve surface by ECG in mitral stenosis].

From a group of 50 patients affected by pure mitral stenosis with sinus rhythm, we have tried to obtain multiple correlation equations which enable in to calculate, in each particular case, the mean pressure of the pulmonary artery and mitral valvular area from purely electrocardiographic facts. In an additional group of 16 patients we have been able to verify the mentioned equations, placing all the calculated points within the presumed limits of tolerance. So it appears that the electrocardiographic calculation of these hemodynamic parameters is possible, with sufficient approximation for clinical requirements.

Blood Pressure Determination

[Lipomatous hypertrophy of the interauricular septum: diagnosis with transesophageal echocardiography and computerized tomography].

Transesophageal echocardiographic and computed tomographic imaging in a patient with lipomatous hypertrophy of the interatrial septum and advanced left bundle branch block are presented. Transesophageal echocardiography demonstrated a thickening of the atrial septum sparing the fossa ovalis region. Computed tomographic scan at the level of the interatrial septum showed a mass of fat density, and the HIS bundle electrogram showed an advanced infra-his block after intravenous ajmaline. We stress the utility of transesophageal echocardiography in this disorder, and the possible relationship between lipomatous hypertrophy of the interatrial septum and conduction disturbances.

D-xylose blood test (ninety-minute) in assymptomatic children from marginal areas of São Paulo City.

Assymptomatic children from marginal socio-economical area of São Paulo city, classified as "normal" according to the growth parameters for Brazilian children, were submitted to an oral D-Xylose test and the blood pentose values were checked in fasting time and ninety minutes after sugar ingestion. Based on body weight, the children were divided in three groups. The homogeneity of the three groups' results, as confirmed by statistics, permitted the aggregation of the values in a single one. The lower degree of dispersion suggested a narrow range of intestinal absorption capacity when compared with the value of normal children from well-developed countries. The lower critical normal limit of blood D-Xylose value, around 20 mg/100 ml. coincided with the same parameter for children from matured countries. These facts suggest an adaptation to the difficult environment conditions, that probably is a characteristic among children living in the marginal areas of great cities in developing countries.
