Alcohol-containing mouthwashes and oropharyngeal cancer: a spurious association due to underascertainment of confounders?

Recently it has been suggested that the use of alcohol-containing mouthwashes may increase the risk of oropharyngeal cancer. Heavy alcohol intake and tobacco use are established causes of oropharyngeal cancer. Their use is associated with mouthwash use. In addition, alcohol and tobacco use both tend to be underreported. Here the authors show that, under the hypothesis that mouthwash does not increase the risk of oropharyngeal cancer, confounding due to underascertained exposure to alcohol and tobacco would result in a spuriously elevated odds ratio for mouthwash use. As a general principle, a null association becomes apparently positive if a confounding variable is incompletely ascertained: a spurious association may be produced even in the absence of a difference in the extent of the underascertainment of the confounder among the comparison groups.

Survival analysis techniques in angina pectoris trials.

The variable 'walking time to moderate angina' on an exercise stress test is the primary means to judge the efficacy of new treatments for angina pectoris. Unfortunately, 'walking time to moderate angina' is often censored by fatigue or other reasons for premature termination of the exercise stress test. If time to fatigue is not treatment-dependent, we propose use of survival analysis techniques in such trials. We present an example from a placebo-controlled multicentre clinical trial and results of simulations that compare various methods of analysis.

Analysis of two-period crossover design in a multicenter clinical trial.

A technique is discussed for analyzing a two-period crossover design for a multicenter trial using identical study protocols. The technique is a modification of the analysis originally proposed by Grizzle (1965, Biometrics 21, 467-480; 1974, Biometrics 30, 727) for analyzing a two-period crossover design when study is not a factor. A mixed model using the first baseline as a covariate is analyzed to increase the power of the test of significance of the treatment-by-period interaction. The baseline values are also used in a preliminary test.

A clinical trial with an interim analysis.

We consider a fixed-sample parallel-group clinical trial with an interim analysis that tests H0:mu x = mu y against H1:mu x less than mu y. If we do not reject at the interim analysis, then the probability of making a type I error by rejecting H0 in favour of H2:mu x greater than mu y and the power at the final analysis are not appreciably affected by performing the interim analysis for certain relevant critical regions.
