Kveim induced lymphocyte reactivity in patients and household contacts of patients with Crohn's disease.

In vitro lymphocyte thymidine uptake in response to validated Kveim antigen CR-1 was studied in 18 patients with Crohn's disease and in 30 of their household contacts. Two of the patients and 3 of the contacts demonstrated a stimulation index of 2 or greater. None of 15 control subjects demonstrated such reactivity. Lymphocytes from patient and household contact groups exhibited a greater mean increment in cpm than those from the control group (110, 130 and 33 cpm, respectively) (p < 0.05 and p < 0.005, respectively). This finding indicates a low level of lymphocyte reactivity to Kveim antigen in patients and in contacts, suggesting sensitization to Kveim antigen or to a cross-reacting antigen.

Morphologic, biochemical and physiologic alterations in a case of idiopathic hypoalbuminemia (analbuminemia).

A patient with idiopathic hypoalbuminemia is described. A study of albumin kinetics demonstrated slowed albumin degradation suggesting low albumin synthesis. Morphologic observation of hepatocellular alterations suggested decreased protein synthesis. The intra- and extravascular space was low with an abnormally large postural shift of intravascular fluid into the extravascular compartment. Disturbances in the concentration of plasma lipids and of several plasma proteins were detected. The effect of albumin infusion on these physiologic and biochemical abnormalities suggests that most occurred as a secondary response to the hypoalbuminemic state. Evaluation of the patient's kindred revealed no members with hypoalbuminemia.

Gastrointestinal polyposis with orocutaneous hamartomas (Cowden's disease).

A patient with Cowden's disease and multiple gastrointestinal polyps is presented. An association of gastrointestinal polyposis with this syndrome is made and the histopathological features are described. The cliniical features of Cowden's disease along with its familial tendency are discussed. Emphasis is placed on the importance of considering this diagnosis when evaluating patients with multiple gastrointestinal polyposis. Screening of these patients and their families for the associated malignancies should be performed.

Antiviral activity in sera of patients with Crohn's disease.

Sera from patients with Crohn's disease were tested for antiviral activity using a microtiter assay with end points showing 50% reduction of cytopathic effects. Patients were significantly more likely to have serum antiviral activity equivalent to interferon levels of 20 or more units/ml than were control subjects. The mean antiviral activity in the patients' sera (34 +/- 20 (s.d.) units/ml) was significantly greater than that in the control subjects' sera (0.6 +/- 1.7 (s.d.) units/ml). Following acidification the mean antiviral activity of the patients' sera (28 +/- 17 (s.d.) units/ml) had not changed significantly while heat inactivation resulted in significant loss of activity (7.5 +/- 2.7 (s.d.) units/ml). These findings are suggestive of the presence of circulating leucocyte interferon in the sera of patients with Crohn's disease.
