The management of abdominal wall stomata: skin protection, peristomal wound healing and support for the collecting bag. A multi-center study.

The specific properties of Stomahesive tested in 116 stomal and 20 fistulous patients render it really useful in stomatherapy where it increases the comfort of the patients: 1. it offers an excellent peristomal skin protection whatever the nature of the stoma or origin of the fistula; 2. it promotes rapid healing of skin lesions, even in ileostomy, transverse colostomy, ureterostomy and fistula patients; 3. it ensures an effective degree of impermeability; 4. it provides a large base for adhesion of the collecting bag; 5. it can be kept on for about 6 days in most cases; 6. The local tolerance is excellent (Acta chir. belg., 1977, 76, 533-537).

Surgical anatomy of hyperparathyroidism.

The number of glands and the exact location of each normal and pathological parathyroid identified in 416 parathyroidectomies for primary and secondary hyperparathyroidism have been recorded during the last 25 years (1966-1990). Nineteen per cent of all glands were found in ectopic locations. Half of these were distant from the thyroid lobes, most frequently posteriorly alongside the oesophagus, or in the upper anterior mediastinum encapsulated in the thymic remnant. Their discovery required, in some cases, extensive dissections, but a sternotomy was necessary in two cases only. Parathyroid glands may be concealed in the thyroid or in other aberrant locations, more rarely encountered. Supernumerary parathyroids were observed in 5% of cases. They were often pathological and were often in abnormal positions. A sound knowledge of the anatomic variations and of the paths of embryologic migration of the parathyroids are the essential clues for their detection.

[Selection and training of future surgeons. A survey on Belgian surgical services].

A survey in 47 surgical departments has investigated the selection of applicants for a surgical residency and their education and training. The study indicates that certain factors already endanger the quality of surgical education, that might even worsen during the next years. Most important are the fragmentation of surgical training in numerous departments and the excessive number of surgeons in our country.

Deficiency Diseases↗

[Mediastinal parathyroid cyst].

Parathyroid cysts of the mediastinum are rare lesions. They are, in most cases, non functional and the symptoms are those of a mass compressing adjacent organs. The clinical characters and the pathogenesis of these cysts are discussed.

Granuloma of the distal extremity of the ureter in kidney transplantation.

A granuloma of the distal extremity of the transplanted ureter was diagnosed in 8 of 180 patients operated on according to the Lich-Gregoir technique. This lesion was not observed in 116 patients with Politano-Leadbetter ureterovesical anastomosis nor in 61 patients with a ureterocystostomy constructed according to the technique of De Campos Freire et al. This complication seems to be specific of the Lich-Gregoir technique.

[Acute renal insufficiency in surgery: the surgeon's role].

The surgeon has an active role to play in the treatment of acute renal failure (ARF) occurring in surgical patients. Surgical complications are very often the cause of ARF and their treatment is the only way to cure uremia.

Acute Kidney Injury↗

[Sipple's syndrome (author's transl)].

Presentation of a case of Sipple's syndrome with bilateral malignant pheochromocytoma, bilateral thyroid medullary carcinoma and two parathyroid adenomas. The principal characters of the syndrome, the diagnostic tests, the necessity of a prospective survey of family relatives are discussed.
