Distribution of the receptor for advanced glycation end products gene polymorphisms in patients with chronic periodontitis: a preliminary study.

BACKGROUND: Periodontal diseases are viewed today as multifactorial problems initiated and sustained by bacteria but significantly modified by the body's response to bacterial plaque. A recent study suggested that receptor for advanced glycation end products (RAGE) could be involved in the pathophysiology of periodontitis. The aim of this study was to investigate a possible association of 3 common polymorphisms in the RAGE gene with chronic periodontitis. METHODS: We studied 101 Caucasian patients with chronic periodontitis together with 162 orally healthy subjects. Three polymorphisms, one in intron 7 (1704G/T), second in intron 8 (2184A/G), and the third in exon 3 (G82S) of the RAGE gene, were investigated by polymerase chain reaction methods (PCR) with subsequent enzymatic restriction with Bfal, BsmFI, or Alu Il, respectively. RESULTS: A statistically significant difference in allele frequencies between patients and the reference group was found for intron variant 1704G/T (P= 0.02, Pcorr >0.05). There was no significant difference in genotype or allele frequency distributions between groups for intron variant 2184A/G or for the exon variant exchanging amino acid Gly for Ser at position 82 (G82S). CONCLUSIONS: We can speculate that susceptibility to the development of chronic periodontitis could be influenced by the 1704G/T polymorphism of the RAGE gene, independently of diabetes.

[Diabetes mellitus in ambulatory stomatosurgical practice].

The authors evaluated in 58 patients aged 27-65 years the pre- and postextraction bacteremia after a simple extraction at intervals of 5 minutes, 1 hour, 24 and 72 hours. From the group of 58 examined patients 35 suffered from diabetes mellitus--11 type 1, 24 type 2. Before extraction bacteremia was not found in any of the examined patients. Five minutes after extraction bacteremia was recorded in 50% of the diabetic patients, more frequently in type 1 than type 2. Bacteremia was found in 8% of patients with type 1 diabetes even after 24 hours. The authors recommend to assess in addition to the blood sugar level also so-called glycohaemoglobin which they began to assess regularly in diabetic patients.

[Problems of work incapacity of ambulatory patients at the Second Stomatological Clinic, Faculty Hospital with Policlinic, Pekarská, Brno].

The authors analyze the development of work incapacity in ambulatory patients treated in 1979-1987 by sex, age and period of work incapacity. They used graphic illustrations of time series with a regression line which indicates a rising or declining trend of work incapacity. The analysis revealed that the work incapacity has, with the exception of men aged 15-24 years, a rising trend in particular in women. According the authors this can be partly explained by changes and extension of therapeutic activities in ambulatory surgeries. Certain reserves are also in the planning of therapeutic procedures and control activity.

Freeman-Sheldon syndrome: a disorder of congenital myopathic origin?

Freeman-Sheldon syndrome was diagnosed in an unrelated adult man and woman, with severe abnormalities of the extremities but only slight anomalies of the face. Electromyography and muscle biopsy showed a myopathy which was classified as a congenital disproportion of fibre type and seemed to be the primary cause of the deformities. This allowed classification of the syndrome as a separate type of myopathic arthrogryposis.

[A fatal disease of the heart in primary and secondary hemochromatosis].

In a 53 year-old man suffering from idiopathic hemochromatosis a massive myocardial siderosis occurred with disperse necroses and fibroses especially in subepicardial zone of the left ventricle. A similar picture was observed at secondary hemochromatosis in a 33 year-old woman with sideroblastic anemia after repeated blood transfusions. Both patients died of the failure of hypertrophic heart.

Spondyloepiphyseal dysplasia.

Skeletal dysplasia with autosomal dominant inheritance was found in four members of one family and in one sporadic case. The syndrome comprises brachydactyly E, platyspondyly, abnormality of the sacroiliac joint, disturbance of metaphyseal modelling, epiphyseal dysplasia, and short stature. This study deals with a particular type of spondyloepiphyseal dysplasia and compares it with similar cases in two previously published papers.
