Haemodialysis-related porphyria cutanea tarda is a rare, but serious and mutilating skin condition, resulting from extremely high plasma porphyrin levels because of their inadequate clearance by haemodialysis. The treatment is very difficult as chloroquine is ineffective and venesection, the conventional treatment of this disease, is not always an option because of anaemia of end-stage renal disease. We report a case of haemodialysis-related porphyria cutanea tarda and her successful management by recombinant human erythropoietin treatment.
Why 100 patients failed to keep an outpatient appointment-audit in a dermatology department.
Failure to keep outpatient appointments is common at all clinics and various explanations may be offered. One hundred attending patients who had failed to keep their previous dermatology clinic appointment were asked the reason for their non-attendance. Many and varied reasons were given but illness (28%), and problems related to appointments (33%) were prominent. They were also asked how they had obtained a further appointment. Stricter follow-up of non-attenders by the hospital including informing the GP, and subsequent GP action if necessary, may improve attendance figures.

Woolly hair--study of a family.
A white English family is described with autosomal dominant woolly wiry hair. The propositus had woolly curly scalp hair, congenital perceptive deafness and dominant ichthyosis vulgaris. Her brother was epileptic and also deaf, but his skin and hair were normal and a male first cousin had dominant ichthyosis vulgaris but normal hair and hearing. Consanguinity was an interesting additional feature in this family.
Angiokeratoma of vulva.
A female of 66 who presented with long-standing pruritus vulvae showed angiokeratoma of the vulva as an incidental finding. This benign tumour is uncommon and in a survey of 150 patients over 50 years old (mean age 76.9), no further example was found.

Buschke--Ollendorff syndrome (disseminated dermatofibrosis with osteopoikilosis).
The case is reported of a woman aged 47 years with disseminated dermatofibrosis associated with osteopoikilosis. Her two children also showed skin lesions typical of the Buschke--Ollendorff syndrome and these lesions were said to have been present at birth.
