[Laryngeal and tracheal complications of prolonged intubation].

Based on a retrospective study of 595 patients having undergone prolonged intubation, the authors present the main complications encountered and, in particular, the mucous ulceration which appears to be systematic and is itself at the origin of secondary stenosizing or granulomatous sequelae. Research is still needed concerning the follow-up of the intubated patients in order to limit the pressures exerted between the cordal mucosa or the tracheal mucosa in contact with the endotracheal tube. A systematic check upon removal of the tube decreases the secondary sequelae by starting adapted antacid, anti-inflammatory and antibiotic treatments, as well as certain acts of laryngeal microsurgery and, in some cases, laryngeal rehabilitation for both the voice and deglutition.

[Technical indications and results of reconstructive laryngectomies].

The results of a series of 164 patients operated on using reconstructive laryngectomy, gives satisfaction for this type of operation if the indications are respected after a very specific endoscopic, radiological and per-operative loco-regional and local check-up. The functional results are clearly improved by a pluridisciplinary treatment associating a nursing team, orthophonists and physical therapists.

[Vocal pathology of teachers].

Out of 100 teachers referred to a phoniatric department for dysphonia, we recorded 11 dysfunctional lesion-free dysphonias, 86 dysphonias with lesions, 3 post-surgical dysphonias, 76 patients referred for a dysphonia with a modification of the spoken voice, and only 3 for the singing voice. 96 suffered from vocal fatigue, and in the course of the clinical examination 85 patients presented a faulty vocal attitude. The most frequent lesions were nodules and para-nodular formations. Vocal re-education was prescribed in 90 cases, and laryngeal microsurgery in 25 cases. The other medical therapies are not be overlooked, such as physiotherapy, thermal cures, psychiatric treatment, treatments of the terrain, and endocrinic treatments. A vocal examination, better information and vocal training of future teachers will make it possible to obviate such disorders.

[Malignant midline granuloma and T-cell lymphoma. Apropos of a case].

Malignant granuloma of the face, a rare condition with many clinical designations of controversial etiopathology, would in fact appear to correspond, in the majority of recently published cases, to a T malignant lymphoma pathology. The freezing of a sample for immunohistochemical analysis, and more effective therapeutic choices, even though their aggressivity may seem out of proportion to the patient's condition, should make it possible to improve the prognosis of this condition which is still dangerous as the reported case shows.
