Arterial embolization of post-partum hemorrhage.

The case of a 24-year-old patient with severe post-partum hemorrhage resistant to medical therapy is reported. Active bleeding was documented on CT and confirmed by angiography. Selective embolization of the cervico-uterine artery promptly stopped the bleeding. Selective embolization is recommended as the treatment of choice for intractable post-partum hemorrhage.

Lhermitte-Duclos disease. A case report.

We report one case of histologically proved Lhermitte-Duclos disease. Suspected on the basis of CT and angiography, the pre-operative diagnosis has been confirmed by the characteristic features of the lesion on MRI: a unilateral posterior fossa mass, hyperintense on proton density and T2-weighted images, hypointense on T1-weighted images, non enhancing after Gd administration. The gyral pattern was preserved, but a specific thickening of cerebellar folia was demonstrated. Despite a good post-operative short term prognosis, recurrence may occur. With a better depiction of lesion limits, MRI could improve the surgical approach and reduce the risk of subtotal excision.

[Aneurysm of the renal artery and its infarct complications (clinical case and literature review)].

The authors are reporting the case of a 74 year old man, presenting a renal artery aneurysm. Regarding clinical aspects, medical treatment was elected. The most frequent cause of renal infarct is embolisation arising from the heart, followed by large vessels emboli that are more exceptional. Embolisation arising from the renal artery aneurysms is not very common. Unless contra-indications, surgical cure of the aneurysm is necessary, except those that are non complicated, with a diameter of less than 1.5 cm.

[Traumatic arterio-venous fistula. Experience with 9 cases].

Traumatic arteriovenous fistulas (TAF) are rare forms of vascular injuries. The authors report an experience of nine new cases. Three of them result from a minor traumatism, while the six others are iatrogenic. The symptoms and the physical examination are very helpful in the diagnosis. Selective arteriography is the most useful paraclinical examination to confirm the affection and for the choice of the mode of therapy: surgery, selective arterial embolization, alone or used with surgery according to the characteristics and the location of the fistula.

A modified method of CT pelvimetry.

The authors describe a simplified routine procedure for CT pelvimetry. The procedure was successfully performed in 122 women with a 2% false positive rate corresponding to obese women. The originality of the procedure consists in the development of a simple technique of correction of the widest transverse diameter based on the anteroposterior topogram. The values obtained are transformed into real values of the transverse inlet diameter measurements. The procedure presented is simple, accurate, and enables considerable reduction of fetal radiation exposure.

[Preoperative localization of parathyroid adenomas: significance, indications and results].

Preoperative localizing technics of a parathyroid tumour. The authors present a limited experience about preoperative localizing technics of a parathyroid tumour. Oesophagography and selenomethionine 75 scintigraphy were not useful. Ultrasonography and computed tomography localised 50% of the tumours. Selective thyroïd venous catheterization and radioimmunoassay are correctly predictive in 87,5% of the studied cases. The interest, morbidity and results of these different technics are discussed.

Management of hepatic artery aneurysm. Case report and literature review.

The authors report the case of a 72-year-old man with new but vague epigastric pain. CT scan and selective coeliac trunk angiography demonstrated a common hepatic artery aneurysm (HAA). He was treated surgically by ligation and excision without revascularization. Asymptomatic hepatic aneurysm should be treated aggressively because the natural course seems to be progression to rupture associated with a high mortality rate. The authors discuss the clinical scenario of patients with HAA and stress the importance of the early treatment. In addition the therapeutic options are explored.
