[Dysphagia and cervical osteophytosis: case report and review of the literature].

Dysphagia with anterior osteophytosis of the cervical bone is a more and more reported affection, especially after the age of 60. However it remains a rare event. The cause to effect relationship between these two entities should be documented by a complete assessment. Surgical treatment is reserved for patients whose medical treatment has failed. We describe an original case of cervical bone osteophytosis which was diagnosed during an acute and febrile aphagia and review the literature about its assessment and treatment.

What future in the 21st century for tibial bone allografts in ossicular chain reconstruction: the state of the issue.

After using tibial cortical bone allografts in reconstructing the middle ear over a period of ten years, the authors analyze their results. They compare the functional and anatomical outcome at 4 years of 24 patients who received this allografts with a series of 25 patients who underwent incus transposition. Results were analyzed in two different ways: the Air Bone Gap and the Glasgow Benefit Plot. The discussion addresses the techniques used to reduce risks of iatrogenic transmission of viruses and prions, the current legislation in Belgium and the comparative costs, after refunding of the various reconstruction materials. The authors conclude that, because the present-day preparation techniques offer a certain security with regard to transmission risks and because of its numerous advantages (mechanical properties, tolerance, cost and availability), tibial cortical bone still has a promising future.

Upper airways locations of plasmocytoma.

We report one case of extramedullary plasmocytoma located in the nasopharynx. After 8 years, the tumor recurred in the same location and 3 years later, the patient develop a multiple myeloma. Three clinical and pathological entities of plasmocytoma are described: multiple myeloma, solitary bone plasmocytoma and extramedullary plasmocytoma. When histological examination show plasmocytoma, further examinations can or not reveal a multiple myeloma. In 80% of cases, extramedullary plasmocytoma occur in the upper respiratory tract. This development of multiple myeloma from extramedullary plasmocytoma is seen in 5 to 32% of patients and sometimes an extremely long time is required for the change to occur. The treatments are surgery and radiation in the absence of generalized disease.

Genomic signature: characterization and classification of species assessed by chaos game representation of sequences.

We explored DNA structures of genomes by means of a new tool derived from the "chaotic dynamical systems" theory (the so-called chaos game representation [CGR]), which allows the depiction of frequencies of oligonucleotides in the form of images. Using CGR, we observe that subsequences of a genome exhibit the main characteristics of the whole genome, attesting to the validity of the genomic signature concept. Base concentrations, stretches (runs of complementary bases or purines/pyrimidines), and patches (over- or underexpressed words of various lengths) are the main factors explaining the variability observed among sequences. The distance between images may be considered a measure of phylogenetic proximity. Eukaryotes and prokaryotes can be identified merely on the basis of their DNA structures.

Narrow and vacant internal auditory canal.

A case of unilateral congenital deafness revealing a narrow vacant internal auditory canal and a more anterior and superior second canal where the facial and vestibulocochlear nerves are well visualised is presented. Having reviewed the scientific and embryological data, the authors consider the mechanism of this malformation.

[Primary meningioma of the geniculate ganglion: apropos of a case].

Based on a clinical observation and a literature review, etiologic hypothesis, clinical, radiological and therapeutics characteristics of intratympanic primitive meningioma are described. Those tumors arise from arachnoid cells, some may be carried along the nerve sheath within the Fallopian canal during embryology.

Long-term results of various surgical treatments for cholesteatoma.

The authors have studied 11 series of results following cholesteatoma surgery by closed or open technique. The closed technique was performed in 77%. The notion of surgical revision is not accepted by all surgeons; it is practised with an average of 59% for the whole series. Residual cholesteatoma and/or recurrences are observed in 34% of the cases of which 20% have been discovered at the time of revision surgery. Surgical revision appears mandatory to the authors.

Bone allografts in reconstructive middle ear surgery.

The authors present their current experience with stored bone grafts, using allografts shaped from the cortices of long bones for reconstructing the tympano-ossicular chain. The materials and the methods are described. The anatomical results have been good in 97% of the cases, while the functional results are as satisfactory as those obtained with bioceramics. In addition to ossiculoplasty, the bone allografts can also be used in otology for reconstructing large bony defects of the temporal bone.

[Osseous reconstruction in microsurgery of the middle ear].

The authors report their technique and results about the use of bone allografts in tympano-ossicular reconstruction. They use a mixture of bone dust and fibrin sealant for repairing small bone defects. Furthermore, the results of cavity reconstruction techniques with hydroxylapatite and tricalcium phosphates are compared.
