Molecular prenatal diagnosis in families with fetal mitochondrial trifunctional protein mutations.

OBJECTIVES: To evaluate the feasibility of molecular prenatal diagnosis in families with mitochondrial trifunctional protein (TFP) mutations and prospectively study the effects of fetal genotype on pregnancy outcome. TFP catalyzes the last 3 steps in mitochondrial long-chain fatty acid oxidation. STUDY DESIGN: We performed molecular prenatal diagnosis in 9 pregnancies, 8 in 6 families with isolated long-chain 3-hydroxyacyl-coenzyme A dehydrogenase (LCHAD) deficiency and one in a family with complete TFP deficiency. Analyses were performed on chorionic villous samples in 7 pregnancies and on amniocytes in 2. RESULTS: Molecular prenatal diagnosis successfully identified the fetal genotype in all 9 pregnancies. Two fetuses were affected, and both pregnancies were terminated by family decision. Two other fetuses had normal genotype and 5 others were heterozygotes. These 7 pregnancies were uncomplicated, and all the offspring are alive and apparently healthy. Genotypes of the aborted fetuses and neonates were confirmed by molecular analysis and enzymatic assays. CONCLUSIONS: Molecular prenatal diagnosis is possible and valid in guiding management of pregnancies in families with known TFP defects. Women heterozygous for TFP alpha-subunit mutations who carry fetuses with wild-type or heterozygous genotypes have uncomplicated pregnancies.

Attentional requirements for object-location priming.

The purpose of the present experiments was to investigate whether a verbal and a spatial secondary task would disrupt priming for object-location associations. Symbols were placed one at a time in one of nine locations in a rectangle. Implicit memory was tested with a reaction time (RT) task. All symbols were placed in the same location of the rectangle across 10 trial blocks; then, all their locations were changed. Responses were made on the numeric keypad, which corresponded to the spatial locations used in the rectangle. A decrease in RTs across the first 10 trial blocks followed by an increase in RTs when the symbols changed locations would indicate priming for the associations. The results were that implicit memory for object-location associations was obtained under single-, but not under dual-task conditions. We have interpreted the results in terms of a working memory model that posits that implicit memory will suffer when cognitive resources are limited.

Pharmacological management of post-traumatic stress disorder: clinical summary of a five-year retrospective study, 1990-1995.

Beginning in 1990, the Department of Psychiatry. Tripler Army Medical Center developed a formal treatment program for post-traumatic stress disorder (PTSD). Between 1990 and 1996, 632 patients, the vast majority of whom suffered from combat-related PTSD, were treated. Historically, many PTSD patients were treated with benzodiazepines, often in high dosages. The risks attendant to benzodiazepine management of PTSD, coupled with poor clinical outcome, prompted the staff to explore treatment alternatives. This paper describes the role of pharmacotherapy in the management of PTSD. The medications described in this paper have other primary uses in clinical practice (e.g., hypertension, insomnia, seizure control, depression, and anxiety). Medications were selected for use based on the putative modes of action and the degree of symptom relief. The therapeutic rationale was to decrease hyperarousal and sleep disturbance to permit the patients to engage in other psychotherapeutic efforts.

A 6-hydroxydopamine-induced selective parkinsonian rat model.

Previous parkinsonian rat models have generally been characterized by unilateral destruction of both the nigrostriatal pathway and the mesolimbic pathway using the neurotoxin 6-hydroxydopamine (6-OHDA). We created a hemiparkinsonian model in which there is 6-OHDA-induced destruction of the dopaminergic nigrostriatal pathway but sparing of the dopaminergic mesolimbic pathway. This resulted in reproducible, quantifiable rotational behavior in response to either amphetamine or apomorphine and a near total depletion of dopamine in the striatum ipsilateral to the lesion with a dorsolateral distribution of supersensitive dopaminergic D2 receptors. This model parallels the MPTP (1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine)-induced hemiparkinsonian model in primates and more closely approximates the extent of neurodegeneration seen in human idiopathic Parkinson's disease than previous parkinsonian rat models. It may therefore prove a convenient model for studying the recently reported phenomenon of sprouting from host dopaminergic neurons following tissue implantation.

Isolation of drugs from post mortem specimens with the automated sample processor. Part I. Barbiturate derivatives.

Employing an automated sample processor, barbiturate positive tissue homogenates were analyzed to yield gas and liquid chromatographic data. Excellent quality extracts were produced that could be assayed by either GC/NPD or HPLC. Good precision and accuracy was found (CV 2-5%) with approximately 200 mg of tissue sample. The technique has been applied to authentic post mortem tissues and fluids.
