[Prostaglandin E1 in the long-term treatment of erectile dysfunction].

Intravenous injections of Prostaglandin E1 were administered to 65 men suffering from erectile dysfunction. Later on, 59 of them administered the substance to themselves. Our experience as well as literary data have confirmed the efficacy of prostaglandin E1 administered intracavernously in the therapy of different forms of impotence, including the so-called psychogenic form. Its advantage is that it metabolizes directly in the cavernous tissue. Virtually no significant systemic side reactions were recorded and neither priapism nor fibrosis of the corpora cavernosa were observed after long-term treatment. Prostaglandin E1, a product of Léciva Praha, was used within the clinical study of this preparation.

[Experimental cryptorchism].

The authors evaluate the effect of hormonal treatment with chorionic gonadotropin on histological structures of the testes in experimental work and in artificially induced cryptorchism in the rabbit; they evaluate histological findings in cryptorchid and normally descended testes.

Common skin lesions of the elderly.

The elderly patient can present with a variety of skin lesions of the head and neck. This article is designed to acquaint the otolaryngologist-head and neck surgeon with normal skin morphology and with the changes that occur with aging and sun exposure. Common benign and malignant skin lesions of the aging patient are discussed.

[The sex life of men after radical cystoprostatectomy].

Radical cystoprostectomy during treatment of infiltrating tumours of the urinary bladder interfere with the haemodynamics of the lesser pelvis and innervation of the sexual organs and leads to loss of erectility. Intravenous administration of vasoactive substances such as papaverine and phentolamine improved the erectile mechanism in six of seven patients and enabled three patients to resume an active sexual life. Intracavernous administration of vasoactive substances is effective in the treatment of erectile dysfunction after radical cystoprostatectomy. In indicated cases it can replace the use endoprostheses of the penis and enables the patient to resume sexual life.

[Use of semirigid penile prosthesis in the treatment of erectile dysfunction].

Implantation of a pair of semirigid penile prosthesis is a simple and satisfactory mode of treatment for erectile dysfunction in selected patients. Though the functional cavernous tissue is destroyed by implantation of the prosthesis into the cavernous body, the sensitive nerves are not damaged by the procedure. Therefore, the sensitivity of the penis, as a factor important for further sexual life, remain intact. The authors report their own clinical experience with implantation of semirigid silicone penile prostheses, made in Czechoslovakia. In 26 out of 28 patients they observed good functional and cosmetic results and the treatment allowed the patients to return to the active sexual life. The advantages of the used Czechoslovak prostheses, which are not to be neglected, are their availability and relatively low price.

Adult

Low sperm counts as a prognostic factor of progression in stage I non-seminomatous germ cell testicular tumours.

The prognostic factors of disease progression were studied in a prospective study in patients with Stage I non-seminomatous testicular tumours. Patients received no treatment after orchiectomy until progression of disease was confirmed. The results showed a good correlation between sperm counts in semen collected around the time of orchiectomy and progression of disease. While progression of disease occurred in 9 of the 20 patients with sperm counts lower than 10 X 10(6)/ml, it was not found in any of the patients with a sperm count exceeding 10 X 10(6)/ml.

Adolescent

Wegener's granulomatosis with bilateral necrotizing scleritis, polyarthritis and renal failure efficiently treated with immunosuppressive therapy.

A case report of a female patient with Wegener's granulomatosis is presented. After an initial involvement of the upper respiratory tract in the form of a sinusitis, there followed a severe necrotizing bilateral scleritis necessitating the enucleation of the left eye ball. Renal involvement developed as late as 24 months after the onset of the disease and led to renal failure within three months. Throughout the duration of her disease, the patient had joint symptoms in the form of episodes of migratory nondeforming polyarthritis. The administration of corticosteroids alone in daily doses up to 60 mg prednisone failed to control the progression of the disease, while immunosuppressive therapy with cyclophosphamide combined with methylprednisolone pulse therapy and haemodialysis resulted in a marked improvement of renal function and in the subsidence of the ocular and articular symptoms.

Testicular dysfunctions in patients after cytostatic treatment of non-seminomatous testis tumours.

In recent two years 28 patients were treated by combined cytostatic therapy for non-seminomatous testis tumour. Eleven months on the average since finishing chemotherapy they underwent andrological examination aimed at the changes of testicular function. Influence of cytostatics--bleomycin, cisplatinum and vinblastine on spermiogenesis of patient, on contralateral testis unaffected by tumour and on potency (data on erection, orgasm, volume of ejaculate and coitus frequency) was observed. 20% of patients had diminished contralateral testis. Decrease of ejaculate volume was stated by 64% of patients. Spermiogenesis damage was established in all patients (86% of patients had azoospermia and 14% had difficult oligoasthenospermia). One of the possible solutions of this negative effect on male fertility is represented by cryopreservation of semen before introduction of chemotherapy.

Ring-shaped alkali burns of the rabbit cornea. Epithelial and endothelial changes.

Rabbit corneas were burned by touching their surfaces with rings of Whatman No. 1 chromatographic paper soaked in 1N NaOH. The corneal epithelium and endothelium were studied by impregnating flat sections with McGovern's and Jabonero's silver stains. Immediate destruction as well as regeneration of both cellular layers is described.
