Tumors of the rectum and anus.

Within a ten year period we have performed 433 radical operations on the rectum. An anterior resection of rectum was done in 110 cases, low anterior resection in 60 patients, very low anterior resection was done by stapler in 32 patients and was sewn by hand in 10 patients. Colo-anal anastomoses were performed 86 times and abdominoperineal amputation of rectum sec. Miles 20 times. Local transsphincteric procedures were used 14 times. In 12 patients the colo-anal anastomosis was performed either for recurrence of tumor or after a Hartmann's procedure. In the carcinoma of the lower half of the rectum there is still an unsolved question: whether to perform a low or a very deep low anastomosis or whether an extirpation of rectum with sparing the sphincters and sewing a colo-anal anastomosis is more beneficial. In the years 1980-1995 we treated surgically 64 patients with a tumor in anal area. All patient underwent preoperative radio- and chemotherapy. In our group adenocarcinoma was demonstrated in 6 patients, squamous carcinoma in 48 patients and squamous cells of the carcinoma in situ in 10 patients.

[Repeat laparoscopy in the management of biliary leaks after laparoscopic cholecystectomy].

The incidence of bile leaks increased with the introduction of laparoscopic cholecystectomy in surgery. The present paper is focused on biliary leaks-their diagnosis and treatment in a large group of patients, with special emphasis on a miniinvasive approach. Nine biliary leaks were found (i.e. 0.72%). Four leaks resolved spontaneously, five were treated surgically with good results.

[Spontaneous pneumothorax--modern views on its therapy].

Based on a retrospective study the authors describe their experience with the treatment of spontaneous pneumothorax in 159 patients during a 5-year period/1989-1992/. For the diagnosis they used, in addition to clinical and X-ray examination, in indicated cases also scintigraphy of the lungs, CT examination, thoracoscopy and bronchoscopy. During the period of investigation the basic therapeutic operation was thoracic drainage/one or several drains/, whereby in recent years there were more indications for surgery. Relapses were recorded after simple drainage in 46.5% of the patients, after surgery the authors did not record any relapses of spontaneous pneumothorax. Based on their own experience and data in the literature the authors recommend the radical approach to treatment of spontaneous pneumothorax using the least invasive videothoracoscopic technique which they have been using ever since 1993.

[Surgical treatment of small-cell pulmonary carcinoma].

Based on their longstanding experience of a department with surgical treatment of small-cell carcinoma of the lungs, the authors give an account of their experience. During the five-year period (1988-1992), when also patients in the third stage of the disease were operated, the authors followed up the survival after operation in relation to the presence or absence of N2. During the above period 31 patients were operated on account of small-cell carcinoma. This number comprised 9 patients in stage I and II, 14 patients in stage III, and in 8 patients on account of the advanced stage of the disease explorative thoracotomy was performed. Although the patient groups during the mentioned period were not large, the results are comparable with those of other, larger investigations. Patients operated in stages without tumourous affection of the mediastinal lymph nodes survived resection of the lungs supplemented by adjuvant chemotherapy significantly longer than patients with N2 affections and a similar therapeutic procedure. Based on the achieved results the authors do not recommend resection of the lungs in the IIIrd stage of the disease as the primary or as an adjuvant therapeutic modality.

[The present status of thoracic surgery in Slovakia].

The authors submit a report on the state of thoracic surgery in the Slovak Republic, based on the number and structure of thoracic surgical operations made in 1983-1987. Twenty departments were concerned with thoracic surgery and 21 surgeons had the qualification of thoracic surgeons. The authors mention only briefly problems pertaining to the organization of the discipline.

[Idiopathic unilateral chylothorax].

A 40-year-old female patient developed spontaneous right-sided chylothorax which after a single thoracocentesis did not relapse. Available examination methods did not reveal the cause of the disease. From the clinical course it may be assumed that the chyle drainage was impaired in the thoracic duct or in a minor lymphatic vessel by subclinical injury, whereby the damage obviously was small, capable of spontaneous closure and healing. The authors present some contemporary views on the pathogenesis, diagnosis and treatment of this rare disease.

The role of relaparoscopy in the management of bile leaks after laparoscopic cholecystectomy.

The incidence of bile leaks has increased with the advent of laparoscopic cholecystectomy. The present paper is focused on bile leaks--their diagnosis and management approaches with special consideration to relaparoscopy. From February 1992 to May 1995 a total of 1223 laparoscopic cholecystectomies were performed in two hospitals. Eight biliary leaks were found in the series under study (i.e. 0.65%). The diagnosis was confirmed by means of ultrasound, CT scans and ERCP. Three leaks resolved spontaneously on external drains placed during the operation. Two and three patients underwent laparotomy and relaparoscopy, respectively. The etiology of the leakage included three cases of ducts of Luschka, one cystic duct leak, and one retained CBD stone. the source of the remaining leaks was not determined. In all cases the management approaches comprised relaparoscopy and laparotomy. It may be concluded that an early diagnosis and management of bile leaks after laparoscopic cholecystectomies would prevent further complications.
