Experience with an elemental diet (Vivonex).

Six patients with intractable atopic dermatitis completed this study which consisted of a baseline week, 1 to 2 weeks on an elemental diet, then sequential specific food additions. Symptom/sign scores, total eosinophil counts, and IgE and IgG4 radioimmunoassays for wheat alpha-gliadin, bovine serum albumin, beta-lactoglobulin, and egg white were carried out as part of the protocol. Symptom/sign scores improved in five of six patients, some dramatically. Total eosinophil counts generally dropped after a week on the elemental diet and then increased as foods were added to the diet. The elemental diet used (Vivonex) appears to be beneficial in the management of at least some adult patients who have severe atopic dermatitis.

A new electronic sphygmomanometer: testing observer reliability and instrument comparability.

A new electronic sphygmomanometer, Visomat II, featuring a new microphone sound chamber and signal light as a substitute for stethoscope use, was tested in two experiments. The study first measured the degree to which two persons observing the same instrument would have similar readings. Standard aneroid and mercury instruments were also tested. This was done to compare readability of the three scales. There were no statistically significant differences in the readings of the three types of units. In the second experiment, pairs of different sphygmomanometers were connected with Y fittings. Technicians compared in random order the similarity of readings of the three possible pairs of units. There were no statistically significant differences in the three pairings. The authors conclude that the new electronic instruments is comparable to the standard aneroid and mercury units.

Use of laminar air flow portable filter in asthmatic children.

A new, portable laminar air-flow filter (Enviracaire) was tested on 13 asthmatic children, using a double-blind single crossover study. There was a statistical improvement in symptom scores during filter use; however, peak flows did not improve significantly. The authors feel this method of avoiding indoor airborne irritants is useful and warrants further study.

A family practice survey in Ventura County.

Seventy-five percent of family practitioners in Ventura County, California, responded to a seven-page mailed questionnaire. One third of the practitioners do no obstetrics. The physicians counsel seven patients to every delivery performed and to every surgical procedure performed. Family practice consists of medicine and pediatrics, rather than surgery, obstetrics and psychiatry. Family practitioners use referrals to community health care services in a limited fashion. Lack of information about availability of community resources may be the reason.
