[Formaldehyde sediment in incubators following disinfection].

Measurements in incubators revealed the presence of formaldehyde concentrations involving a health risk for premature and normal newborns kept and cared for in incubators. Prior to measurements, the incubators had been disinfected by means of formaldehyde vapours in an "Aseptor" disinfecting cabinet (Drägerwerk AG, Lübeck) and then ventilated in strict adherence to operating instructions. The elevated formaldehyde concentrations found had been due to residues of paraformaldehyde and urotropin on the surfaces of the disinfected apparatus, liberating formaldehyde by hydrolysis depending on temperature and relative humidity. There should be a basic reconsideration of the present practice of incubator disinfection. From experiments with activated-carbon filters in incubators it would seem that there is a chance of reducing such formaldehyde concentrations.

Prevalence of abnormal involuntary movements ("spontaneous dyskinesias") in the normal elderly.

Advancing age has been associated with increased prevalence of abnormal involuntary movements in neuroleptic-treated patients. In order to investigate the possibility that the aging process itself might produce involuntary movements clinically indistinguishable from tardive dyskinesia, a group of healthy elderly volunteers were systematically examined. A 4% prevalence rate of mild involuntary movements was found, suggesting that the aging process itself is not likely to produce dyskinesias without other contributing factors coming into play.

Platelet associated IgG, platelet survival, and platelet sequestration in thrombocytopenic states.

Platelet-associated IgG (PAIgG), platelet mean life span (MLS), and platelet sequestration sites were studied in 69 patients with immune (ITP) and presumed nonimmune thrombocytopenias (NTP). A shortened MLS was associated with elevated PAIgG (N = 46), and with normal PAIgG (N = 15). Four patients had a normal MLS, but elevated PAIgG, four patients were normal for both parameters. The highest PAIgG values occurred in ITP patients with a very short MLS. Nine NTP patients had also elevated PAIgG, but a normal or slightly shortened MLS. There was a significant double log correlation between PAIgG and MLS for ITP, but not for NTP patients. Judged from the coefficient of determination, only 10% of PAIgG were directly related to a shortened MLS. 70% of patients (N = 63) had exclusively splenic and 30% hepatosplenic sequestration. PAIgG was elevated in 29/44 patients with splenic (66%) and in 16/19 patients with hepatosplenic sequestration (84%). In ITP, PAIgG-positive cases were observed in 69% of splenic v 82% of hepatosplenic sequestration, while in NTP the corresponding figures were 6/11 and 2/2. No significant correlation between PAIgG and either sequestration type was demonstrable. We conclude that in immunologically mediated thrombocytopenia only a small portion of PAIgG accounts for a decreased MLS, and that the concentration of PAIgG per se does not determine the platelet sequestration type.

A prospective study of tardive dyskinesia development: preliminary results.

Preliminary findings from a prospective study of tardive dyskinesia (TD) development in a population of psychiatric patients suggest a TD incidence of 12% after 4 years of cumulative neuroleptic exposure. No cases of abnormal involuntary movements that might be mistaken for TD have been found in patients with no history of neuroleptic exposure. Early findings with regard to risk factors are discussed.

[The significance of air exchange for air quality in homes].

The air exchange is an important parameter for the quality of the air in living spaces. It should be guaranteed by appropriate construction measures, such as by the choice of air permeability of joints, that the air exchange rate in living spaces is high enough to reduce the concentration of pollutants emitted indoors. Based of the measurement of the air exchange rate in about 100 rooms, a minimum ventilation rate of 0.8 h-1 is proposed for living spaces. Since higher rates may cause an increase in energy consumption, this value should not be exceeded by much. Appropriate measures should be taken to limit emissions from building materials, furniture, cleaning and polishing products and the like. If suitable heat exchange devices were also used in the ventilation of homes, better solutions than those presently existing may be found.

[Hygienic relevance of devices for indoor air treatment].

Shortcomings regarding design, construction, operation (including emissions), maintenance/repair and control of buildings with rooms for the accommodation of persons may be the reason to install air conditioning devices. According to manufacturers' data, such devices may be applied for various purposes, e.g. the creation of a defined air temperature or humidity, an increase of the supply of outdoor air, the cleaning and deodorization of indoor air or the alteration of the so-called electric climate of a room. The hygienic health evaluation of the different types of air conditioning devices should establish whether --there are aspects of health necessitating alterations of the microclimate of a room; --such alterations could be brought about in a more economic way by purely constructional or individual measures; --the function of individual apparatuses could be accomplished in a better way by replacing them by a larger device serving several rooms; --the operation of such devices may produce adverse health effects such as nuisance by noise, formation of undesirable gases (ozone), danger owing to non-adherence to electric safety rules; --there will be no damage to rooms and furniture, e.g. by water droplets. A look at a number of commercially available devices shows that they are generally dispensable. There are, however, special rare cases where the use of such devices may result in an improvement of the quality of indoor environments.

The prevalence of presumed tardive dyskinesia in psychiatric inpatients and outpatients.

Randomly selected psychiatric patients (271 total) were examined by raters blind to diagnosis and treatment history for the presence of abnormal movements. The prevalence of presumed tardive dyskinesia among neuroleptic-exposed patients was 4.6%. If minimal rating scale criteria were applied, 9% of those patients with no history of neuroleptic exposure might have been given 'presumptive' diagnoses of dyskinesia. Problems in establishing diagnostic criteria are discussed and a longitudinal approach toward validating diagnoses is recommended.
