[A clinical trial of pefloxacin (Abaktal) in the treatment of acute pyelonephritis].

The therapeutic effect of Pefloxacin in the treatment of acute pyelonephritis was examined in a randomized clinical trial. The patients in the control group have been treated by gentamycin. With a success rate of 80% in the treatment of acute pyelonephritis caused by various bacteria (E. coli, Pseudomonas, Klebsiella, Staphylococcus aureus, Proteus mirab.), a side frequency of side effects and a good compliance Pefloxacin is recommended as an effective antimicrobiell drug.

[Comparative studies of the effect of mictonorm (propiverin hydrochloride) and Spasuret (flavoxate hydrochloride) on the bladder detrusor muscle].

46 patients suffering from urgency/urge incontinency were treated with Mictonorm and Spasuret in a crossing-over study. In consideration of the placebo effect in this disease both agents were tested versus a non-verum. The application period was 4 weeks and the dosage was 45 mg/d Mictonorm and 300 mg/d Spasuret, respectively. Both with Mictonorm and with Spasuret a significant reduction of micturition frequency and an increase of the compliance could be observed, whereas the placebo was ineffective. A markedly growth of the maximal bladder capacity (16.9%) was obtained only by Mictonorm. Both agents keep likewise to an improvement of the symptoms or urgency/urge incontinency. Side effects could observed in a small size without a breaking-off of the treatment.

Radical pelvic surgery and radiation therapy in the management of localized carcinoma of the prostate.

A total of 181 prostatic cancer patients under radiation monotherapy were tested for the therapeutic effect of pelvic lymphadenectomy (PLA). Sixty-seven belonging to the pN0 category formed a highly selective group and presented a 89% rate of 7-year survival. Localized radiation therapy with pelvic irradiation (81 patients) with conventional exclusion of lymphogenic macrometastases in the probable presence of micrometastases produced equally good results. Extended pelvic irradiation in cases of conventionally verified regional lymphogenic macrometastases remained without therapeutic effect. The combination of PLA with pelvic irradiation, when regional metastases have already occurred (23 patients), may be suited for localizing the tumor process, since the survival rates showed no significant difference compared to those of the pN0 group. For that reason and also because of the difficulty in determining the N category accurately by conventional methods, it is recommended to perform PLA prior to radiation monotherapy.

Combined Modality Therapy

[Intracavernous injection of vasoactive substances in the treatment of erectile dysfunction].

Herein we report our experience with a clinical trial of intracavernous injection of vasoactive substances (Papaverin, Dibenzyran, Regitin) in 41 patients suffering from erectile dysfunction for longer than 1 year. In 14 out of 33 patients the application of papaverin alone was already sufficient to produce an excellent erection (T/E-grade 4) under clinical conditions. The simultaneous injection of two agents (Papaverin + Dibenzyran or Papaverin + Regitin) in patients with no or insufficient response to papaverin improved the results additionally. Besides a positively effect of delay from the injection to the erection could be observed in comparison with an injection of only papaverin. In conclusion, this method combined with an autoinjection therapy is an alternative approach to treat certain patients with erectile dysfunction.

[Personal experiences with phenoxybenzamine (Dibenzyran) in the treatment of functional disorders of bladder emptying].

The influence of phenoxybenzamine on micturition was examined in 46 patients with functional bladder neck obstruction. Due to blockade of the alpha-adrenergic receptors at the bladder neck an increase of the maximal flow rate from a mean value of 13.6 to 24.6 ml s-1 was found during the treatment. In addition, there was a relaxation of the functional urethral length and a reduction of the maximal urethral closure pressure from 96.3 to 74.4 cm H2O. The most significant changes resulting from the application of Dibenzyran were observed in the neuropathic bladder with a reduction of the functional urethral length from 24.3 to 20.5 mm and a decrease of Pura max from 100.5 to 82.7 cm H2O. Furthermore, we achieved an increase of the maximal flow rate from 13.4 to 22.5 ml s-1.

[Lymphogenous metastasis of urinary bladder cancer].

In 83 patients with locally curable bladder cancer of stage T1-T3 an exploratory pelvic lymphadenectomy was performed. The rate of metastases in all patients was 15.66%. In the analysis of lymphogenous metastasizing routes an obligatory direct deposition primarily in the lymph nodes of the obturatorius and/or external group with constantly by-passing of the viscero-vesical nodes was found. From this some conclusion to a modified technical-tactical approach of the exploratory pelvic lymphadenectomy have been done.

[Transurethral electrostimulation in the therapy of postoperative disorders of bladder emptying following extended hysterectomy].

As a suitable method of the treatment of postoperative voiding disturbance following gynaecological surgery (hysterectomy) the indirect electrostimulation of detrusor vesicae is presented. It is possible with this method to treat the congenital (myelomeningocele) and the obtained (spinal cord dysfunction) but also the disturbances following hysterectomy successfully. In cases with such a hypotonic bladder the stimulation of the bladder wall is made with an intermittent application of exponential current so that sufficient contractions to the voiding will occur after the treatment.

[New technical possibilities for simultaneous cystometry].

With the introduction of integrated semiconductor pressure admitters into the urodynamic examination methodology an optimization of the manometry of the bladder and the urethra was carried out, since the changer of the measuring value in this case is directly situated in the lumen of the bladder. Thus it is achieved that the possibilities of errors which are connected with the conventional measuring method with the catheter system filled with fluid or gas, such as movements, concussions, hydrostatic influence of the coupling medium by different altitude level of the catheter and the connection tube, respectively, but also constipations in haematuria or excretions of the mucus may be excluded. The presented tip manometre IBW 74 due to its measures and radiopaque properties is also suited for the measurement of the pressure on the upper urinary tracts including the pyelon. Hereby the pressure signals gained are further elaborated through a special insertion for the biomonitor system and thus may be recorded on every multiple recorder.

[Effect of anticholinergics on bladder tonus and on the voiding function].

When the anticholinergic drugs with an effect which above all peripherally and selectively affects the urinary bladder the following may be proved: 1. Increase of the capacity of the urinary bladder according to the initial situation, 2. no influence on the intravesical pressure in maximum filling of the bladder, 3. decrease of the maximum intravesical pressure during the miction, 4. decrease of the rate of the flow of the urine and 5. increase of the maximum urethral pressure as well as no influence on the functional length of the urethra. From these parameters the adequate clinical application in diseases of the urinary bladder may be derived and at a strong indication good successes in treatment can be achieved.

[The value of the urethral pressure profile in urinary incontinence].

For the differentiation of the incontinence the tonometry of bladder and urethra has become indispensable in diagnostics. The author deals with the advantages of the possibility of a combined examination, the technical performance of the urethra-pressure profile and with the necessity of the same outer conditions. Taking into consideration the functional length of the urethra, the maximum pressure of the urethra as well as the difference between pressure of urinary bladder and urethra in rest and under conditions of stress the criteria for the degrees of incontinence are represented and discussed on the basis of own examinations. Not the position and the absolute length are responsible for the continence, but the relation between the attainable urethra-pressure in contrast to the appearing pressure of the urinary bladder.

[Uroflowmetry and its methodology. Normal limits. II. Normal range].

Prerequisite for every evaluation of a uroflowmetry curve are normal regions, on which the diagnostic evidence of the examination method is based. In the present paper a definite attitude to the limits of the normal was adopted, in which case apart from the course of the curve the relations from maximum flow to miction volume were entered. Diagrams of the normal region were established for men, women, and children, in which cases apart from the interindividual diffusion breadth also the individual diffusion breadth was taken into consideration. Error in the diagnostic course develop always then, when only mean values are taken into consideration. The critical consideration of the normal values to age seems to be important, in which cases a peak of the maximum values of the urinary flow is to be established between the 13th and the 40th year. In the later decades of life also without any subvesical obstruction an exponential function similar decrease of the maximum values of the flow develops.

[Uroflowmetry and its methodology for establishment of urinary flow parameters. I].

In the present paper the author deals in a short form with the importance of the uroflowmetry as a functional examination method. Prerequisite for every documentation of the picture of the miction curve is the diagram of the urinary flow. Here only the direct curve of the flow is suited for the immediate readability of the adequate values as well as for the simultaneous recognition. For the superficial orientation about the course of the flow the control of the curve is enough, but for the differentiated evaluation the following factors must be taken into consideration: Time of flow, time of waiting, different and maximum urinary flow, increase of the curve of the urinary flow and number of the peaks of flow. Also the differentiating uroflowmetry should be taken into consideration, in order to separate neurogenic disturbances of the urinary bladder from obstructive changes of the vesical outlet in the screening method.
