[Countertransference in psychoanalytic therapy of children and adolescents with obsessive-compulsive disorders].

Understanding countertransference in dynamic therapy of children and adolescents is important for the clarification of unconscious conflicts because of the frequent absence of dreams and free association in the analytic process. When compulsions determine the course of the psychic disorder countertransference becomes an important landmark for comprehending and resolving the affect-loaded sadomasochistic collusion as an externalization of intrapsychic conflicts. Compulsive acts and thoughts helping to preserve seriously threatened structures of the self and the ego engender different countertransference reactions in the analyst than the ego-superego-conflicts of compulsion neurosis. This makes them essential for diagnosis and therapeutic interventions.

Anxiety in adolescence and its significance for psychoanalysis.

The theme of this paper is the well-known resistance of adolescents to participate in analytic treatment and analysts' reservations about treating adolescents, which is perhaps less well known. Several reasons are discussed: For adolescents, the revival of separational and fusional anxieties during the process of individuation as well as resistance to the analytic setting are held responsible; analysts may lack sufficient awareness of their own adolescence and its resulting anxieties to be able to confront this developmental period. In addition, there are countertransference anxieties that are triggered and kept alive by the behavior of the juvenile patient. The different types of analyst anxieties as well as those of the adolescent under analysis are explained and their significance in the process of individuation and the course of therapy is noted.

[A contribution to the psychophysiology of infantile dysmegalopsy (author's transl)].

A 9 year old boy was able to evoke microptic experiences voluntarily. Clinical and historical signs of minimal brain dysfunction were present. An epileptic etiology could neither be proven nor ruled out. Synchronized EEG- and Video recording allowed us to analyze the EEG and the corresponding behavior by using sequential spectral analysis of the EEG data. Coinciding with a progressive increase in rhythmic 6-7 cps. activity we observed a behavior which appeared to correspond to a state of narrowed attention. He signalized the onset of a micropsy a few seconds after the transition from this pattern into one which showed a relative voltage decrease and the reappearance of some alpha-activity. The end was signalized by him coinciding with the recurrence of normal waking activity. Our findings--like those of others--suggest that the gradual development of hypnagogic phenomena is bound to a state of lowered vigilance. The reappearance of alpha activity and the voltage decrease seem to represent an unstable intermediary state with partial arousal preceding the restitution of normal wakefulness. Since the onset of the micropsy was signalized only after manifestation of the intermediary EEG-state, we conclude that partial arousal is a prerequisite for giving an indication of hypnagogic experiences in a quasi retrospective manner.

[Neuropsychological and psychiatric aspects of language development disorder of a nine year old patient.(author's transl)].

Problems of terminology, differential diagnosis, etiology and therapy of language development disorders are demonstrated by a casuistic example. Besides the importance of clinical date special attention was laid on neuropsychological diagnostic procedures and their therapeutic implications. Therapeutic interventions concerning language deficits should be developed individually and in accordance to the diagnostic findings. An early and reliable objectivation of etiology as well as of the degree of the language development disorder is necessary in order to avoid detours of the children through different intitutions.

[The long good-bye--paths of psychoanalysis in late adolescence].

These reflections about late adolescence are inspired by a novel by Peter Handke, entitled "Der kurze Brief zum langen Abschied" (original; translated: A short letter about the long good-bye). The protagonist's troublesome and painful separation from his wife taking place during a trip through the United States is interpreted as an attachment-conflict towards the mother that has not been dissolved during late adolescence. The essential characteristic of late adolescence is taking one's final leave from the infantile and adolescent parental imagines and the reapproachment to the real parents. In case of a failure late adolescence may easily lead to a syndrome of rejection or of overadjustment or of drop-out-identity. Especially in the rejection or in the drop-out-identity one can find object-relationships resembling those found in narcicistic or borderline-disorders. In many cases these adolescents lack the transitional scope they would have needed to find their way to autonomy or to the ability of containing (BION).
