Endothelinb receptor agonists produce pulmonary vasodilation in intact newborn lambs with pulmonary hypertension.

The hemodynamic effects of endothelin-1 (ET-1) are mediated by at least two distinct receptors: ETa and ETb receptors. Recently, ETb receptor agonists (4 Ala ET-1 and IRL 1620) were developed. To investigate the role of ETb receptor activation on the pulmonary and systemic circulations, we studied the hemodynamic effects of intrapulmonary arterial injections of these receptor agonists in 10 intact newborn lambs. At rest, 4 Ala ET-1 (290-1,725 ng/kg) changed no hemodynamic variables. IRL 1620 (180-1,095 ng/kg) decreased mean pulmonary arterial pressure (PAP, 16.8% +/- 15.0 and 17.8% +/- 8.5, p < 0.05) and left pulmonary artery blood flow (21.6% +/- 22.1 and 33.4% +/- 27.7, p < 0.05) at the two highest doses only. During U46619-induced pulmonary hypertension, both 4 Ala ET-1 (3.2% +/- 8.0 to 15.9% +/- 6.4, p < 0.05) and IRL 1620 (8.7% +/- 6.3 to 21.9% +/- 4.1, p < 0.05) produced selective dose-dependent decreases in PAP. The decrease in mean PAP induced by 4 Ala ET-1 and IRL 1620 was attenuated by N omega-nitro-L-arginine [an inhibitor of endothelium-derived nitric oxide (EDNO) synthesis] (16.6% +/- 3.5 vs. 5.9% +/- 2.3 and 16.2% +/- 3.4 vs. 6.6% +/- 2.8, p < 0.05) and by glybenclamide (a blocker of ATP-dependent potassium channels) (18.2% +/- 7.9 vs. 7.5% +/- 8.3 and 14.7% +/- 3.6 vs. 6.3% +/- 3.2, p < 0.05). ETb receptor activation produces selective pulmonary vasodilation during pulmonary hypertension in intact newborn lambs. The vasodilating properties are mediated in part by release of ENDO and by potassium channel activation.

Mosaic "tetrasomy" 8p: case report and review of the literature.

A male infant presenting with multiple anomalies including a midline cleft palate, anasarca, hepatomegaly, pulmonary edema, agenesis of the corpus collosum, and complex congential cardiac anomalies was found to have mosaicism for an additional chromosome that appeared (following GTG-banding and FISH) to be a monocentric isochromosome of the short arm of chromosome 8 (46,XY/47,XY, +i(8p)). Nine other cases of mosaicism for an additional i(8p) were reviewed. Considerable phenotypic variation was noted. Consistent features were identified including agenesis of the corpus callosum, cardiac malformations, and minor facial dysmorphology. The phenotype of these patients partially overlaps those of trisomy 8 and trisomy 8p. By studying additional individuals with this condition, mosaic tetrasomy 8p may emerge as a recognizable clinical phenotype.

Models of accreditation in health care and their origins are briefly described. Current approaches to Quality Management systems used in the United Kingdom are discussed, and a recommendation is made for professional leadership in the independent accreditation of health services.

The effects of childhood trauma upon adult recovery from injury and illness.

Many patients with chronic injury or illness suffer from childhood trauma syndrome. As a result, they do not take proper care of themselves and are at risk of developing long-term disabilities. Physicians who understand the dynamics of CTS can help these patients regain control over their own bodies and health. By building therapeutic relationships with these patients, physicians can restore trust and help them lead happier, healthier lives.

An analysis of the sources of musculoskeletal system impedance.

When antagonistic muscles co-contract, the impedance of musculoskeletal systems to applied loads is known to increase. In this paper a physiologically-based, higher-order, nonlinear antagonistic muscle-joint model is utilized to clarify the sources of impedance modulation during a variety of tasks, ranging from resisting transient loads to holding steady loads to making fast movements in unpredictable surroundings. It is shown that impedance modulation occurs automatically as a function of the specific operating ranges utilized during a given task by each of four different muscle-joint mechanical relations. The relative contribution of each relation depends on the type of task, with impedance during quasi-static conditions sensitive to muscle tension-length and sometimes joint parallel elastic properties and during dynamic tasks dominated by the series element and muscle force-velocity properties. Elimination of any of these causes a decrease in built-in biomechanical capabilities. These findings raise questions concerning past theories on stiffness-impedance modulation which appear to underestimate the role of inherent biomechanical properties.
