[Effect of the method of treatment on mean survival time of patients with glial brain tumors].

In the years 1976-1981 54 patients were treated for malignant glial tumours confirmed by histological examination. The purpose of the reported analysis was a comparison of the mean survival time and the mean time of survival without evidence of recurrence in 3 groups of patients. In group I of 13 patients not treated surgically but receiving only conservative management the mean survival time was 7.4 weeks. In group II treated surgically but without other therapy this time was 29.2 weeks. In group III in which the patients were operated on and treated with chemotherapy and radiotherapy this time was 58.9 weeks. The differences between the groups were statistically significant. Similarly, the mean survival time without recurrence was significantly longer in the group receiving comprehensive treatment than in the group treated only surgically (46.5 vs 18.0 weeks).

[Medulloblastoma with multiple metastases beyond the nervous system].

A case of desmoplastic cerebellar medulloblastoma with numerous metastases beyond the nervous system is reported. Fourteen months after the operation the patient began complaining about pains in hip and knee joints and dysphagia. One week before death profuse bleeding from the digestive tract developed. On autopsy numerous metastases were found in the bones, lymph nodes, spleen, pancreas and kidneys. Histological examination demonstrated that the metastases were composed of the same cells as the primary cerebellar tumour, but reticulin fibres were scant in them. In none of the metastatic lesions any significant reticulin proliferation was present, these fibres were found only around small vessels.

[Simultaneous occurrence of meningioma and astrocytoma].

The authors present a case of a 47 year-old woman admitted to Neurosurgical Clinic with diagnosis of left frontal lobe tumour. At operation, during removal of glioma of the frontal lobe, a parasagital meningioma, 3 cm in diameter, was found and removed. Both tumours were adjacent. Histopatologic examination revealed: grade 1 astrocytoma and fibroblastic meningioma. Some clinical and pathological problems associated with simultaneous occurrence of two tumours of various histologic origin are discussed.

[Embryonal hemangioma and adenoma of the pituitary gland in a patient. Case report].

The authors describe a 54-year-old woman treated surgically 28 years earlier for embryonal haemangioma of the cerebellum and admitted with signs of hypophyseal tumour. Radiological examinations confirmed this suspicion and the patient was treated surgically from the approach through the nose and sphenoidal bone. The previous disease and intraoperative observation suggested the possibility of embryonal angioma in the sella but histological examination demonstrated that the tumour was a chromophobe adenoma.

[Treatment of acromegaly by the surgical approach through the sphenoid bone. I. Clinical results].

Forty-five cases of pituitary adenomas with clinical characteristics of acromegaly treated surgically from an approach through the nose and sphenoid sinus are presented. After the operation the somatic features of acromegaly decreased in 66% of cases, headaches regressed in 88%, visual field defects disappeared in 20% of cases. Selective removal of pituitary microadenoma caused return of regular menstrual cycles in 12 women and in 3 of them successful pregnancy and labour resulted. In cases of extrasellar growth of the tumour no satisfactory clinical remission was obtained.

[Internal carotid artery aneurysm in the sella turcica area simulating pituitary tumor].

A giant aneurysm of the internal artery is described which was situated in the sella and simulated hormonally inactive hypophyseal adenoma. Axial computer tomography demonstrated a hypophyseal tumour. During operation from intranasal approach through sphenoid sinus massive haemorrhage developed but was controlled with fibrin sponge. Postoperative angiography demonstrated a giant aneurysm of the internal artery situated in the sella. Although this location of aneurysm is exceptionally rare, each case of inactive hormonally pituitary adenoma should be subjected before the operation to cerebral angiography.

[A case of tumor of the pineal body treated with gamma rays of Co-60].

A 21-year-old patients was admitted to hospital with symptoms suggesting a tumour in the area of the epiphysis. Computer tomography confirmed the initial diagnosis. Since signs were present of increased intracranial pressure a Pudenz valve was implanted and in the second stage of treatment radiotherapy with radioactive cobalt was applied. A very good result was obtained with control CT failing to demonstrate presence of the tumour and hydrocephalus.

[Surgical treatment of acromegaly by the trans-sphenoidal approach. II. Hormonal results].

Growth hormone determinations were done in the serum of 45 patients with acromegaly. In 3 cases the basal GH levels was normal, in the remaining ones it was raised. Of the functional tests those with insulin, glucose and L-Dopa were done. The tests with glucose and L-Dopa loading were found to be more useful diagnostically than the insulin test. After surgical treatment the GH level returned to its normal value in 10 cases, but in most cases it fell to at least one half of the initial value. In 5 cases with more pronounced expansion of the pituitary tumour the GH value was not significantly changed.

[Hippel-Lindau disease. Description of 2 cases].

The authors describe two cases of Hippel-Lindau's disease, one of them associated with numerous morphological changes. The authors discuss the diagnostic criteria of this disease and certain aspects of its inheritance, therapeutic management and prognosis.
