IgDK multiple myeloma presenting as unilateral proptosis.

A 72-year-old woman presented with painful proptosis of the right eye and a large destructive tumour of the middle cranial fossa. A diagnosis of IgDK multiple myeloma was made, based on histopathologic and immunologic studies of the biopsy. Biochemistry and bone marrow examination further confirmed the myeloma as IgDK type. The clinical, radiological, and pathological findings are presented. The patient was treated with radiotherapy with satisfactory results.

Antagonists of the luteinizing hormone releasing hormone (LHRH) with emphasis on the TRP7 of the salmon and chicken II LHRH's.

The sequences of four naturally occurring luteinizing hormone releasing hormones (LHRH's) differ only in positions 5, 7 and 8. Salmon and chicken II LHRH's have Trp7; porcine/ovine (P/O) and chicken I LHRH's have Leu7. The receptor for P/O LHRH might effectively bind certain antagonists with Trp7. Thirteen antagonists having Trp7 and eight antagonists with other substitutions in position 7 were synthesized. One of the thirteen antagonists with the natural Trp7, [N-Ac-D-2-Nal1,D-pClPhe2,D-3-Pal3,D-Arg6,Trp7,D- Ala10]-LHRH, not only maintained activity, but had increased potency (ca. 58%; 90% antiovulatory activity/250 ng; rats) in comparison with the companion analog with the natural Leu7 of P/O LHRH. The other twelve Trp7-antagonists had lower potency.

Iron is sequestered as ferritin in macrophages in skeletal muscle of vitamin E-deficient rabbits.

Weanling rabbits were fed a purified diet with or without vitamin E supplementation to evaluate the abnormal sequestration of iron in skeletal muscle associated with vitamin E deficiency. A severe myopathy developed in unsupplemented rabbits within 3 to 4 weeks. At this time, the concentration of soluble nonheme iron in biceps femoris muscles had increased from 2.1 +/- 0.4 microgram/g wet weight (mean +/- SD) for six control rabbits to 4.3 +/- 1.4 for 10 vitamin E-deficient rabbits, and total nonheme iron had increased from 5.0 +/- 1.2 to 8.4 +/- 3.3. Soleus muscles had even greater increases in total and soluble nonheme iron concentrations. Intramuscular injection of iron-dextran caused large increases in total and soluble nonheme iron in noninjected muscle of vitamin E-deficient rabbits, which further exaggerated the difference between the two groups. By radioimmunoassay using an antibody to rabbit liver ferritin, the concentration of ferritin in biceps femoris muscles increased from 0.47 +/- 0.18 microgram/g wet weight for seven control rabbits to 6.34 +/- 1.70 for 14 vitamin E-deficient rabbits. Uptake of intravenously injected transferrin-bound iron into muscle of vitamin E-deficient rabbits was not increased in a short term experiment (6 h), but radioiron did accumulate in muscle in a long term experiment (6 days). There was no trapping of heat-damaged erythrocytes, no phagocytosis of intravenously injected carbon particles, and no erythrophagocytosis in muscle. An immunohistological staining method designed to detect ferritin in tissue sections stained muscle from normal rabbits very scantily but intensely stained macrophages in the muscle of vitamin E-deficient rabbits.(ABSTRACT TRUNCATED AT 250 WORDS)

Pericarditis as presenting manifestation of acute nonlymphocytic leukemia in a young child.

A case of acute nonlymphocytic leukemia presenting as pericarditis is reported in a five-year-old boy. Initially, a clinical diagnosis of viral pericarditis was made, because the child did not demonstrate hematologic or clinical manifestations of leukemia. Acute undifferentiated or lymphocytic leukemia. Acute undifferentiated or lymphocytic leukemia was diagnosed one week after admission when his peripheral blood count became abnormal. The patient did not respond to vincristine and prednisone. When cytochemical evaluation indicated acute myelomonocytic leukemia, employment of cytosine arabinoside and 6-thioguanine was instituted and the child began to improve. Currently, he is still in good remission and has no evidence of recurrence of pericarditis, 1 1/2 years after his initial presentation. In reviewing the literature, we found 17 patients who had leukemic pericardial effusion with cardiac tamponade. There are three reported cases of young children with pericardial effusion as the initial manifestation of acute lymphocytic leukemia, but no reported cases due to nonlymphocytic leukemia, as in this child.

Synthesis and anticonvulsant testing of 4-phenylsemicarbazides.

A series of compounds based on the semicarbazide structure have been synthesized. Anticonvulsant activity was found in a majority of the compounds using both the maximal electroshock seizure and the subcutaneous pentylenetetrazol seizure threshold tests. Activity of the compounds was weaker than the 1,1,2-trisubstituted semicarbazides previously reported.

Isolated testicular relapse five years after cessation of chemotherapy for acute lymphoblastic leukemia.

A 12-year-old boy was diagnosed as having acute lymphoblastic leukemia (ALL) at the age of 2 years 10 months. Chemotherapy was discontinued after 33 months of continuous remission of ALL. Isolated testicular relapse occurred 5 years after cessation of treatment. Isolated testicular infiltration if it occurs, usually does so within one year after discontinuing treatment. Late bone marrow or extramedullary relapse usually does not occur after four years post therapy. Testicular relapse in this patient occurred five years after completing chemotherapy.

Autoimmune hemolytic anemia in childhood Hodgkin's disease.

Autoimmune hemolytic anemia (AIHA) is very rarely associated with neoplasia in childhood. However, we have observed AIHA in three children with Hodgkin's disease (HD) in the past 7 years. AIHA presented either prior to, simultaneously with, or years after the diagnosis of HD. Antibodies against erythrocytes were nonspecific. The relationship of AIHA to the activity of underlying HD was quite varied among these three patients. Surgery, radiation therapy, and chemotherapy all may alter the course of AIHA. Coombs' tests may continue to be persistently positive when anemia is absent. Currently, all three patients have no evidence of recurrence of HD, no anemia, negative Coombs' tests 5 years, 22 months, and 14 years 10 months after the initial diagnosis of HD. AIHA should be considered in a child with HD who develops anemia and HD should also be considered in the differential diagnosis in a child with chronic AIHA.

Ovarian tumor as manifestation of relapse in acute lymphoblastic leukemia.

Acute lymphoblastic leukemia (ALL) was initially diagnosed in a 12-year-old girl. Maintenance chemotherapy was discontinued after 32 months of continuous remission. Relapse of ALL occurred ten months after cessation of chemotherapy. Twenty-one months after relapse, she presented with a large ovarian tumor due to leukemic infiltration while her bone marrow and central nervous system (CNS) were still in remission. She remained in bone marrow and CNS remission when disseminated leukemic infiltration of the peritoneum was found seven months later. She died of bone marrow relapse 11 months after ovarian relapse, six years and two months after the initial diagnosis. In contrast to testicular relapse, ovarian relapses in acute lymphoblastic leukemia are rarely reported.

Low-dose adriamycin remission maintenance for advanced acute lymphoblastic leukemia: a Southwest Oncology, Group study.

A low-dose maintenance schedule of adriamycin was evaluated in children with advanced acute lymphoblastic leukemia. Thirty-six evaluable patients who achieved two or more remissions were given adriamycin, 10 mg/m2/week, as a single maintenance agent. The median duration of second remission was 17 weeks, but only 5 weeks for third and fourth remission. Although the therapy was not found effective in more advanced disease, the median duration of second remission approached that obtained from various multi-agent treatment schedules. We conclude that the therapeutic efficacy of this treatment schedule makes it appealing for incorporation into multi-drug combination regimens.

Anemia in children and adolescents with hypothyroidism.

In a review of 17 adolescents and children (excluding newborns) with definite clinical signs, symptoms, and laboratory findings of hypothyroidism, 11 patients (65%) had anemia. The mean corpuscular volume (MCV) of the red blood cells was either macrocytic or normocytic. The hemoglobin did not correlate with the serum thyroxine level. Anemia occurred only in those patients with heights below the third percentile, but there was no similar correlation with weights. Of the 10 patients who had radiographs for bone age, all showed severe delay (47 to 103 months) and had heights below the third percentile. Nine of these patients were anemic, but the severity of the anemia did not correlate with the delay in bone age. Neither microcytic anemia nor pernicious anemia, noted in many adult hypothyroid patients, was found in the children and adolescents with hypothyroidism studied here. The "uncomplicated" anemia secondary to hypothyroidism responded to thyroid replacement therapy alone. Anemia can be the most prominent feature of hypothyroidism. In patients with mild to moderate anemia of unknown origin, especially those with fall-off in linear growth and increased MCV, hypothyroidism should be considered in the differential diagnosis of the anemia.

Chronic idiopathic thrombocytopenic purpura followed by chronic myelocytic leukemia.

An 8-year old girl was diagnosed as having idiopathic thrombocytopenic purpura which later became refractory to steroids, splenectomy, and immunosuppressive therapy. While she was being treated for pneumococcal meningitis, 7 1/2 years later, she was found to have Philadelphia-chromosome-positive chronic myelocytic leukemia. She died 2 1/2 years after the diagnosis of leukemia. The association of autoimmune disorders and hematologic malignancies has been well-recognized. To our knowledge, there has been no previous report of chronic idiopathic thrombocytopenic purpura preceding the development of chronic myelocytic leukemia in the literature.

Simple sedimentation method for cytology of cerebrospinal fluid.

A simpler and more practical sedimentation method for cerebrospinal fluid cytology is described. One-tenth milliliter of cerebrospinal fluid was applied dropwise within the circle of a Reich counting slide, and after sedimentation of the cells to the glass, the fluid was removed. Slides can be stained automatically with a slide stainer. Less cell distortion was observed with the sedimentation method. This method is much simpler and less time-consuming than the original sedimentation method.
